Effectiveness of an antimicrobial treatment scheme in a confined glanders outbreak.

BACKGROUND: Glanders is a contagious and fatal zoonotic disease of solipeds caused by the Gram-negative bacterium Burkholderia (B.) mallei. Although regulations call for culling of diseased animals, certain situations e.g. wild life conservation, highly valuable breeding stock, could benefit from effective treatment schemes and post-exposure prophylaxis. RESULTS: Twenty three culture positive glanderous horses were successfully treated during a confined outbreak by applying a treatment protocol of 12 weeks duration based on the parenteral administration of enrofloxacin and trimethoprim plus s

Screening for group B streptococcus: a private laboratory experience.

We examined group B streptococcus (GBS) isolates colonizing women at the 35-37 weeks of pregnancy. A total of 257 group B streptococcus (GBS) isolates for serotyped using direct agglutination with a set of commercially available antisera (Ia, Ib, II, III, IV, and V) and tested for susceptibility to antimicrobials (penicillin, macrolides, lincosamides, fluoroquinolones and tetracyclines). Fourteen isolates could not be serotyped with the antisera set used in the study. Serotype III was the predominant serotype (33%), followed by serotypes V (23%), and Ia (20%). Whereas all isolates were suscept

Protective effects of reconstituted high-density lipoprotein in rabbit gram-negative bacteremia models.

Reconstituted high-density lipoproteins (rHDLs) have the ability to bind bacterial lipopolysaccharide and to reduce its endotoxin activity in vitro and in vivo. The aim of the present studies was to investigate the therapeutic potential of rHDL in bacteremia models. Gram-negative sepsis was induced in anesthetized rabbits by intravenous infusion of Escherichia coli organisms (4 x 10(9) CFU/kg infused over 2 hours) and treated with appropriate antibiotics. rHDL or placebo was infused either before (prophylaxis) or 1 hour after (therapy) the beginning of the bacterial challenge. In the control g

Invasive fungal disease in patients with end-stage liver disease.

Invasive fungal disease (IFD) has become a critical complication in patients with end-stage liver disease (ESLD), marked by a rising prevalence and the emergence of drug-resistant strains in recent decades. The heightened susceptibility to IFD in ESLD stems from liver-related immune dysfunction, structural changes, and iatrogenic factors, such as invasive procedures, broad-spectrum antibiotics, and immunosuppressive therapies. Timely diagnosis is essential for effective management, yet it remains challenging. This review examines both conventional culture-based and advanced culture-independent

A review on fungal surgical site infections: epidemiology, risk factors, main fungal agents, and prevention.

Fungal surgical site infections (SSIs) may be less common than bacterial SSIs but are a significant clinical issue due to their challenging diagnosis, higher morbidity, and rising incidence, particularly in immunocompromised patients. The epidemiology, risk factors, prevalent fungal pathogens, and prevention of SSIs caused by fungi are discussed in this narrative review. Systematic literature search for the period 2000 to 2024 was conducted on top databases using relevant MeSH keywords. The most frequent solitary pathogens were Candida spp., followed by Aspergillus and Mucor spp., especially i

Use of Azithromycin in Pregnancy: More Doubts than Certainties.

Macrolides such as azithromycin are commonly prescribed antibiotics during pregnancy. The good oral bioavailability and transplacental transfer of azithromycin make this drug suitable for the treatment of sexually transmitted diseases, toxoplasmosis, and malaria. Moreover, azithromycin is useful both in the management of preterm pre-labor rupture of membranes and in the adjunctive prophylaxis for cesarean delivery. The aim of this comprehensive narrative review is to critically analyze and summarize the available literature on the main aspects of azithromycin use in pregnant women, with a spec

Performance Measurement and Quality Improvement Initiatives for Bladder Cancer Care.

PURPOSE OF REVIEW: Bladder cancer care is costly due to long surveillance periods for non-muscle-invasive bladder cancer (NMIBC) and comorbidities associated with the surgical treatment of muscle-invasive bladder cancer (MIBC). We reviewed current evidence-based practices and propose quality metrics for NMIBC and MIBC. RECENT FINDINGS: For patients with NMIBC, we propose four categories of candidate quality metrics: (1) appropriate use of imaging, (2) re-staging transurethral resection of bladder tumor, (3) perioperative intravesical chemotherapy, and (4) induction and maintenance BCG in high

Anthrax prophylaxis: recent advances and future directions.

Anthrax is a serious, potentially fatal disease that can present in four distinct clinical patterns depending on the route of infection (cutaneous, gastrointestinal, pneumonic, or injectional); effective strategies for prophylaxis and therapy are therefore required. This review addresses the complex mechanisms of pathogenesis employed by the bacterium and describes how, as understanding of these has developed over many years, so too have current strategies for vaccination and therapy. It covers the clinical and veterinary use of live attenuated strains of anthrax and the subsequent identificat

[Monitoring of side effects and estimation of cycloferon efficacy in treatment of children with frequent and prolonged diseases].

