Anterior Quadratus Lumborum Block with Liposomal Bupivacaine versus Ropivacaine for Postoperative Recovery in Laparoscopic Colorectal Surgery: A Randomized Controlled Trial.

BACKGROUND: The clinical utility of liposomal bupivacaine (LB) in fascial plane blocks remains controversial, with many trials focusing on early postoperative outcomes. We investigated whether LB in quadratus lumborum block (QLB) provides an extended recovery benefit following laparoscopic colorectal surgery. METHODS: In this randomized, double-blind, controlled, superiority trial, 120 patients undergoing laparoscopic colorectal surgery between May 12, 2025, and December 25, 2025, were allocated to receive ultrasound-guided QLB-3 with either 1.3% LB (266 mg) or 0.33% ropivacaine (132 mg). Extrafascial out-of-plane continuous interscalene block using the C6 transverse process as a bony landmark -A case series.

BACKGROUND: Background: Extrafascial interscalene block has been proposed to reduce phrenic nerve involvement while maintaining analgesic efficacy for shoulder surgery. However, a reproducible continuous catheter technique using a clear anatomical landmark has not been well described. CASE: We describe a case series of six patients undergoing shoulder surgery who received a continuous extrafascial interscalene block using the C6 transverse process as a bony landmark. Under ultrasound guidance, an 18-gauge Tuohy needle was advanced in a cranial-to-caudal out-of-plane trajectory toward the post

Effects of Ropivacaine-Based Thoracic Paravertebral Block versus Erector Spinae Plane Block on Quality of Recovery in Laparoscopic Colorectal Cancer Surgery: A Randomized Controlled Trial.

BACKGROUND: Colorectal cancer remains a leading cause of cancer-related mortality worldwide. Postoperative pain can impair recovery. Thoracic paravertebral block (TPVB) and erector spinae plane block (ESPB) are regional analgesic techniques that may enhance recovery, but their comparative efficacy in colorectal surgery is not well established. This study aimed to compare the effects of TPVB and ESPB on the quality of recovery in patients undergoing colorectal cancer surgery. METHODS: In this single-center, single-blind, randomized controlled trial, 87 patients scheduled for elective laparosco

Activation of mitophagy by lidocaine limits mROS and prevents microglial M1 polarization to alleviate postoperative delirium.

OBJECTIVE: Postoperative delirium (POD) is a common complication in elderly patients, yet effective interventions remain limited. Mitochondrial dysfunction and microglial M1 polarization contribute to POD pathogenesis, but the underlying mechanisms are incompletely understood. This study aimed to investigate whether lidocaine (LID) ameliorates POD by activating mitophagy to suppress mitochondrial reactive oxygen species (mROS) and inhibit M1 microglial polarization. METHODS: A mouse model of POD was established by laparotomy in male C57BL/6 J mice, which were randomly divided into four groups
