[Genesis of vestibulo-autonomic disorders in space flight].

Some considerations on the mechanisms of vestibulo-vegetative disturbance in space flight are given on the basis of clinical observations, experience of otoneurological clinic, simulation experiments and examinations of cosmonauts. The starting point in these disturbances was a significant fact of blood redistribution in weightlessness and subsequent changes in hemodynamics, metabolism, etc. The body reactions to weightlessness seem to prepare pronounced vestibulo-vegetative reflexes to threshold and subthreshold vestibular stimuli induced by head and body movements in flight. Some aspects of

Acute Intoxication With Poison Hemlock (Conium maculatum).

Poison hemlock (Conium maculatum) is regarded as one of the most poisonous plants worldwide, and it can easily be misidentified as edible species as celery, parsnip (for their leaves), or carrots (for their roots). Toxicity comes from piperidine alkaloids, which mimic nicotine effect on the autonomous nervous system giving a clinical picture of nicotinic syndrome with an initial excitatory phase and secondary inhibitory one. In Portugal, this plant is used as an ancient technique for river fishing. Death comes from respiratory arrest and hemodynamic collapse; rapid diagnosis and medical suppor

[Gastroenterology and FSS].

Functional gastrointestinal disorders (FGIDs) such as functional dyspepsia (FD) and irritable bowel syndrome (IBS) are included in functional somatic syndromes (FSS), which are designated as medically unexplained physical symptoms. Pathopysiology of these diseases are characterized by multi-functional disorders of the gastrointestinal (GI) tract including psychosomatic sensation, brain-gut interaction, autonomic nervous systems, motility disorders, local inflammation, and visceral perception. However, the standard therapy for these diseases is not established, since predominant factors among t…

Physiologic consequences of pneumonectomy. Consequences on the esophageal function.

Esophageal and upper gastrointestinal dysmotility occur after both pneumonectomy without pulmonary replacement and recipient pneumonectomy f … es, local ischemia, postoperative scarring of the esophagus and mediastinum, and disturbance of the autonomic nervous systems are the major causes of the abnormality. To reduce the incidence of esophageal dysmotility after pneumonectomy, every effort should be made during surgery to prevent direct injury of the esophagus or the vagal nerves.

Hydrogen Sulfide Modulates Astrocytic Toxicity in Mouse Spinal Cord Cultures: Implications for Amyotrophic Lateral Sclerosis.

Hydrogen sulfide (H2S), a known inhibitor of the electron transport chain, is endogenously produced in the periphery as well as in the central nervous system, where is mainly generated by glial cells. It affects, as a cellular signaling molecule, many different biochemical processes. In the central nervous system, depending on its concentration, it can be protective or damaging to neurons. In the study, we have demonstrated, in a primary mouse spinal cord cultures, that it is particularly harmful to motor neurons, is produced by glial cells, and is stimulated by inflammation. However, its role

Electrical Stimulation of the Infraorbital Nerve Induces Diving Reflex in a Dose-Controlled Manner.

The "diving reflex" (DR) is a very powerful autonomic reflex that facilitates survival in hypoxic/anoxic conditions and could trigger multifaceted physiologic effects for the treatment of various diseases by modulating the cardiovascular, respiratory, and nervous systems. The DR can be induced by cold water or noxious gases applied to the anterior nasal mucosa and paranasal regions, which can stimulate trigeminal thermo- or chemo-receptors to send afferent signals to medullary nuclei which mediate the sympathetic and parasympathetic nervous systems. Although promising, these approaches have ye

[Fibromyalgia syndrome].

Nowadays, fibromyalgia syndrome (FMS) should be diagnosed according to established criteria in order to differentiate it from other specified or unspecified pain conditions. Various underlying reasons for pain exist and possible correlations with FMS should be thoroughly discussed with the patient. Recent pathophysiological examinations suggest that fibromyalgia syndrome may constitute a disorder of the central nervous system, especially of the hypothalamus-hypophysis-axis, and/or of the autonomous nervous system and of pain regulating nerves. The most common co-morbidity comprises sleep distu

Cardiovascular effects of aldosterone: insight from adult carriers of mineralocorticoid receptor mutations.

BACKGROUND: High plasma aldosterone has deleterious cardiovascular effects that are independent of blood pressure, but the role of the mineralocorticoid receptor remains unclear. Renal pseudohypoaldosteronism type 1 is a rare autosomal-dominant disease caused by NR3C2 loss-of-function mutations, which is characterized by renal salt loss and compensatory high renin and aldo secretion. We aimed to assess the cardiovascular outcomes in adults carrying NR3C2 mutations. METHODS AND RESULTS: In this case-control study, 39 NR3C2 mutation carriers were compared with sex- and age-paired noncarriers. P

Exploring CNS Involvement in Pain Insensitivity in Hereditary Sensory and Autonomic Neuropathy Type 4: Insights from Tc-99m ECD SPECT Imaging.

Hereditary sensory and autonomic neuropathy type 4 (HSAN4), also known as congenital insensitivity to pain with anhidrosis (CIPA), is a rare genetic disorder caused by NTRK1 gene mutations, affecting nerve growth factor signaling. This study investigates the central nervous system's (CNS) involvement and its relation to pain insensitivity in HSAN4. We present a 15-year-old girl with HSAN4, displaying clinical signs suggestive of CNS impact, including spasticity and a positive Babinski's sign. Using Technetium-99m ethyl cysteinate dimer single-photon emission computed tomography (Tc-99m ECD SPE

Control of postganglionic neurone phenotype by the rat pineal gland.

