Efficacy and safety of romiplostim with horse anti-thymocyte globulin and cyclosporine in acquired aplastic anemia.

This study evaluated the efficacy and safety of high-dose romiplostim (ROMI) combined with horse anti-thymocyte globulin (hATG) and cyclosporine A (CyA) as first-line immunosuppressive therapy for aplastic anemia (AA). We retrospectively analyzed eight patients who received hATG + CyA + ROMI at a single institution between October 2023 and January 2025. ROMI was initiated at 10 μg/kg/week and escalated to 20 μg/kg. Hematologic responses were evaluated at weeks 14 and 27. At week 27, the overall response rate was 100% and the complete response rate was 85.7%. Only one episode of grade ≥ 3 anaph…

Maternal anemia at admission for labor in twin pregnancies: an indicator of adverse maternal and neonatal outcome.

PURPOSE: Maternal anemia during pregnancy is associated with adverse obstetrical outcomes. This study aimed to assess maternal and neonatal outcomes in women with anemia in twin pregnancies, compared to women with normal hemoglobin levels. METHODS: A multicenter retrospective cohort study was conducted including women with twin pregnancies who delivered at 24-42 weeks between 2005 and 2021. Maternal and neonatal outcomes were compared between those who had diagnosis of maternal anemia upon admission for labor (hemoglobin < 11 g/dL), to those who have normal hemoglobin level. The primary outco…

KDOQI US Commentary on the KDIGO 2026 Clinical Practice Guideline for the Management of Anemia in CKD.

The prevalence of anemia is high in people with chronic kidney disease (CKD) and increases as the disease advances. The Kidney Disease Outcomes Quality Initiative (KDOQI) convened a work group to review the Kidney Disease: Improving Global Outcomes (KDIGO) 2026 clinical practice guideline for the management of anemia in CKD. The previous KDIGO anemia guideline was published in 2012; in 2019 and 2021 KDIGO convened conferences addressing controversies in iron management and hypoxia-inducible factor-prolyl hydroxylase inhibitor (HIF-PHI) use, respectively. The KDOQI work group provides perspecti…

Association of growth differentiation factor-15 with muscle wasting and anemia in chronic kidney disease.

BACKGROUND: Cachexia, characterized by muscle wasting and anemia, is a major complication of chronic kidney disease (CKD). Growth differentiation factor-15 (GDF-15), a stress-responsive cytokine implicated in cancer cachexia, is markedly elevated in CKD and linked to disease progression. However, the role of GDF-15 in CKD-related cachexia remains unclear. METHODS: We conducted a cross-sectional study of 268 patients with stage 3-5 CKD not yet on dialysis. Plasma GDF-15 concentrations were measured using immunoassays. Lean body mass was assessed by multifrequency bioimpedance spectroscopy with…

Serum erythropoietin level in cases of iron deficiency anemia with thrombocytosis: A cross-sectional study.

BACKGROUND: Iron deficiency anemia (IDA) is the most prevalent nutritional anemia worldwide and is frequently associated with reactive thrombocytosis. The physiological response to anemia includes increased production of erythropoietin (EPO); however, its role in the development of thrombocytosis in IDA remains unclear and controversial. AIM: To evaluate serum erythropoietin levels in patients with iron deficiency anemia associated with thrombocytosis and to assess the relationship between hemoglobin concentration, platelet count, and serum erythropoietin levels. MATERIALS AND METHODS: This …

Refeeding Syndrome May Increase the Risk of Anemia of Prematurity: Is Early Enteral Nutrition the Solution?

BACKGROUND/OBJECTIVES: Preterm infants are particularly vulnerable to nutritional deficiencies and electrolyte imbalances during the early s … ecreases their stability. It is therefore hypothesized that RS may contribute to the development of anemia of prematurity (AOP). At the same time, early enteral nutrition (EN) may promote metabolic adaptation and reduce exposure to the complications of prolonged parenteral support, potentially protecting against AOP. The primary aim of this study was to …

Severe aplastic anemia as a rare autoimmune complication of ankylosing spondylitis: A rare case report.

