Atypical mature bone in the otosclerotic otic capsule as the differentiated zone of an invasive osseous neoplasm.

CONCLUSION: A large proportion of the mature otic capsule bone in cases of otosclerosis lies in plaques in direct contiguity with active otosclerosis and, because it shows significant structural defects, it should be regarded as part of the otosclerotic process. These appearances support our previously described suggestion that otosclerosis is an invasive osseous neoplasm, the mature atypical bone representing differentiation of earlier-formed invasive neoplastic osseous tissue. OBJECTIVES: We sought structural features in differentiated bone within the otic capsules of cases of otosclerosis

Auditory and vestibular assessment of patients with Ménière's disease who suffer Tumarkin attacks.

Tumarkin attacks are a feature of Ménière's disease that generate a significant degree of disability. The surprising nature of these events is the main reason behind their associated morbidity. In this study we set out to evaluate auditory and vestibular function, as well as disability, in a population of patients who suffer Tumarkin attacks. We found that patients who suffer Tumarkin attacks are more disabled and experience severer and more frequent autonomic symptoms and that their hearing level is significantly worse in the asymptomatic ear, especially at low frequencies. Accordingly, we co

Imaging characteristics of metastatic lesions to the cerebellopontine angle.

OBJECTIVE: To describe characteristic features of metastatic lesion to the cerebellopontine angle (CPA) and internal auditory canal (IAC). STUDY DESIGN: Retrospective review. SETTING: Tertiary care medical center. PATIENTS: Patients with metastatic lesions to the CPA. INTERVENTION: Diagnostic. MAIN OUTCOME MEASUREMENTS: Clinical presentation and imaging characteristics on magnetic resonance imaging and computed tomography. RESULTS: A total of 25 cases were reviewed. The average patient age was 56 years, and almost all patients presented with palsy of the cranial VII and VIII nerves. Ther

Determinants of tumor size and growth in vestibular schwannomas.

OBJECT: This study was undertaken to analyze factors associated with the size of unilateral vestibular schwannomas (VSs). METHODS: A retrospective analysis of an unselected and sequential series of 433 patients with unilateral VSs was conducted. Tumor size was defined by the largest dimension of the tumor in the cerebellopontine angle, and the size was tested for a relationship with patient age and sex. In a subgroup of 231 patients in whom data were available, tumor size was also tested for a relationship with tumor cysts or the absence of an internal auditory canal (IAC) component. Some pat

Pulsatile and nonpulsatile tinnitus: a systemic approach.

Tinnitus is an annoying symptom that affects approximately 10% of the population. Unfortunately, in more than 60% of patients, even an extensive workup may not lead to diagnosis, partly because of the diverse causes of the symptom. The imaging protocol study should therefore be adapted to the history of the patient, and each imaging study should underline which etiologies it may exclude. Imaging studies should only be performed after a careful clinical examination and otoscopy. Some characteristic clinical findings are suggestive of such etiologies as pulsed synchronous tinnitus or positional

Steps toward recovery of function after hemilabyrinthectomy in frogs.

Removal of the labyrinthine organs on one side results in a number of severe postural and dynamic reflex deficits. Over time some of these behavioral deficits normalize again. At a chronic stage the brain of frogs exhibits a number of changes in vestibular and propriospinal circuits on the operated side that were studied in vitro. The onset of changes in the vestibular nuclear complex was delayed, became evident only after head posture had recovered by more than 50%, and was independent of the presence or absence of a degeneration of vestibular nerve afferent fibers. The time course of changes

Vestibular neurectomy: retrosigmoid-intracanalicular versus retrolabyrinthine approach.

Selective vestibular neurectomy is an effective treatment for intractable vertigo of peripheral vestibular origin when preservation of hearing is a goal. The retrolabyrinthine and retrosigmoid-intracanalicular approaches have been used predominantly at our institutions over the last 10 years. The results and complications of these two techniques were compared. No significant differences were found between hearing results in these two patient groups. The retrosigmoid-internal auditory can approach yielded better control of recurrent episodic vertigo, as well as superior ablation of postoperativ

Frequency characteristics of summating potential in Menière's disease.

SP iso-response functions in 9 patients with Menière's disease were examined for differences in the properties between +SP and -SP. The iso-response functions of +SP and -SP exhibited a large difference. The iso-response function for +SP was centered at high frequencies (around 9 kHz in most cases) with fairly steep slopes on both sides towards both higher and lower frequencies, whereas -SP showed a low-pass function. The tendency was found for patients with normal hearing at high frequencies to have a larger Q10dB than patients with a hearing loss. The difference in properties between +SP and

Surgical management of Meniere's disease with endolymphatic sac decompression by wide bony decompression of the posterior fossa dura: technique and results.

The results of wide bony decompression of the posterior fossa in 49 consecutive individuals with idiopathic unilateral Meniere's disease are reported. These results are similar to many previously published series, 71% of individuals are free of vertigo and 86% have unchanged or improved hearing. Utilizing the classification proposed by the American Academy of Ophthalmology and Otolaryngology, 73% fell into Class A, B, or C with relief of vertigo. These results together with the low complication rate encourages us to consider surgery at an earlier date, hoping to stabilize hearing at a more use

[Vibrant Sound Bridge System. A new kind hearing prosthesis for patients with sensorineural hearing loss. 2. Audiological results].

OBJECTIVE: Implantable hearing aids present a new treatment modality for patients suffering from sensorineural hearing loss. The functional gain obtained with the partially implantable Symphonix soundbridge system was evaluated in a clinical study. The audiological results achieved with n = 34 patients over a period of up to three years are presented in this second part of the publication. PATIENTS AND METHODS: 34 patients have received the Symphonix Vibrant soundbridge system since February 1997. The average age at implantation was 47.2 years (minimum: 18.9 years; maximum: 80.3 years). All p

Audiological Profile and Factors Associated With Hearing Impairment Among Adults With Allergic Rhinitis in Tamil Nadu, India: A Hospital-Based Cross-Sectional Study.

Background Allergic rhinitis (AR) is a common chronic inflammatory disorder that can adversely affect both middle and inner ear function through Eustachian tube dysfunction (ETD) and cochlear involvement. Despite increasing evidence of auditory complications, hearing impairment among adults with AR remains under-recognized in routine clinical practice. This study aimed to estimate the prevalence of hearing impairment and identify sociodemographic and clinical factors independently associated with hearing impairment among adults with allergic rhinitis attending a tertiary care centre in Tiruvan

Frequency dependence and harmonic distortion of stapes displacement and intracochlear pressure in response to very high level sounds.

Previous reports have suggested that intracochlear pressures (PIC) measured at the base of the cochlea increase directly proportionally with stapes displacement (DStap) in response to moderately high (<130 dB SPL) level sounds. Consistent with this assumption, we have reported that for low frequency sounds (<1 kHz), stapes displacement and intracochlear pressures increase linearly with sound pressure level (SPL) for moderately high levels (<130 dB SPL), but saturate at higher exposure levels (>130 dB SPL). However, the magnitudes of each response were found to be frequency dependent, thus the