As neurones develop they are faced with choices as to which genes to express, to match their final phenotype to their role in the nervous system. A number of processes can guide these decisions. Within the autonomic and sensory nervous systems, there are a handful of examples that suggest that one mechanism that may match phenotype to function is the presence of target-derived differentiation factors. We tested whether the rat pineal gland controls the expression of a neuropeptide (neuropeptide Y) and a calcium-binding protein (calbindin) in sympathetic postganglionic neurones that innervate i

[Effects of general anesthetics on neuronal nicotinic acetylcholine receptors and their roles in the mechanism of anesthesia].

Neuronal nicotinic acetylcholine receptors (nAchRs) are widely expressed in the central and autonomic nervous systems and have subunit compositions with biophysical and pharmacological properties distinct from those of the receptors at the neuromuscular junction. They are thought to modulate synaptic transmission in the central nervous system (CNS) mainly by regulating the release of neurotransmitters. Although roles of neuronal nAchRs in the CNS are poorly understood, these receptors are involved in cognitive performance, nociception and psychoneurological disorders such as Alzheimer's and Pa…

Non-invasive auricular vagus nerve stimulation in fibromyalgia: Impacts on autonomic function, central sensitization and pain catastrophizing.

INTRODUCTION: Fibromyalgia is a complex chronic pain disorder frequently associated with autonomic dysregulation, central sensitization, and cognitive-emotional disturbances. Transcutaneous vagus nerve stimulation (tVNS) has recently emerged as a non-invasive neuromodulatory approach with potential to modulate autonomic function, pain perception, and affective processing. We aim to evaluate the short-term effects of a 28-day tVNS protocol on autonomic function, symptom severity, central sensitization, neuropathic-like pain features, and pain catastrophizing in patients with fibromyalgia. Brain-heart autonomic axis across different clinical status and severity of chronic obstructive pulmonary disease.

PURPOSE: Impairment of cardiac autonomic integrity is common in chronic obstructive pulmonary disease (COPD) patients. The influence of the interaction between clinical and severity status on brain-heart autonomic axis (BHAA) is not well known. We aimed to investigate the BHAA function across different clinical status and severity of COPD. METHODS: Cross-sectional study involving 77 COPD patients allocated into four groups according to clinical status [acute exacerbation (GAE) or stable (GST)] and severity [less (-) or more (+)]: 1) GAE-, n = 13; 2) GAE+, n = 20; 3) GST-, n = 23; and 4) GST+,

Exposure to antenatal corticosteroids and reduced respiratory sinus arrhythmia in adult survivors of extremely low birth weight.

Purpose/aim: Antenatal corticosteroid (ACS) therapy has dramatically increased survival rates among extremely low birth weight (ELBW) infants. However, the long-term effects of ACS on autonomic nervous system function have not been explored. Using the world's oldest longitudinally followed cohort of ELBW infants we compared respiratory sinus arrhythmia (RSA) among ELBW survivors whose mothers received ACS (ELBW-S), those who did not (ELBW-NS) and normal birth weight (NBW) controls in their 20 and 30 s. Methods: Resting electrocardiogram (ECG) was recorded from ELBW-S (n = 28), ELBW-NS (n = 36)…

Cardiac autonomic regulation in response to a mixed meal is impaired in obese children and adolescents: the role played by insulin resistance.

CONTEXT: Obesity in children/adolescents has been associated with subtle cardiac abnormalities, including myocardial dysfunction and cardiac autonomic dysregulation at rest, both likely responsible for a higher mortality in adulthood. Food intake induces remarkable adjustments of cardiovascular autonomic activity in healthy subjects. OBJECTIVE: The objective of the study was to evaluate in obese children/adolescents meal-induced cardiac autonomic response and the role played by insulin resistance. DESIGN AND SETTING: Sixty-eight obese and 30 matched normal-weight children/adolescents underwe

HOX11L1: a promoter study to evaluate possible expression defects in intestinal motility disorders.

Intestinal Neuronal Dysplasia (IND) is a congenital disorder characterized by intestinal motility defects associated with hyperplasia of enteric ganglia. A phenotype resembling human IND has been observed in mice knocked-out for a member of the Hox11 homeobox gene family, Hox11l1, suggesting that the human homologue of this gene could be responsible for congenital disorders of intestinal innervation. However, previous mutation analysis of the coding sequence of the HOX11L1 gene in patients affected with IND detected neither mutations nor other nucleotide variants. In the present work, a detail

Synchronous and baroceptor-sensitive oscillations in skin microcirculation: evidence for central autonomic control.

To determine whether skin blood flow is local or takes part in general regulatory mechanisms, we recorded laser-Doppler flowmetry (LDF; left and right index fingers), blood pressure, muscle sympathetic nerve activity (MSNA), R-R interval, and respiration in 10 healthy volunteers and 3 subjects after sympathectomy. We evaluated 1) the synchronism of LDF fluctuations in two index fingers, 2) the relationship with autonomically mediated fluctuations in other signals, and 3) the LDF ability to respond to arterial baroreflex stimulation (by neck suction at frequencies from 0.02 to 0.20 Hz), using s
