Surgical ventricular restoration versus cardiac transplantation: a comparison of cost, outcomes, and survival.

BACKGROUND: Cardiac transplantation is the accepted standard treatment for end-stage heart disease but is donor limited. Surgical ventricular remodeling is an established treatment for patients with ischemic cardiomyopathy. We sought to compare charges, outcomes, and survival in patients undergoing surgical ventricular restoration (SVR) versus cardiac transplantation (CTx). METHODS AND RESULTS: We retrospectively analyzed hospital charges, length of stay (LOS), and survival for 69 SVR and 53 CTx patients at our institution between January 2002 and June 2005. We also compared New York Heart As…

Simvastatin prevents coronary microvascular remodeling in renovascular hypertensive pigs.

Patients with hypertension and chronic kidney disease are at risk for cardiovascular diseases, possibly related to inflammation. Statins have beneficial anti-inflammatory effects on vascular structure regardless of cholesterol reduction. It was hypothesized that alterations in myocardial microvascular structure in swine renovascular hypertension (RVH) would be improved by simvastatin treatment. Three groups of pigs were studied after 12 wk: normal (n = 7), RVH (n = 7), or RVH+simvastatin (RVH+S; 80 mg/d; n = 6). Left ventricular muscle mass and myocardial perfusion were determined in vivo usin

Mineralocorticoid signaling in transition to heart failure with normal ejection fraction.

Heart failure with normal ejection fraction occurs in elderly patients with hypertensive heart disease. We hypothesized that, in such patients, mineralocorticoid receptor activation accelerates the types of ventricular and vascular remodeling and dysfunction believed important in the transition to heart failure. We tested this hypothesis by administering deoxycorticosterone acetate (DOCA) without salt loading or nephrectomy to elderly dogs with experimental hypertension. Elderly dogs were made hypertensive by renal wrapping. After 5 weeks, dogs were randomly assigned to DOCA (1 mg/kg per day I

The hypoxia-inducible factor HIF-1 promotes intramyocardial expression of VEGF in infants with congenital cardiac defects.

OBJECTIVES: The response to hypoxia is primarily mediated by the transcription factor hypoxia-inducible factor-1 (HIF-1) which leads to the induction of a variety of adaptive gene products including vascular endothelial growth factor (VEGF) and endothelial nitric oxide synthase (eNOS). This study was designed to test the hypothesis that HIF-1 and its target genes would be upregulated in the ventricular myocardium of infants with cyanotic congenital cardiac defects. METHODS: 14 infants with cyanotic (n = 7) or acyanotic cardiac defects (n = 7) were investigated. Samples from the right ventricu

Cardioprotective mechanisms of eplerenone on cardiac performance and remodeling in failing rat hearts.

Aldosterone may play a pivotal role in the pathophysiology of heart failure. To elucidate the beneficial cardioprotective mechanism of eplerenone, a novel selective aldosterone blocker, we hypothesized that eplerenone stimulates endothelial NO synthase (eNOS) through Akt and inhibits inducible NO synthase (iNOS) via nuclear factor kappaB (NF-kappaB) after the development of oxidative stress and activation of the lectin-like, oxidized, low-density lipoprotein receptor 1 (LOX-1) pathway in Dahl salt-sensitive rats with heart failure. Eplerenone (10, 30, and 100 mg/kg per day) was given from the

Isoproterenol-induced cardiac hypertrophy: mechanistic insights, environmental stressor and molecular cardioprotection.

Isoproterenol-induced cardiac hypertrophy is a well-established experimental model that mimics environmental stress exposure and the pathological consequences of chronic sympathetic overstimulation on the heart. This model suggests critical understanding in to molecular and cellular mechanisms underlying maladaptive cardiac remodeling, a precursor to heart failure. At the mechanistic level, isoproterenol-induced cardiac hypertrophy is driven by multiple interconnected signaling pathways and triggered by sustained mechanical load, neurohormonal stimulation, inflammatory responses and environmen

Elevated levels of Letm1 drives mitochondrial dysfunction and cardiomyocyte stress-mediated apoptosis in cultured cardiomyocytes.

BACKGROUND: Cardiac ischemia, a predominant cause of heart failure, is marked by profound mitochondrial dysfunction, dysregulated ion homeostasis, and maladaptive cellular remodeling, all of which compromise cardiac performance. The mitochondrial inner membrane protein Leucine zipper-EF-hand containing Transmembrane Protein 1 (Letm1), implicated in Wolf-Hirschhorn Syndrome, is essential for mitochondrial function. Although genetic alterations in Letm1 are linked to cardiomyopathies, its specific contributions to cardiac pathophysiology, particularly in the context of ischemic heart disease, re

Role of insulin signaling dysregulation in pulmonary vascular remodeling in rats with monocrotaline-induced pulmonary arterial hypertension.

BACKGROUND: Pulmonary arterial hypertension (PAH) is a severe disease marked by the remodeling of arteries due to the abnormal growth of vascular cells, including pulmonary arterial smooth muscle cells (PASMCs). The insulin receptor substrate-1 (IRS-1) plays a crucial role in the insulin signaling pathway; however, its function in PAH is still not fully understood. The objective of this research was to explore the role of the protein kinase C (PKC)/IRS-1/ERK signaling pathway in the progression of PAH and its influence on the proliferation and migration of PASMCs. METHODS: To establish the PA

Hypersplenism: an independent risk factor for myocardial remodeling in chronic heart failure patients.