Two hundred fifty patients, including 100 children with frequent and prolonged diseases at the age of 4 to 7 years, 76 children at the age of 7 to 18 years and 74 subjects at the age of 22 to 57 years were observed. The patients were treated with cycloferon in two courses with a 2-week interval according to the standard scheme. The tonsil surface microflora and its susceptibility to antibiotics were determined. Cycloferon lowered the Staphylococcus aureus titre and increased the culture susceptibility to benzylpenicillin, oxacillin, rifampicin, and erythromycin, reducing the variety of the fau

[Spontaneous bacterial peritonitis, a severe complication in patients with liver cirrhosis].

Three patients with hepatic cirrhosis and ascites, a 65-year-old man, a 17-year-old woman and a 49-year-old man, were admitted to hospital for progressive drowsiness, increased ascites, and melaena, respectively. An elevated number of polymorphonuclear leukocytes was found in the ascites. The three patients became more and more seriously ill. On the basis of the laboratory findings, a diagnosis of 'spontaneous bacterial peritonitis' was made. The patients recovered after administration of antibiotics. The signs and symptoms of spontaneous bacterial peritonitis can range from subtle, renal dysf

Primary pseudomonas meningitis in an adult, splenectomized, multitransfused thalassaemia major patient.

A 19-year-old splenectomized, multitransfused female patient with beta-thalassaemia major developed primary meningitis due to P. putida. Her blood cultures were negative. P. putida is an unusual nosocomial organism to cause primary meningitis. Infection due to this organism carries high mortality. However, owing to early diagnosis and energetic treatment this patient survived without any sequelae. A review of serious infections over the last 7 years in patients in our thalassaemia care centre revealed 11 serious infections among our splenectomized patients (n = 46) and none in the non-splenect

Recurrent penicillin-resistant pneumococcal sepsis after matched unrelated donor (MUD) transplantation for refractory T cell lymphoma.

Patients who undergo splenectomy and recipients of allogeneic marrow (alloBMT) or peripheral stem cell transplantation are at increased risk of overwhelming infection from encapsulated organisms such as Streptococcus pneumoniae, Haemophilus influenzae and Neisseria meningitidis. As prophylaxis against these pathogens splenectomised patients are immunised and may also receive antibiotics for life. We report relapsing overwhelming sepsis caused by penicillin-resistant pneumococcus in a patient who was immunised and received prophylactic phenoxymethylpenicillin for 8 months following splenectomy

Efficacy of an immune modulator in enzootic pneumonia of dairy calves.

The objective of this cohort study was the evaluation of the efficacy of an immune modulator in the prophylaxis, metaphylaxis and treatment of enzootic bronchopneumonia (EBP). A total of 552 dairy calves on 13 farms were included. Groups of calves were included in the study if the prevalence of respiratory disease on the first visit was at least 50%, and the number of animals aged 2-16 weeks was at least 10. Controls were treated with a placebo. Animals of both groups were treated antibiotically if body temperature exceeded 39.5 degrees C. A clinimetric system was devised and used for classify

[Ansatipine: only preventive treatment available for opportunistic MAI infections in AIDS. From an interview with Dr. Agnes Mignon].

Ansatipine, the international name of which is rifabutine, is a new specialty that was licensed in February 1993 and marketed in France in June 1993. It is an antibiotic of the rifamycin family, the result of research by Farmitalia Carlo Erba and is now presented by the Laboratoire Pharmacia. Today, rifabutine is the only molecule in the world that has been shown to be efficacious in preventative treatment for infections by the Mycobacterium avium complex (MAC) and to be officially recognised as such. Rifabutine (Ansatipine) was developed for prophylaxis of infections by MAC, because of its ba

Short-term cephalosporin prophylaxis in transurethral surgery.

The effects of three short-term prophylactic cephalosporin regimens were compared in a prospective, randomized, double-blind study. One hundred sixteen men, with sterile urine preoperatively, underwent transurethral surgery (resection of the prostate or bladder neck, 73; resection or fulguration of bladder tumors, 38; or internal urethrotomy, 5). One group received 1 gm of cefotaxime parenterally 30 to 90 minutes preoperatively, every 60 to 90 minutes during surgery, and immediately postoperatively. In the other two groups, 1 gm of cefotaxime or cefazolin was administered according to the regi

Gas gangrene and related infection: classification, clinical features and aetiology, management and mortality. A report of 88 cases.

The clinical features of gas gangrene and related infection seen in 88 patients over a 10-year period are described. It is suggested that clostridial infection could be simply classified as either 'gas-forming' or 'non-gas-forming'. The gas-forming group represents the more severe form of infection. Non-clostridial gas gangrene may present in a variety of forms. The anaerobic streptococcus was the organism most frequently responsible, but these cases were indistinguishable from clostridial infection on clinical grounds. The treatment of gas gangrene in this series of patients is reported. Emph

Antibiotics for preventing meningococcal infections.

BACKGROUND: Meningococcal disease is a contagious bacterial infection caused by Neisseria meningitidis (N. meningitidis). Household contacts have the highest risk of contracting the disease during the first week of a case being detected. Prophylaxis is considered for close contacts of people with a meningococcal infection and populations with known high carriage rates. OBJECTIVES: To study the effectiveness, adverse events and development of drug resistance of different antibiotics as prophylactic treatment regimens for meningococcal infection. SEARCH METHODS: We searched CENTRAL 2013, Issue
