Long-term risks of major adverse cardiovascular events after acute kidney injury: a systematic review and meta-analysis.

BACKGROUND AND AIMS: Acute kidney injury (AKI) is increasingly recognized as a long-term risk factor for chronic kidney disease and cardiovascular disease (CVD). However, it remains uncertain which patients with AKI are at greatest CVD risk. We performed a systematic review and meta-analysis to quantify CVD risks post-AKI and to determine whether these risks differ by AKI severity, duration, and clinical setting. METHODS: PubMed and Embase were systematically searched for studies comparing individuals with and without AKI and reporting major adverse cardiovascular events (MACE) or individual

Shorter Wound-healing Times Associated With Select Perioperative Medical Therapies in Hidradenitis Suppurativa Surgery, and a Limited Clinical Role of Intraoperative Cultures: A 194-Case Retrospective Review.

BACKGROUND: Hidradenitis suppurativa (HS) is a chronic inflammatory disease of apocrine gland-bearing areas characterized by abscesses, tunneling, scarring, and recurrent flares. Surgical excision is central in refractory disease, but perioperative protocols are variable. We evaluated whether specific perioperative medical therapies are associated with shorter time to documented postoperative wound-healing progress after HS surgery, and whether intraoperative cultures meaningfully guide antibiotic management. METHODS: We conducted a retrospective cohort study of 194 HS surgeries performed at

Continuity of care for patients with chronic lymphocytic leukemia: an analysis of real-world data.

AIMS: This study hypothesized that greater continuity of care (CoC) would be associated with lower all-cause healthcare resource utilization and improved overall survival (OS) among patients with chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL) among patients who received covalent Bruton tyrosine kinase inhibitor (cBTKi)-based therapy. METHODS: Optum's de-identified Clinformatics® Data Mart Database was used for this retrospective study. Patient-level CoC measured by continuity of hematologist/oncologist provider care was evaluated using published measures; the Herfindahl-Hirschm

Allocation of Semaglutide According to Coronary Artery Calcium and BMI: Applying the SELECT Trial to MESA.

BACKGROUND: Implementation of semaglutide weight loss therapy has been challenging due to drug supply and cost, underscoring a need to identify those who derive the greatest absolute benefit. OBJECTIVES: Allocation of semaglutide was modeled according to coronary artery calcium (CAC) among individuals without diabetes or established atherosclerotic cardiovascular disease (CVD). METHODS: In this analysis, 3,129 participants in the MESA (Multi-Ethnic Study of Atherosclerosis) without diabetes or clinical CVD met body mass index criteria for semaglutide and underwent CAC scoring on noncontrast

Predictors of readmission after esophagectomy for esophageal cancer: a systematic review and meta-analysis.

INTRODUCTION: Esophageal cancer represents a major global health challenge. Multimodality treatment, involving esophagectomy with chemotherapy, radiotherapy, or immunotherapy, remains the standard of care for locally advanced esophageal cancers. OBJECTIVES: Unplanned hospital readmission has emerged as a key indicator of surgical quality and long-term patient outcomes after esophageal cancer surgery. This meta-analysis aims to provide a comprehensive review of predictors of unplanned readmission following esophagectomy. METHODS: A systematic review and meta-analysis were conducted in accorda

Impact of multidimensional assessment on anti-fracture treatment decisions in patients with fragility hip fractures within a Fracture Liaison Service.

BACKGROUND: Osteoporosis is a chronic condition characterized by increased fracture risk. Fragility fractures, especially hip fractures, represent a significant health and economic burden due to population aging. Despite the efficacy of approved treatments in lowering fracture recurrence, post-fracture treatment rates remain suboptimal. To address these issues, various post-fracture care programs, including Fracture Liaison Services (FLS), have been implemented worldwide. While FLS models effectively reduce refracture risk and maintain cost-effectiveness, it is unclear if these benefits apply

Potential mental health-related harms associated with the universal screening of anxiety and depressive symptoms in Australian secondary schools.

BACKGROUND: Anxiety and depressive disorders typically emerge in adolescence and can be chronic and disabling if not identified and treated early. School-based universal mental health screening may identify young people in need of mental health support and facilitate access to treatment. However, few studies have assessed the potential harms of this approach. This paper examines some of the potential mental health-related harms associated with the universal screening of anxiety and depression administered in Australian secondary schools. METHODS: A total of 1802 adolescent students from 22 se

Beta-adrenergic antagonist for the healing of chronic diabetic foot ulcers: study protocol for a prospective, randomized, double-blinded, controlled and parallel-group study.

