A double-blind, placebo-controlled, phase II, randomized study of lovastatin therapy in the treatment of mildly active rheumatoid arthritis.

OBJECTIVES: 3-hydroxy-3-methylglutaryl coenzyme-A (HMG Co-A) reductase inhibitors (statins) are standard treatment for hyperlipidaemia. In addition to lipid-lowering abilities, statins exhibit multiple anti-inflammatory effects. The objectives of this study were to determine whether treatment of patients with RA with lovastatin decreased CRP or reduced disease activity. METHODS: We conducted a randomized double-blind placebo-controlled 12 week trial of lovastatin vs placebo in 64 RA patients with mild clinical disease activity but an elevated CRP. The primary efficacy end point was the reduct

Comparison of Different Remission and Low Disease Definitions in Psoriatic Arthritis and Evaluation of Their Prognostic Value.

OBJECTIVE: There is no agreement on the optimal definitions for assessing disease state in patients with psoriatic arthritis (PsA), and some of the commonly used definitions do not include assessment of skin lesions. We investigated the performance of various definitions in patients with PsA and psoriasis. METHODS: This was a posthoc analysis of data from the PRESTA study. The remission definitions analyzed were very low disease activity (VLDA) index, defined as 7/7 of the minimal disease activity (MDA) cutoffs; Disease Activity Index for PsA (DAPSA); and clinical (c-) DAPSA. The low disease

Inflammation as a Risk of Developing Chronic Kidney Disease in Rheumatoid Arthritis.

OBJECTIVE: The relationship between chronic inflammation and the incidence of chronic kidney disease (CKD) remained not-clear in patients with rheumatoid arthritis (RA). This study aims to examine the relationship between persistently high C-reactive protein (CRP), a marker of inflammation, and the incidence of CKD in RA. METHODS: We retrospectively examined the relationship between the levels of CRP and incidence of CKD in 345 RA patients. The outcome of interest was incidence of CKD, defined as an estimated glomerular filtration rate (eGFR) <60 mL/min/1.73 m2 and/or positive dipstick testin

Factors Contributing to Discordance between the 2011 ACR/EULAR Criteria and Physician Clinical Judgment for the Identification of Remission in Patients with Rheumatoid Arthritis.

Remission is a primary end point of in clinical practice and trials of treatments for rheumatoid arthritis (RA). The 2011 American College of Rheumatology/European League Against Rheumatism (ACR/EULAR) remission criteria were developed to provide a consensus definition of remission. This study aimed to assess the concordance between the new remission criteria and the physician's clinical judgment of remission and also to identify factors that affect the discordance between these two approaches. A total of 3,209 patients with RA were included from the KORean Observational Study Network for Arth

The autoimmune-associated genetic variant PTPN22 R620W enhances neutrophil activation and function in patients with rheumatoid arthritis and healthy individuals.

OBJECTIVES: A genetic variant of the leukocyte phosphatase PTPN22 (R620W) is strongly associated with autoimmune diseases including rheumatoid arthritis (RA). Functional studies on the variant have focussed on lymphocytes, but it is most highly expressed in neutrophils. We have investigated the effects of the variant on neutrophil function in health and in patients with RA. METHODS: Healthy individuals and patients with RA were genotyped for PTPN22 (R620W) and neutrophils isolated from peripheral blood. Neutrophil adhesion and migration across inflamed endothelium were measured. Calcium (Ca(2

The impact of DMARD and anti-TNF therapy on functional characterization of short-term T-cell activation in patients with rheumatoid arthritis--a follow-up study.

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by a systemic dysfunction of T-cells. In this study we tested the impact of DMARD and anti-TNF agents on short-term activation characteristics of T-cells. We enrolled 12 patients with newly diagnosed RA (naïve RA) who were treated with methothrexate (MTX) and glucocorticsteroid (GCS) and 22 patients with established RA non responding to conventional DMARD therapy who were treated with different anti-TNF agents. Nine healthy volunteers served as controls. Blood samples were taken at baseline, then at 4th and 8th week of the

Associations between serum anti-CCP antibody, rheumatoid factor levels and HLA-DR4 expression in Hungarian patients with rheumatoid arthritis.

BACKGROUND: The presence of anti-cyclic citrullinated peptide autoantibody is highly specific for rheumatoid arthritis. Certain HLA-DR4 (HLA-DRB1*04) alleles, also known as the "shared epitope," are associated with increased susceptibility to RA. In addition, these alleles may also have relevance for disease outcome. Anti-CCP antibody positivity has been associated with the presence of HLA-DR4 alleles in patients with RA. However, there is little information regarding a relationship between quantitative anti-CCP production (serum anti-CCP concentrations) and the shared epitope. OBJECTIVES: To

Type I collagen degradation product in serum of patients with early rheumatoid arthritis: relationship to disease activity and radiological progression in a 3-year follow-up.