BACKGROUND: During the progression of chronic heart failure (CHF), decreased cardiac functioning is often associated with congestion in the inferior vena vein, which in turn induces splenomegaly and subsequent hypersplenism. Hypersplenism has been shown to exacerbate endothelial dysfunction and adverse cardiac remodeling in HF mice. However, it is unknown whether this effect also occurs in CHF patients with hypersplenism. Here, we compared different patterns of myocardial remodeling between patients with and without hypersplenism. METHODS: 33 CHF patients with hypersplenism were selected and

Electromechanical factors associated with favourable outcome in cardiac resynchronization therapy.

AIMS: Electromechanical coupling in patients receiving cardiac resynchronization therapy (CRT) is not fully understood. Our aim was to determine the best combination of electrical and mechanical substrates associated with effective CRT. METHODS AND RESULTS: Sixty-two patients were prospectively enrolled from two centres. Patients underwent 12-lead electrocardiogram (ECG), cardiovascular magnetic resonance (CMR), echocardiography, and anatomo-electromechanical mapping (AEMM). Remodelling was measured as the end-systolic volume (ΔESV) decrease at 6 months. CRT was defined effective with ΔESV ≤

Feasibility of high-intensity interval training in patients with left ventricular assist devices: a pilot study.

AIMS: Patients with left ventricular assist device (LVAD) suffer from persistent exercise limitation despite improvement of their heart failure syndrome. Exercise training (ET) programmes to improve aerobic capacity have shown modest efficacy. High-intensity interval training (HIIT), as an alternative to moderate continuous training, has not been systematically tested in this population. We examine the feasibility of a short, personalized HIIT programme in patients with LVAD and describe its effects on aerobic capacity and left ventricular remodelling. METHODS AND RESULTS: Patients on durable

Time based versus strain based myocardial performance indices in hypertrophic cardiomyopathy, the merging role of left atrial strain.

AIMS: The myocardial performance index (MPI) is a time-based index of global myocardial performance. In this study, we sought to compare the prognostic value of the MPI with other strain and remodelling indices in hypertrophic cardiomyopathy (HCM). METHODS AND RESULTS: We enrolled 126 patients with HCM and 50 age- and sex-matched controls. Along with traditional echocardiographic assessment, MPI, left ventricular global longitudinal strain (LVGLS), E/e' ratio, and total left atrial (LA) global strain (LAS) were also measured. Time-based MPI was calculated from flow or tissue-based pulse wave

Impact of increased transmitral gradients after undersized annuloplasty for chronic ischemic mitral regurgitation.

BACKGROUND: Recent studies have demonstrated that undersized ring mitral annuloplasty (URMA) for chronic ischemic mitral regurgitation (CIMR) can induce iatrogenic mitral stenosis. The impact of this functional mitral stenosis on clinical and echocardiographic results is not well established. METHODS: 125 consecutive URMA for CIMR were dichotomized according to postoperative mean trans-mitral gradient (Δp) into Group A (61 patients, >5 mm Hg) and Group B (64 patients, ≤5 mm Hg). Echocardiographic, clinical and functional outcomes were prospectively recorded and compared. RESULTS: There were

Machine-learning approach on echocardiography to improve the detection of transthyretin amyloid cardiomyopathy: GRAAL algorithm.

AIMS: Transthyretin amyloid cardiomyopathy (ATTR-CM) is an increasingly recognized cause of heart failure, yet detection remains challenging due to its echocardiographic similarities with age- and hypertension-related cardiac remodelling. METHODS AND RESULTS: We retrospectively included 260 patients (76.5 ± 12.9 years old, 59.6% male) referred for suspected ATTR-CM. A supervised machine-learning diagnosis algorithm differentiating patients with (n = 111) and without (n = 149) ATTR-CM based on echocardiographic data, and subsequently validated in an external multicentre cohort of 454 patients

Rationale and design of a randomized, sham-controlled, clinical trial for evaluation of the Edwards APTURE transcatheter shunt system (ALT-FLOW II).

AIMS: Heart failure (HF) with preserved (HFpEF) and mildly reduced (HFmrEF) ejection fraction are prevalent, yet frequently underdiagnosed syndromes associated with high morbidity and mortality, impaired quality of life, and escalating healthcare expenditures. Although guideline-directed medications like SGLT2 inhibitors have been shown to reduce the risk of hospitalization and cardiovascular death, their use remains limited and substantial residual risk persists. This trial aims to evaluate the safety and performance of a novel left-atrium-to-coronary-sinus shunt (Edwards APTURE transcatheter

Variations of coronary sinus tributaries among patients undergoing cardiac resynchronisation therapy.

BACKGROUND: In cardiac resynchronisation therapy (CRT), the coronary venous system is used for left ventricular pacing electrode placement. Despite the well- -known anatomy of the coronary sinus and its tributaries, heart failure patients' remodelled and enlarged left ventricles may impede the successful lead placement because of acquired anatomical obstacles. MATERIALS AND METHODS: Fifty-five patients qualified for CRT treatment were divided into ischaemic and non-ischaemic cardiomyopathy. Forty-four control groups without heart failure underwent dual-source computed tomography (CT). Rendere

Predicting the clinical course in hypertrophic cardiomyopathy using thallium-201 myocardial scintigraphy.

AIMS: This study aimed to evaluate the changes in left ventricular remodelling with time in patients with hypertrophic cardiomyopathy (HCM) using thallium-201 myocardial scintigraphy. METHODS AND RESULTS: Forty-eight patients with HCM participated in the study. The extent score (ES) and a newly devised index termed the 'mean count change' (MCC) were used to evaluate the myocardial perfusion defects. Using the amount of thallium-201 uptake (TU), MCC (%) was calculated using the following formula: (last TU - initial TU)∕initial TU × 100. To confirm the site of the lesion, the left ventricle was