RATIONALE: Aplastic anemia (AA) is a rare, life-threatening bone marrow failure syndrome, most often driven by immune-mediated destruction of hematopoietic stem and progenitor cells. Ankylosing spondylitis (AS) is a chronic inflammatory spondyloarthropathy characterized by aberrant T-cell activity and elevated pro-inflammatory cytokines. Although both diseases share immune dysregulation as a common pathogenic feature, their coexistence has not been previously documented in the literature. PATIENT CONCERNS: A 42-year-old male with a known history of AS presented with recurrent mucocutaneous bl…

Inherited microcytic anemias due to disorders of iron and heme metabolism: An updated clinical review.

Microcytic anemia is among the most common hematological abnormalities in clinical practice and is usually attributable to iron deficiency, thalassemia traits, or anemia of inflammation. A small but clinically important subset of patients, however, has inherited disorders of iron metabolism or heme synthesis presenting with persistent, unexplained, familial, or iron-refractory microcytosis. Over the past decade, advances in the understanding of the hepcidin-ferroportin axis, erythroid iron regulation, standardized hepcidin assays, and next-generation sequencing have substantially refined diagn…

An Updated Review on Iron Deficiency Anemia: From Epidemiology to Diagnosis, Etiology, Prevention and Treatment.

Iron deficiency anemia (IDA) accounts for two-thirds of all anemias worldwide, affecting around 1.2 billion people globally. For the diagnosis of iron deficiency, a bone marrow aspiration sample study is considered the gold standard. Individuals with IDA may experience fatigue, weakness, poor productivity, tachycardia, dyspnea, headaches, tinnitus, vertigo, skin pallor, cheilosis and koilonychia. The most common causes of IDA are deficiency of iron (caused by low iron intake, impaired iron absorption, increased iron requirements or chronic blood loss), helicobacter infection and hookworm infec…

Prevalence and Determinants of Anemia Among Women of Reproductive Age and Children Under Five in Tajikistan.

Introduction: Anemia is a global public health concern affecting mainly women of reproductive age (WRA) and preschoolers (PSC, 6-59 months) due to their higher demand for iron. The etiology of anemia is multifactorial, and nutritional anemia is the most common type worldwide and is predominantly due to deficiencies in iron, vitamin B12, and folate. This study aimed to assess the associated factors, etiology, and prevalence of anemia among WRA and preschoolers in the Gorno-Badakhshan Autonomous Oblast (GBAO) region in Tajikistan. Methods: We conducted a cross-sectional study in the GBAO region …

Autoantibodies to Extracellular Erythrocyte Band 3 Epitopes Are Associated With Anemia in Plasmodium vivax Infection.

Plasmodium vivax-associated anemia is a multifactorial clinical outcome, in which the destruction of uninfected red blood cells (uRBCs) plays a major role in disease development and severity. Here, we identified autoantibody targets within the extracellular loops of Band 3 (B3), a major erythrocyte transmembrane protein. In silico epitope prediction identified six of the seven potentially immunogenic extracellular domains of B3. Patients with P. vivax infection who were anemic (n = 79) and non-anemic (n = 95), and 40 healthy donors from a non-endemic area were included as negative controls. In…

Umbilical cord blood infusion in the treatment of aplastic anemia: A single center prospective study.

The current standard first-line therapy for aplastic anemia (AA) consists of antithymocyte globulin (ATG), cyclosporine, and thrombopoietin receptor agonists (TPO-RAs). A subset of patients exhibit treatment intolerance or develop refractory/relapsed disease, for whom treatment options are limited. Umbilical cord blood exhibits immunomodulatory effects, regulates the bone marrow microenvironment, and promotes hematopoietic reconstitution, thereby demonstrating therapeutic potential for AA. In this prospective clinical study, cord blood infusion was administered in combination with cyclosporine…