The new assay of cross-linked carboxyterminal telopeptide of type I collagen (ICTP), a serum marker for bone collagen degradation, was evaluated in serial measurements of 66 patients with early RA during a 3-yr prospective study. Initially 51% of RA patients had elevated levels of serum ICTP compared to healthy controls. During the subsequent months after starting anti-rheumatic treatment, the mean ICTP levels decreased in parallel with the clinical and laboratory variables measuring disease activity. Despite marked clinical improvement with anti-rheumatic treatment, a steady increase in radio

Evidence for B cell activation in patients with active rheumatoid arthritis.

Peripheral blood lymphocytes and in some cases synovial eluate cells from 51 patients with rheumatoid arthritis (RA), were analysed for the percentages of cells bearing surface light chains (total B cells), IgM and IgD. In addition, their capacity to form rosettes with mouse erythrocytes (mRFC)--a property of a B cell subpopulation--was determined. Activity of the disease was assessed by clinical and laboratory criteria and classified as very active, moderately active and inactive. Normal, age and sex matched individuals and a group of patients with a variety of other rheumatological disorders

HL-A histocompatibility antigens and their relation to disease.

Table 14 summarizes the data presented in this review. The table lists the various diseases, along with whatever HL-A-antigen associations may be present, and the ranking that is based on the accumulated confidence of the associations; also, it lists the individual antigens and those disease states in which the incidence of the various antigens is increased ro decreased. The Table should expand rapidly in future years. There are a number of problems involved in this area of research: (1) There is a scarcity and lack of stability of good reagents, as well as the general difficulty of HL-A serol

Smartphone-assisted optical quantification of d-penicillamine in human plasm sample using toluidine blue as efficient chromogenic probe: introducing a novel portable device toward therapeutic drug monitoring (TDM).

d-Penicillamine (DPA) is a crucial pharmacological agent widely employed in the clinical management of Wilson's disease, rheumatoid arthritis, and heavy metal poisoning due to its potent metal-chelating properties. Given the narrow therapeutic index of DPA and the critical need for precise monitoring to avoid systemic toxicity, developing rapid, sensitive, and accessible analytical methods is highly demanded by clinicians. This study introduces a novel, cost-effective, and sensitive optical sensor for DPA quantification using Toluidine Blue (TB) as the efficient chromogenic probe. Using smartp

Preclinical and clinical outcomes in patients with high rheumatoid factor levels treated with TNF inhibitors:a systematic review.

OBJECTIVES: Rheumatoid factor (RF) binds to the fragment crystallisable (Fc) portion of immunoglobulin G1 and can also bind to the Fc region of biologic agents, forming immune complexes that are subsequently cleared by macrophages, contributing to reduced drug availability and diminished therapeutic response. In this systematic review, we examined the binding mechanism and impact of RF on tumour necrosis factor inhibitors (TNFis) with and without an Fc portion, along with the impact of RF levels on drug concentration and clinical outcomes in patients with rheumatoid arthritis (RA). METHODS: S

Curcumin-mediated Modulation of T-bet and CD8+ T Cells: A Potential Anti-inflammatory Mechanism in Knee Osteoarthritis.

Osteoarthritis (OA) is the most common form of arthritis, characterized by pathological changes in joint components. Increasing evidence suggests that helper T (TH) lymphocytes play a pivotal role in the inflammatory processes associated with OA. Curcumin, the primary polyphenolic compound found in Curcuma longa, exhibits potent antioxidant and anti-inflammatory properties. This study aimed to evaluate the effects of curcumin on the gene expression of key transcription factors of TH1 and TH2 cells and to explore their associations with clinical and immunological parameters in patients with kne

Evaluation of Post-operative Serum Biomarkers (D-Dimer, ESR, and CRP) in Patients Following Total Joint Arthroplasty: A Prospective Observational Study.

INTRODUCTION: Total joint arthroplasty, including total hip and knee replacement, is among the most successful orthopedic interventions for end-stage arthritis. However, complications, such as periprosthetic joint infection and venous thromboembolism remain significant causes of morbidity. Serum biomarkers, such as D-dimer, erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP), are commonly used to assess post-operative inflammation and potential complications. Despite extensive literature worldwide, normative post-operative biomarker trends in Indian patients remain poorly define

Cardiovascular risk assessment using the DAS28-GGT score in rheumatoid arthritis patients initiating JAK inhibitors: Results from the MAJIK-SFR registry.

OBJECTIVES: Cardiovascular disease is the leading cause of death in rheumatoid arthritis (RA). By replacing C-reactive protein (CRP) with gamma-glutamyl transferase (GGT), the DAS28-GGT was developed to integrate both inflammatory disease activity and cardiovascular (CV) risk into a single score. We evaluated its ability to classify CV risk in RA patients initiating Janus kinase inhibitors (JAKi) and compared its performance with DAS28-CRP. METHODS: This cross-sectional study used data from the MAJIK-SFR observational registry. RA patients initiating a JAKi with available GGT were included. C
