We present a case of left subclavian artery aneurysm in a 48-year-old man with Marfan syndrome. Aneurysmectomy and interposition with an artificial graft were successfully performed through an infraclavicular incision by dividing the clavicle at its midshaft. The clavicle bone was reconstructed with a steel plate, and the postoperative course was uneventful. Because the arterial wall is fragile in cases of connective tissue disorders such as Marfan syndrome, our surgical approach was considered to be helpful for gentle maneuvering in an adequate operative field.

The surgical treatment of malignant fractures of the extremities includes a wide range of methods today. The intramedullary Küntscher nailing of the femur and tibia by the AO (Society for Osteosynthesis) method, Hackethal's multiple nailing in the upper arm and fore arm, and the bone glue-plate compound osteosynthesis methods are described. The alloplastic joint prostheses for hip and knee are extended to proximal and distal femur prostheses. Total femur prosthesis with simultaneous hip and knee joint alloplasty seems to be full of prospect.

PURPOSE OF THE STUDY: Fifth metacarpal neck fractures rank among the most common fractures of the hand skeleton. Only about 10% of these fractures are treated surgically. There are multiple surgical treatment options. This study aims to compare the outcomes of plate osteosynthesis with those of the "bouquet" technique.
MATERIAL AND METHODS: Two cohorts of patients with fractures to the neck of fifth metacarpal treated at a district hospital and at a university hospital between 2017 and 2022 were evaluated, namely due to a different approach to fracture management. The district hospital cohort

BACKGROUND: Single superior symphyseal plating is the most effective method for managing vertically unstable Tile C1-2 and C1-3 pelvic ring injuries. However, high rates of implant failure were more frequently observed in obese patients (body mass index < 30). The study aimed to determine the potential reduction in implant failure rates by adding an anterior symphyseal plate in obese patients (class I).
METHODS: The study was designed as a prospective, randomized controlled trial with a single-blind methodology, conducted at a level 1 trauma center. The study involved 36 patients with Tile C1

BACKGROUND: The use of cephalo-medullary nails (CMN) is a widely accepted management option for the treatment of unstable per-trochanteric hip fractures. A growing body of literature has reported good functional and radiological outcomes in patients managed with a dynamic hip screw supplemented with a trochanteric stabilisation plate (DHS w/ TSP). However, a robust meta-analysis does not exist in the current literature comparing the two fixation methods.
PURPOSES: Management of these kinds of injuries is very challenging in orthopaedic practice, yet no strong evidence is in place to delineate

OBJECTIVE: This study was performed to evaluate the safety and efficacy of a single cannulated iliac screw combined with reconstruction plate fixation for crescent pelvic fractures (Day type II).
METHOD: Thirty adult patients (23 men, 7 women) with a mean age of 42.3 years (range, 22–81 years) were retrospectively reviewed. The time interval between the injury and the operation ranged from 3 to 14 days. The duration of screw insertion, the frequency of C-arm X-ray examination, and the intraoperative blood loss volume were recorded. The minimum follow-up was 12 months (mean, 29 months; range,

The calcanail system is a novel intramedullary approach for calcaneal fractures but is believed to be insufficient to treat complex fractures. We propose a modified Calcanail technique by adding a transfixation screw to improve stability. The aim of this study was to evaluate the biomechanical stability of the modified Calcanail system and compare it with the traditional Calcanail system and plate fixation. A Sanders type-IIIAB calcaneal fracture model was built and simulated fixation with the three implants. A vertical loading of 700 N was applied to the subtalar joint surfaces, and the poste

This article presents the case of an 83-year-old woman with a peri-implant femoral fracture after hip arthrodesis in adolescence. Due to the rarity of such operations, there is no standardized approach for these cases. In order to secure the treatment goal of rapid pain-adapted full weight bearing despite reduced bone quality, it was decided to perform a new osteosynthesis with a retrograde femoral nail after removal of some fixation screws of the existing arthrodesis plate. Due to this type of treatment, the preservation of activity and independence of an older trauma patient could be sustain

In patients with moderate lower lid laxity, the lower orbicularis oculi muscle becomes atonic or ptotic. Hence, in such patient populations, with periorbital fractures, additional vertical support endorsement either by lateral canthopexy or orbicularis oculi muscle suspension flap must accompany plate and screw fixations. In this report, we shared our experience in applying prophylactic suspension to the lower lid with turnover orbicularis oculi transposition muscle flap in zygomatic fractures treated by subciliary approach in 98 patients. Our results show that turnover orbicularis oculi muscl

