[Neurostimulation techniques in the therapeutic management of chronic pelvic and perineal pain].

INTRODUCTION: Neuromodulation is a nonspecific analgesic treatment whose mechanism of action has not yet been elucidated. The purpose of this article is to review the techniques and results of neuromodulation in the management of chronic pelvic and perineal pain. MATERIAL AND METHODS: A comprehensive review of the literature was performed by searching PUBMED for articles on the various neuromodulation techniques used in the management of chronic pelvic and perineal pain. RESULTS: Several levels of neuromodulation of the somatic nervous system have been evaluated in the management of pelvic p

[Value of electroneuromyography in the diagnosis and follow-up of myasthenia gravis].

The diagnosis of myasthenia gravis is above all clinical, however electroneuromyography (ENMG) and laboratory tests can be helpful. ENMG testing is readily available and with immediate results. Useful with emergent symptoms, it is equally so in the event of more acute pictures found in generalized motor deficit and bulbar findings of respiratory failure. It can be performed in out-patient clinics or at the bedside. ENMG is based on an assessment of repetitive nerve stimulations (RNS), which can be sensitized, and more rarely on jiter studies using single-fiber electromyography (SFEMG). RNS rec

Intraocular epiretinal prosthesis to restore vision in blind humans.

Visual sensations in blind patients suffering from retinal degenerations may be restored by electrical stimulation of retinal neurons using implantable microelectrode arrays. The EPI-RET-3 project was initiated to evaluate a wireless intraocular retinal implant system for human use in terms of safety and efficiency. The implant is a remotely controlled fully intraocular prosthesis consisting of a receiver and a stimulator module. The stimulator is placed onto the retina's surface. Data and energy are transmitted via an inductive link from outside the eye to the implant. The EPI-RET-3 device wa

Slow-transit constipation: evaluation and treatment.

Slow-transit constipation is characterized by delay in transit of stool through the colon, caused by either myopathy or neuropathy. The severity of constipation is highly variable, but may be severe enough to result in complete cessation of spontaneous bowel motions. Diagnostic tests to assess colonic transit include radiopaque marker or radioisotope studies, and intraluminal tests (colonic and small bowel manometry). Most patients with functional constipation respond to laxatives, but a small proportion are resistant to this treatment. In some patients biofeedback is helpful although the mech

Spinal cord stimulation for failed back surgery syndrome and other disorders.

Chronic pain is a complex condition that requires a multi-disciplinary approach to management. Spinal cord stimulation (SCS) has evolved into a relatively easily implemented, reversible technique with low morbidity for the management of chronic, intractable pain in selected patients. Percutaneous placement of electrode arrays, under local anaesthesia. supported by programmable, implanted electronics has been a major technical advance. Multicenter prospective studies were conducted and demonstrated that SCS. as a neuromodulation procedure, is indeed a superior method for treatment of chronic pa

[The venous ulcer therapy in use of the selected physical methods--circumstances for clinical application].

Venous ulcers are significant both surgical and dermatological problem. The chronic and recurrent natures of the disease require a search for new methods to improve treatment efficiency. Therefore, the attempts at using some physical methods are needed. The authors review the current literature on characteristics and biological effects of high voltage stimulation (HVS), ultrasound (sonotherapy) and laser biostimulation. Modern hypotheses (skin battery theory, cavitation, acoustic streaming, fotobiostimulation effect) explaining the mechanisms of applied physical factors arising in soft tissue

Percutaneous neuromodulation.

Neuromodulation for pelvic floor dysfunction has evolved from central sacral stimulation, a relatively invasive, experimental procedure, to percutaneous peripheral neurostimulation, which is both minimally invasive and well-tolerated by patients. Multiple series have now reported consistent positive results for varied manifestations of pelvic floor dysfunction. Future applications will involve an implantable peripheral neurostimulator coupling with the posterior tibial nerve, empowering patients to adjust the frequency or amplitude of stimulation. It is anticipated that broader availability of

[Pulsed radiofrequency treatment in idiopathic and secondary glossopharyngeal neuralgia: preliminary results in 2 cases].