BACKGROUND: Diabetic foot ulcers (DFUs) are the most common cause of leg amputations and their management is extremely challenging. Despite many advances and expensive therapies, there has been little success in improving outcomes of DFUs. In prior work our laboratory has examined the effects of beta-adrenergic antagonists (βAAs) on skin and skin-derived cells. We have shown that βAAs enhance the rate of keratinocyte migration, promote angiogenesis, and hasten wound healing in scratch wounds in vitro, in animal wound models, and in anecdotally reported cases of chronic wounds that healed succe

Activity limitations and participation restrictions in patients with peripheral facial palsy: a cross-sectional study over a six-year period.

BACKGROUND: Peripheral facial palsy is a pathological condition caused by a wide range of etiologies. A damage of VII cranial nerve produces facial disfigurement and limitations in daily life activities, such as drinking, eating and speaking. As a consequence, patients may experience psychological distress and social isolation. To counsel and design a patient-tailored rehabilitation for patients affected by peripheral facial palsy, physical and social limitations should be considered. Moreover, the knowledge of factors associated with disability plays a key role in the early identification and

Predictors of re-exacerbation after an index exacerbation of chronic obstructive pulmonary disease in the REDUCE randomised clinical trial.

BACKGROUND: Acute exacerbations of chronic obstructive pulmonary disease (AECOPD) compromise physical activity and quality of life and contribute significantly to health care costs. Systemic glucocorticoids benefit clinical outcome in AECOPD, and the REDUCE trial demonstrated noninferiority of a 5-day treatment course with prednisone compared with 14 days therapy regarding clinical outcome over 6 months of follow-up. Unexpectedly, we found an inverse correlation between circulating cortisol levels and exacerbation risk during a 6-month follow-up period. OBJECTIVE: To evaluate whether addition

Exploring the associations of tobacco smoking and serum cotinine levels with selected inflammatory markers in adults with HIV in South Africa.

This study examined the associations between tobacco smoking and serum cotinine levels, an objective biochemical measure of tobacco smoke exposure, with markers of inflammation, i.e., interferon-gamma (IFN-γ), interleukin 10 (IL-10), interleukin 2 (IL-2) and tumour necrosis factor-alpha (TNF-α) in people living with HIV (PLWH).These specific markers were selected because of their hypothesised associations with smoking, PLWH and their outcomes. In a random sample of ≥ 18-year-old PLWH receiving care at 17 public healthcare facilities across the Western Cape Province in South Africa, data collec

An Insight through Various Models being used for Assessment of Depression and its Management at Primary Level in Current Scenario.

AIM: The main aim of present study to assess depression and various neurotransmitters involved in it with their evaluating models. BACKGROUND: Depression is a common ailment that can be recurrent or even become chronic. According to the National Service Framework for Mental Health (released in 1999 by the Department of Health), people with depression should continue to be treated primarily in primary care settings. Despite mounting evidence that general practitioners could do a better job of detecting and treating depression, little research has been done on the perspectives of general practi

The prevalence, risk factors and outcomes of anaemia in South African pregnant women: a protocol for a systematic review and meta-analysis.

BACKGROUND: A significant cause of morbidity and mortality during pregnancy is maternal anaemia. The causes of anaemia and the sequelae are varied, and the prevention and management are public health challenges, especially in resource-limited settings and certain geographic locations. South Africa is plagued by a quadruple burden of disease, with high maternal mortality rates affected by hypertensive disorders of pregnancy, HIV, tuberculosis and neglected tropical diseases. This is most prevalent in people of lower socio-economic status. Poor nutrition, chronic infections, lack of access to he

Development of a synthetic antimicrobial peptide targeting MDR wound pathogens and biofilms: effective therapeutics for chronic wound management.

Multidrug-resistant (MDR) pathogens are a major contributor to chronic and non-healing wound infections, where biofilms, high ionic strength and protease-rich exudates severely limit the performance of conventional antimicrobials. By 2050, antimicrobial resistance has been projected to cause 10 million deaths annually, raising an urgent need for therapeutics that can remain active in harsh wound microenvironments. Although antimicrobial peptides (AMPs) offer broad-spectrum activity, their clinical applicability is limited due to poor stability, low bioavailability and host toxicity, thus restr