UNLABELLED: For the treatment of distal humerus an approach involving olecranon osteotomy is frequently preferred as it offers a clearer view, especially in cases of complex intraarticular fractures. It is however associated with the high risk of osteotomy-related complications such as nonunion, delayed healing, implant failure and migration of wires. The aim of the present study was to evaluate the stability of different new procedures that stabilize olecranon osteotomy compared with conventional tension band wiring. We hypothesize that the new implants provide equivalent stability as the con

BACKGROUND: We sought to examine fracture settling and screw penetration after open reduction-internal fixation of 2-, 3-, and 4-part proximal humeral fractures and determine whether the use of calcium phosphate cement reduced these unwanted complications.
METHODS: We performed a retrospective study of prospective data. Inclusion criteria included patient age of 18 years or older and an acute traumatic fracture of the proximal humerus that was treated with open reduction-internal fixation with a locked plate. Metaphyseal defects were treated with 1 of 3 strategies: no augmentation, augmentati

BACKGROUND: This study evaluated the effects of locking vs nonlocking configuration on the biomechanical performance of a calcaneal reconstruction plate in an osteoporotic cadaveric model.
MATERIALS AND METHODS: A Saunders II B type calcaneal fracture was created in ten matched pair of cadaveric calcanei. Each pair was fixed with the Ascension calcaneal reconstruction plate using either locking or nonlocking screws in the same hole pattern. Specimens were axially loaded for 1000 cycles through the talus followed by load to failure. Statistical comparisons were made between the locking and non

Asymmetric load of the ankle joint often results in degenerative disease. Although reconstructive surgery, including osteotomies above and beneath the tibiotalar joint, is possible, there are little data with respect to its evidence. This article presents general considerations for osteotomies around the osteoarthritic ankle and elaborates a rationale for the technical procedure. Additional measures for correcting the deformed and malaligned hindfoot are also elucidated. As a principle, opening-wedge and closing-wedge osteotomies are possible in one or more planes. In some instances, inframall

Minimal invasive osteosynthesis (MIO) should belong nowadays in the armentarium of each trauma surgeon. The tendency to minimize the invasivity of every operation is a logical development considering the goal of each surgeon to reduce the iatrogenic damage caused by the operation. The term 'MIO" stands for this criteria: Fracture zone not opened, reduction by indirect manoeuvres or percutaneously by joy-sticks, small approaches for the application of the implants, intraoperative assessment of the reduction by imaging (fluoroscopy, arthroscopy, endoscopy, etc.). The purpose of this study was to develop a salvage procedure for failed fixation of ankle fractures that occurs in patients with sensory neuropathy. Six patients, with insensitivity to a 5.07 monofilament on the plantar surface of their foot (five with diabetes mellitus, one with alcoholic peripheral neuropathy) presented to a foot and ankle referral center with failed traditional fixation of their Weber B and C ankle fractures. Five fractures were the result of trauma. One fracture occurred without a discernable traumatic event. A laterally placed 4.5 DCP plate on the fibula with multiple 4.5

10 patients with Down's syndrome and hip disorders have been examined. Hip instability was present in 5 patients: habitual dislocations occurred in 2 patients, persistent subluxation was found in 2 patients and 1 patient had bilateral fixed hip dislocation. Hip instability usually occurs after the begin of walking and we suggest to perform regularly radiographic examinations on hip instability. Other hip disorders as slipped femoral capital epiphysis may be complicated by hip instability. Legg-Calvé-Perthes disease was found in 2 patients and slipped femoral capital epiphysis in 3 patients. 7

Axial force applied during epiphyseal distraction has been measured close to skeletal maturity in patients having leg lengthening, in a rabbit model, and in vitro from an amputation specimen. In the patient study, both slow distraction rates and low constant distraction loads were applied. For all the distraction regimens, it was not possible to lengthen the limb significantly without evidence of fracture as demonstrated by a sudden decrease in distraction force. Growth plate failure was observed from 600 to 800 N, these levels being lower than those recorded from the in vitro tests. In the an
