Rare ligamentum flavum cyst causing incapacitating lumbar spinal stenosis: Experience with 3 Chinese patients.

Three Chinese patients suffered from severe lumbar spinal stenosis with debilitating symptoms due to a rare condition of ligamentum flavum cysts in the midline of the lumbar spine. This disease is distinct from synovial cyst of the facet joints or ganglion cysts, both intraoperatively and histopathologically. Magnetic Resonance imaging features of the ligamentum flavum cyst are also demonstrated. We share our surgical experiences of identification of the ligamentum flavum cysts, decompression and excision for two of the patients with demonstrably good recovery. This disease should be considere

Management of intraabdominal hypertension by percutaneous catheter drainage.

The authors report a case of an 18-year-old man with T-cell acute lymphocytic leukemia who developed hemorrhagic pancreatitis after chemotherapy. He subsequently developed abdominal compartment syndrome (ACS). Computed tomography showed a large fluid-filled mass in the area of the pancreas. As a result of the instability of his condition, surgical decompression, the standard therapy for ACS, was believed to carry significant morbidity and potential mortality. The patient underwent ultrasound-guided drainage of the peripancreatic fluid, which decreased his abdominal pressures and improved his c

Recovery from Duret hemorrhage: a rare complication after craniotomy--case report.

A 44-year-old female presented with Duret hemorrhage due to transtentorial herniation by extradural hematoma as a complication after craniotomy for treatment of spontaneous middle cranial fossa cerebrospinal fluid leakage through the oval window. Brain computed tomography revealed linear hemorrhage in the midbrain and the rostral pons. She awoke after 2 weeks in a coma, despite showing ocular bobbing and bilateral intranuclear ophthalmoplegia. She was discharged from the hospital with minimal neurological defects. Duret hemorrhage is usually fatal, but this case shows that early surgical decom

Aseptic meningitis caused by Teflon implantation for microvascular decompression. Case report.

The authors present the case of a 47-year-old man who, after undergoing microvascular decompression for trigeminal neuralgia, experienced symptomatic pain relief but developed prolonged aseptic meningitis. This case is unusual in that the patient remained dependent on steroid medications for nearly 5 months following the initial surgery and the aseptic meningitis did not resolve until after surgical removal of the Teflon used to pad the trigeminal nerve. The pathophysiological characteristics of the body's reaction to implanted Teflon are discussed along with the rationale for removing this su

Paraspinal abscess associated with epidural in labour.

A 24-year-old presented in labour requesting an epidural. She had been diagnosed as having "pelvic arthropathy" at 37 weeks. After an uneventful epidural and instrumental delivery, she discharged herself home. She re-presented 19 days later with left hip pain and abnormal neurological signs in the lower limbs. On MRI, there was a large paraspinal abscess with an epidural granulation mass compressing her spinal cord. She had an urgent surgical decompression. In hindsight, it is likely that the paraspinal mass was present at the time of epidural insertion. The discussion highlights that complica

[Chiari I malformation with syringomyelia treated by decompression of the cranio-spinal junction and tonsillectomy. Apropos of 8 cases].

We report a series of 8 patients with syringomyelia and associated Chiari I malformation treated between 1992 and 1996 using a cranio-vertebral decompression with tonsil resection. The follow up period ranged from 6 months through 4 years (mean: 28 months). Neurological symptoms and signs improved in 7 patients (87%), and were stabilized in 1 (13%). At 6 months, postoperative MRI showed that the syringe has decreased in all patients. We emphasize the respect of the different stages of surgical procedure (limited foramen magnum opening, arachnoid preservation, tonsillar subpial aspiration, arac

Postvitrectomy positioning complicated by ulnar nerve palsy.

PURPOSE: Ulnar nerve palsy is an unusual complication of postvitrectomy positioning. METHOD: We report two such cases. RESULTS: Both patients developed ulnar nerve palsy after maintaining a face-down position for 2 to 4 weeks after vitrectomy with intraocular perfluoro-octane. The neuropathy resolved in one patient after he was instructed to avoid prolonged pressure on his flexed elbows, whereas the other patient ultimately required surgical decompression of the nerve at the elbow. CONCLUSION: Ulnar nerve palsy can result from postvitrectomy positioning associated with prolonged direct pres

[Diagnosis and treatment of neoplastic diseases of extrahepatic bile ducts and major duodenal papilla].

Fifty-three operations were conducted on 48 patients for tumors of the extrahepatic bile ducts and the major duodenal papilla. Early diagnosis of this pathological condition is very difficult, which affects directly the results of treatment; 18% of patients underwent radical operations. Modern diagnostic methods--computer tomography, ultrasonic examination, and endoscopic retrograde pancreatocholangiography made it possible to detect neoplastic diseases in all of the examined patients. Radical surgical treatment is expedient in distal localization of the neoplastic process and in the absence o

Hypointense MR signal in chronically inspissated sinonasal secretions.

Six patients with chronically obstructed sinuses were examined with computed tomography (CT) and magnetic resonance (MR) imaging prior to surgical decompression. In all six patients, hypointense signal was present on all MR sequences despite CT evidence of the presence of high-attenuation material filling the sinus. At surgery, all specimens were viscid or pastelike with no evidence of hemorrhagic products as a cause for the MR findings. Sinonasal secretions may have a spectrum of MR signal intensity, ranging from hyperintense to signal void with all pulse sequences. These findings must be kep

Budd-Chiari syndrome: shunt selection and post-operative assessment.

