Usefulness of three-dimensional full-scale modeling of surgery for a giant cell tumor of the cervical spine.

STUDY DESIGN: Case report. OBJECTIVES: To report a case with giant cell tumor (GCT) of C6 vertebra, in which three-dimensional (3-D) full-scale modeling of the cervical spine was useful for preoperative planning and intraoperative navigation. SETTING: A university hospital in Japan. CASE REPORT: A 27-year-old man with a GCT involving the C6 vertebra presented with severe neck pain. The C6 vertebra was collapsed and the tumor had infiltrated around both vertebral arteries (VAs). A single-stage operation combining anterior and posterior surgical procedures was scheduled to resect the tumor an

Open reduction and screw fixation of mallet fractures.

Twelve patients with mallet fractures treated by open reduction and internal fixation with small screws were reviewed at an average of 31 months after surgery. The indication for surgery was a fracture involving more than one-third of the distal phalanx articular surface or with subluxation of the distal interphalangeal joint. Loss of reduction occurred in one patient and in another one screw loosened slightly without loss of reduction. There were no nail deformities, infections, or secondary procedures. The mean range of motion was from 6 degrees (range, 0-30 degrees ) (extensor lag) to 70 de

Atlantoaxial stabilization: clinical comparison of posterior cervical wiring technique with transarticular screw fixation.

Symptomatic atlantoaxial instability requires atlantoaxial stabilization. In this study the authors compared clinical, radiographic, and cervical outcome questionnaire results in 67 such patients who underwent 71 separate procedures. Thirty-eight patients had traditional posterior C1-C2 cervical wiring and halo-vest immobilization (group 1), whereas 33 were alternatively managed with transarticular screw fixation without rigid external immobilization (group 2). Mean follow-up in group 1 was 53.2 months and mean follow-up in group 2 was 41.0 months. Radiographic evaluation demonstrated seven ps

Locked fracture dislocation of the calcaneus treated with minimal open reduction and percutaneous fixation: a report of two cases and review of the literature.

Fractures of the calcaneus with associated locked dislocation of the posterior facet have been previously described. Two patients with a calcaneal fracture with a locked dislocation of a portion of the posterior facet were treated with minimally invasive open reduction and percutaneous screw fixation of the fragment with cannulated screws. Both patients had satisfactory reductions and healed the fractures without any soft-tissue complications. This technique can be a useful addition to the armamentarium of the surgeon treating these injuries, especially in the patient at high risk for wound co

[Slipped capital femoral epiphysis and the "Работает" opposite hip].

The gold standard in the therapy of SCFE is the bilateral procedure in Central Europe. Several statistics demonstrate more than 30% of bilateral slippage as an average. Complications such as accidental pin perforation have become rare since the introduction of large cannulated screws. The advantage of screws as an implant is the easier removal of the device after termination of growth. Exceptionally, only the involved site may be operated on when cessation of growth is to be expected immediately. Because the disease takes place in the growing area of the femur, leg length differences up to 2 c

Subclavian artery injury caused by a screw in a clavicular compression plate.

A 22-year-old man presented with left-arm ischaemia, 22 months after placement of a clavicular compression plate for a fractured left clavicle. Digital subtraction angiography revealed a distal subclavian artery injury which, during surgery, was found to be caused by a screw in the compression plate. Ligation of the subclavian artery followed by carotid-axillary bypass resulted in a symptom-free left arm.

[Study of osteosynthesis strength in femoral neck fractures stabilized with a Zespol hip fixator].

Strength study have been carried out on two different osteotomies of the femoral neck stabilized with Zespol hip fixator, performed on fresh specimens taken out from cadaver. Stability of fixation has been assessed in transverse osteotomy of the neck (Pauwels I type of fracture) and in one parallel to the long axis of the femur (Pauwels III type). The results were compared to similar studies including different fixing methods (DHS, Thornton nail, von Bahr screw, Knowles pin) but identical types of osteotomy, published by other authors. Zespol fixation proved to be equal and frequently superior

[Recent fractures of the humeral shaft in adults. Role of surgical treatment using screwed plates. Apropos of 35 cases surgically treated].

Chronic pain disorders are often associated with negative emotions, including anxiety and depression. The central nucleus of the amygdala (CeA) has emerged as an integrative hub for nociceptive and affective components during central pain development. Prior adverse injuries are precipitating factors thought to transform nociceptors into a primed state for chronic pain. However, the cellular basis underlying the primed state and the subsequent development of chronic pain remains unknown. Here, we investigated the cellular and synaptic alterations of the CeA in a mouse model of chronic muscle pa

Luque interpeduncular segmental fixation of the lumbosacral spine.

Interpeduncular segmental fixation of the lumbar and lumbosacral spine with Luque screws and plates is becoming increasingly popular. In this retrospective study, 52 consecutive patients treated with this technique were followed for 16 to 44 months (average, 21). Indications for surgical arthrodesis included lumbar instability from degenerative disease (16 cases), spondylolisthesis (25 cases), pseudarthrosis (eight cases), fracture (two cases), and tumor (one case). Fusion with instrumentation spanned one level in nine patients, two in 38 patients, three in four patients, and four in one patie

Open reduction and fixation of proximal humeral fractures and fracture-dislocations.