OBJECTIVES: To describe the use of pulsed radiofrequency (PRF) to treat 2 cases of glossopharyngeal neuralgia, 1 idiopathic and 1 secondary. PATIENTS: The first case involved a man diagnosed with Hodgkin's lymphoma involving the tonsils and manifesting as glossopharyngeal neuralgia that was refractory to all treatment approaches tried, including opioids and neuromodulation. The second case involved a patient diagnosed with essential glossopharyngeal neuralgia that was also resistant to drug treatment. The PRF procedure was performed in the operating room on an outpatient basis, under fluorosc

Evaluating patients with anorectal incontinence.

Fecal incontinence is a devastating disability, and although this condition is widely accepted as a problem in the elderly, it is now becoming apparent that much younger age groups are also frequently affected. Thorough assessment of anorectal incontinence is very important to choosing the most appropriate treatment. Careful history-taking and physical examination can identify the cause of most cases of incontinence and are essential in every patient. Several incontinence scoring systems have been proposed to provide an objective measure of a subject's degree of fecal incontinence, but only on

[How I treat ... essential tremor].

Essential tremor is the most prevalent movement disorder. This condition is often considered as benign but it can be the cause of considerable social, professional, and psychological handicap. It affects people of all ages but its prevalence increases with age (up to 5% of patients 65 or older). After discussing clinical aspects of this disorder, we review the current medical and surgical therapeutic options. Surgery may bring up spectacular improvement.

[Vagus nerve stimulation. A potential therapy for chronic/recurrent depression?].

Vagus Nerve Stimulation (VNS) is since the 1990 a clinically useful anticonvulsant therapy for treatment-resistant epilepsy. Open acute and longer term data suggest the potential clinical utility of VNS as an antidepressant therapy especially in treatment refractory depression. The vagus nerve has connections to the limbic system and other brain structures which modulate affect. PET studies showed functional changes under VNS in such critical areas, which can explain the mechanisms of action of VMS. Ongoing studies will have to better establish its acute and longer-term efficacy, and specific

Review of the functional surgical treatment of dystonia.

A review of functional surgery for dystonia is presented. Recently renewed interest in stereotaxy for dystonia has followed the resurgence of pallidotomy and the introduction of deep brain stimulation (DBS) in Parkinson's disease (PD) in the early 1990s. However, even since the 1950s, small series of patients treated with ablative surgery have been carefully studied, providing useful information, notably regarding the tolerability of surgery. In the setting of dystonia, thalamotomy was first performed with substantial benefits, but some authors outlined the great variability in outcome, and th

Fine structure and multicomponents of the electrically evoked otoacoustic emission in gerbil.

Like the acoustically evoked distortion product otoacoustic emissions (DPOAE), the amplitude spectrum of the extracochlear electrically evoked otoacoustic emission (EEOAE) also shows peaks and valleys, which are termed the fine structure (FS) of the EEOAE. The hypothesis that the FS of the EEOAE is generated by multiple wave interactions in the cochlea is investigated by examining the relationship between the FS and the multiple-delay components of the EEOAE. The bulla of the gerbil was exposed using a ventral surgical approach. One pole of a bipolar electrode was placed in the round window ni

Critical evaluation of electro-stimulation for management of female urinary incontinence.

Electro-stimulation has been reported to be effective in the relief of stress and urge urinary incontinence. The rates of cure and improvement brought about by pelvic floor electro-stimulation in patients with urinary incontinence are 30-50% and 60-90%, respectively. In clinical practice, vaginal, anal and surface electrodes are used for external, short-term stimulation, and sacral root stimulation for internal, chronic (long-term) stimulation. The effectiveness of electro-stimulation has been verified in a randomized, placebo-controlled study. However, its superiority over other conservative

Supplementary sensorimotor area epilepsy in adults.

Based on stimulation studies, observation of seizure phenomena in patients with epilepsy, and diagnostic studies documenting SSMA anatomic and functional abnormalities, there is evidence that epileptic involvement of the SSMA results in a set of clinical phenomena that include asymmetric tonic posturing of the extremities, abduction and elevation of the contralateral upper extremity, and speech arrest with preserved consciousness. Auras and vocalization occur less frequently. These seizures tend to be brief and to have an abrupt onset and cessation. They are more common during sleep than durin