Radiological investigation of patients with the Budd-Chiari syndrome is helpful in making the diagnosis and to determine the most suitable method of surgical decompression of the portal venous system where this is indicated. The radiological findings in four patients with Budd-Chiari syndrome who were treated by meso-atrial shunt operations are described. The key pre-operative investigation is inferior vena cavography with pressure measurements. Postoperatively shunt patency is most reliably assessed by dynamic CT scanning. Doppler ultrasound studies may detect flow within the shunt but patenc

[Acoustic variation after surgery for severe Bell's palsy].

Auditory thresholds were studied before and after surgical decompression of the facial nerve in 35 patients following either transmastoidal (20 cases) or transmastoidal and suprapetrosal (15 cases) operations. Seventeen of the patients were under 40 years of age at the time of surgery, which did not affect hearing in the opposite ear. However, auditory threshold levels fell in the operated ear. These were of the bone conduction type, and possessed characteristics of an acute hearing lesion (maximal in the 4000 frequency range and improving progressively) of 0 to 35 dB (mean 5 dB). Possible cau

Management of trigeminal neuralgia (tic douloureux).

The initial treatment of trigeminal neuralgia is accomplished with drugs, primarily carbamazepine and occasionally phenytoin sodium. However, many patients become refractory to treatment or side effects develop such as drowsiness or ataxia. In these circumstances, surgical therapy is appropriate. At our institution, microsurgical decompression has yielded good to excellent results in 29 of 32 patients and is currently recommended for persons in good general health who are younger than 70 years. Because of the notable incidence of unpleasant dysesthesias in the face, percutaneous radiofrequency

[Surgical technics in tumors of the spine].

The authors have treated two hydatic cysts, three plasmocytomas, one chondrosarcoma, one osteogenic sarcoma and four metastasis at the level of the spine. The surgical procedures may be either palliative or curative. In metastatic lesions, posterior decompression was associated with internal fixation for prevention or improvement of neurological signs, providing more comfort during the patient's survival. Curative procedures were achieved a two-stages excision of the vertebra by anterior and posterior approachs, grafting and fixation using Harrington's rods. Surgery was followed by chemotherap

Temporary Percutaneous Lateral Femoral Cutaneous Nerve Stimulation for Surgically Refractory Meralgia Paresthetica: A Case Report.

BACKGROUND: Meralgia paresthetica (MP) is a neuropathic pain syndrome resulting from lateral femoral cutaneous nerve irritation or injury. Patients who fail to respond to conservative therapies can find relief after interventions, including nerve blocks and decompression surgery, but some continue to experience pain. Temporary percutaneous peripheral nerve stimulation (PNS) is a minimally invasive option for targeted neuropathic pain management, though there is limited evidence of its efficacy in the management of MP. CASE REPORT: A 63-year-old man with MP unresponsive to conservative and sur

Spinal hydatid disease with epidural extension and spinal cord compression: A radiology case report.

Spinal hydatid disease is rare, destructive parasitic infection of the spine. The disease often presents with nonspecific symptoms. that overlaps with spinal tuberculosis, pyogenic infection, and metastatic malignancy. We report a case of 65-year-old man with thoracic spine involvement of Hydatid disease. Magnetic resonance imaging demonstrated destructive vertebral disease with a characteristic cystic morphology. The imaging pattern favoured hydatid cyst as the most likely diagnosis. Correlation with epidemiologic history and serology was used to confirm the diagnosis and guide management. Du

Simultaneous occurrence of epidural, subdural, and subarachnoid hemorrhages in the spinal canal: a rare case report.

Simultaneous spinal epidural, subdural, and subarachnoid hemorrhages are exceedingly rare, with no prior documented case of all three occurring simultaneously in the literature. We present the case of a 59-year-old woman who developed acute lower back pain and progressive weakness in her lower extremities. MRI revealed three distinct types of hemorrhages: an epidural hematoma extending from T12 to the lumbar spine, a subdural hematoma between T11 and S1, and a subarachnoid hemorrhage involving the cauda equina. Surgical decompression was performed, leading to significant clinical improvement.

Surgical anatomy of sigmoid sinus with evaluation of its venous dominance for advances in preoperative planning.

Microvascular decompression (MVD) is a widely adopted neurosurgery in treating cranial nerve diseases providing long-term pain relief. Improving surgical techniques has been a focus of recent studies. Venous structures such as the sigmoid sinus are essential to protect, and whose risk of destruction during surgery increases with size. The medical records of patients who went through MRI ahead of MVD surgery between Dec 2020 and Dec 2021 were reviewed. Section area of sigmoid sinus calculated from the MRI plane of auditory nerve showed a right dominance of the sinus. The improved method concern

Lumbar Epidural Hematoma as a Rare Complication From Minimally Invasive Lumbar Decompression.

Lumbar spinal stenosis (LSS) is a common and debilitating pathology globally. Conservative and surgical treatment options exist for patients. Recently, minimally invasive lumbar decompression (MILD) has been described as a less invasive technique for the treatment of early spinal stenosis ≥2.5mm ligamentum thickening or in patients at high risk for general anesthesia. Often, MILD is performed by interventional pain providers and shows low complication rates. We describe a 76-year-old woman who presented to the emergency department immediately after undergoing a MILD procedure at an outside sur