Open reduction and internal fixation was employed in the treatment of 25 severely displaced fractures and fracture-dislocations of the proximal humerus. Our aims were accurate reduction and stable fixation to allow early mobilisation and to achieve full functional recovery. In 15 fractures an AO T-plate was used and in 10 a bent semitubular plate was employed as a blade plate. Excellent or satisfactory results were obtained in all six patients with two-part fractures involving the surgical neck; in four of the five patients with three-part fractures involving the surgical neck and tuberosities

Ogden plate and other fixations for fractures complicating femoral endoprostheses.

Treatment of femoral shaft fractures complicating endoprostheses remains controversial. Nineteen such fractures were treated with open reduction and internal fixation using the Ogden plate. This modified plate allows for proximal fixation with heavy-duty Parham bands and distal fixation with screws. Sixteen of 19 patients healed their fractures in an average of 3.5 months. Two fractures developed delayed unions, one resulted in non-union. At final follow-up examination, results were rated as excellent (12), satisfactory (5), and poor (2). The procedure is not technically difficult. It can be a

[Percutaneous reconstruction and fixation of closed fractures of the tibial head. Results of a follow-up study].

A retrospective study was undertaken into 105 cases of closed fractures of the tibial head which had been treated by percutaneous elevation and fixation at the Surgical Casualty Ward of Dortmund, between 1973 and 1983. Follow-up check made 24 months from surgery, on average, revealed functional results which were in all respects comparable to those recorded from cases of open reduction and fixation. The approach was found to be applicable to closed reduction and impression fractures with first-grade and second-grade damage to soft tissue. Stability in exercise is claimed together with low surg

Preoperative skeletal traction in Legg-Perthes disease.

In four of 11 patients with Legg-Perthes disease for which a varus femoral osteotomy was planned, routine split Russell traction failed to produce complete preoperative containment. Preoperative skeletal traction was used to distract the joint, displace the extruded lateral physis below the acetabular margin, and then reintroduce the femoral head into the acetabulum with progressive abduction. The femoral heads were rated postoperatively as good (two cases), fair (two cases), and poor (one case). Preoperative skeletal traction is an adjunctive treatment method for patients with "hinge abductio

Periodontal effects of rapid maxillary expansion with tooth-tissue-borne and tooth-borne expanders: a computed tomography evaluation.

INTRODUCTION: The force delivered during rapid maxillary expansion (RME) produces areas of compression on the periodontal ligament of the supporting teeth. The resulting alveolar bone resorption can lead to unwanted tooth movement in the same direction. The purpose of this study was to evaluate periodontal changes by means of computed tomography after RME with tooth-tissue-borne and tooth-borne expanders. METHODS: The sample comprised 8 girls, 11 to 14 years old, with Class I or II malocclusions with unilateral or bilateral posterior crossbites Four girls were treated with tooth-tissue-borne

Morphological and dimensional changes after barrier removal in bone formed beyond the skeletal borders at titanium implants. A kinetic study in the rabbit tibia.

The aim of the present experimental investigation was to study the morphological and dimensional changes of bone, augmented at titanium implants by a membrane technique, taking place after membrane removal. In 12 rabbits, screw-shaped titanium implants were inserted in the tibial metaphyses in such a way that 5 threads became uncovered with bone. Surgery was performed on 2 occasions in order to retrieve specimens with different follow-up times. An e-PTFE barrier and a titanium device were used to provide space for bone formation. In 1 tibia of each rabbit, the membranes and spacers were remove

Late Bone Marrow Edema Associated With Use of a Bioresorbable Polymer Suture Anchor in Foot and Ankle Surgery.

BACKGROUND: Bioresorbable orthopaedic implants have proliferated in recent years, including suture anchors, soft tissue scaffolds, and screws. At our institution, we anecdotally noticed late inflammatory-type reactions in the bone surrounding a particular bioresorbable polymer suture anchor. This bioresorbable anchor (BRA) uses ultrasonic technology to liquefy its tip, allowing interdigitation with surrounding cancellous bone. The polymer is designed to resorb around 2-3 years. In this study, we aimed to characterize complications associated with its use. METHODS: A retrospective review was c

Pullout repair combined with double-plate fixation via inverted V-shaped high tibial osteotomy for medial meniscus posterior root tears with severe varus tibial deformity.

Medial meniscus (MM) posterior root tears (PRTs) disrupt hoop stress transmission and accelerate the progression of medial compartment osteoarthritis. High tibial osteotomy (HTO) combined with transtibial pullout repair is indicated in selected patients with a high body mass index, severe varus malalignment, or high functional demands. Although medial open-wedge HTO (MOWHTO) is widely performed, correction angles exceeding approximately 12° may compromise mechanical stability and increase the risk of lateral hinge fracture and delayed union. In patients with predominantly tibial-based deformit
