Caries clusters at lesion-severity thresholds: A Sierra Leone case study.

OBJECTIVES: Despite being almost entirely preventable, globally, dental caries is extremely prevalent. Moreover, dental caries will continue to present an even larger challenge for lower income countries, particularly those in the African context, as they transition to a more Western diet. Hence, epidemiological data providing insight into disease patterns and trends is critical to inform public health action. The purpose of this study was to examine dental caries clusters by caries detection threshold among 15-year-old adolescents in Sierra Leone, using data from the latest national survey, a

Transitions: Novel Study Methods to Understand Early HIV Risk Among Adolescent Girls and Young Women in Mombasa, Kenya, and Dnipro, Ukraine.

Transitions aims to understand the human immunodeficiency virus (HIV) risk at critical transition points in the sexual life course of adolescent girls and young women (AGYW) who engage in casual sex, transactional sex, and sex work. In this article, we present the Transitions study methods. The Transitions study has the following objectives: (1) to describe how the characteristics and length of the transition period and access gap vary across two epidemiological contexts (Mombasa, Kenya, and Dnipro, Ukraine); (2) to understand how the risk of HIV varies by length and characteristics of the tra

Importance of livestock diseases identified using participatory epidemiology in the highlands of Ethiopia.

Livestock are a major cornerstone for livelihoods of smallholders in the highlands of Ethiopia. However, infectious diseases are a key constraint in livestock production affecting productivity and food security. This study aimed to understand livestock producers' perception on importance and epidemiology of livestock diseases. Participatory tools using focus group discussions were used to identify and rank livestock species, major livestock diseases, and their epidemiological patterns in smallholder systems. The study covered 17 districts in four regions, with two or three kebeles (smallest ad

Exploring Listeria monocytogenes Transcriptomes in Correlation with Divergence of Lineages and Virulence as Measured in Galleria mellonella.

As for many opportunistic pathogens, the virulence potential of Listeria monocytogenes is highly heterogeneous between isolates and correlated, to some extent, with phylogeny and gene repertoires. In sharp contrast with copious data on intraspecies genome diversity, little is known about transcriptome diversity despite the role of complex genetic regulation in pathogenicity. The current study implemented RNA sequencing to characterize the transcriptome profiles of 33 isolates under optimal in vitro growth conditions. Transcript levels of conserved single-copy genes were comprehensively explore

Performance of centralized versus decentralized tuberculosis treatment services in Southern Brazil, 2006-2015.

BACKGROUND: Tuberculosis (TB) control programs face the challenges of decreasing incidence, mortality rates, and drug resistance while increasing treatment adherence. The Brazilian TB control program recommended the decentralization of patient care as a strategy for combating the disease. This study evaluated the performance of this policy in an area with high default rates, comparing epidemiological and operational indicators between two similar municipalities. METHODS: This study analyzed epidemiological and operational indicators on new cases of pulmonary tuberculosis reported in the Brazi

Age-structured gametocyte allocation links immunity to epidemiology in malaria parasites.

BACKGROUND: Despite a long history of attempts to model malaria epidemiology, the over-riding conclusion is that a detailed understanding of host-parasite interactions leading to immunity is required. It is still not known what governs the duration of an infection and how within-human parasite dynamics relate to malaria epidemiology. PRESENTATION OF THE HYPOTHESIS: Immunity to Plasmodium falciparum develops slowly and requires repeated exposure to the parasite, which thus generates age-structure in the host-parasite interaction. An age-structured degree of immunity would present the parasite

Surveillance of infectious diseases in the Italian military as pre-requisite for tailored vaccination programme.

Surveillance and control of infectious diseases in the Italian military include prompt reporting of all occurring cases and prophylaxis through compulsory vaccination practices. The last mandatory immunisation programme in the Italian military was launched in 1998 (more than 10 years after the previous programme was introduced in 1986-1987) and it was planned according to the epidemiological data herein reported. The incidence rates (number of cases x 100000 subjects) of infectious diseases notified during the period 1976-1980 were compared with the corresponding figures notified 15 years late

Ecological factors and genetic features are associated with ecological generalism in pathogenic tick-borne viruses.

Emerging and re-emerging tick-borne viruses (TBVs) have caused numerous outbreaks recently, yet the factors driving their emergence and pathogenicity in humans remain poorly understood across viral taxa. A large-scale analysis integrating epidemiological and genomic data of pathogenic TBVs reveal that highly pathogenic TBVs (HPTBVs) tend to exhibit ecological generalism. Specifically, HPTBVs infect a broader range of tick species, animal hosts, and ecotypes, which likely enhance their transmission to humans. Bird migration is identified as the primary driver for geographic spread of HPTBVs thr

Scope of Practice Laws Affecting Team-Based Heart Disease Care in Rural America: Policy Challenges and Opportunities.

BACKGROUND: Heart disease is the leading cause of death in the United States with a disproportionate impact in rural America, and health worker shortages contribute to these disparities. Team-based heart disease care models can help mitigate health worker shortages, but lack of role clarity and restrictive scope of practice laws can be a barrier to their implementation. METHODS: To better understand the intersection of licensed scope of practice laws with rural heart disease care, we performed a legal epidemiology analysis to identify and describe licensed scope of practice laws for 9 health

UV spectroscopy associated with machine learning as a complementary tool for the diagnosis of Ehrlichia canis in dogs.

Canine ehrlichiosis, caused by Ehrlichia canis, is a tick-borne disease with a global distribution that significantly affects the clinical and epidemiological landscape for dogs. Although laboratory diagnosis is essential, current diagnostic methods exhibit limitations in sensitivity, specificity, and cost-effectiveness. This study evaluated the efficacy of ultraviolet (UV) spectroscopy combined with machine learning techniques to differentiate serological samples that are positive and negative for E. canis. A total of 46 canine serum samples, classified via Enzyme-Linked Immunosorbent Assay/D

From Clinical Narrative to Diagnosis: Scalable Identification of Acquired Epilepsy.

Acquired epilepsy is a disabling, potentially preventable complication of acute brain injury (ABI). Yet, it remains a leading cause of new onset epilepsy in adults. Acquired epilepsy is a particularly challenging subtype of epilepsy to identify, as the ABI and its acute consequences can confound the later diagnosis of acquired epilepsy. We identified a retrospective cohort of patients with ABI (N=828) and optimized a general epilepsy algorithm to extract relevant keywords. We confirmed that applying a broad epilepsy phenotyping algorithm to a high risk, complex population like acute brain inju

Smad4 restricts injury-provoked biliary proliferation and carcinogenesis.

Cholangiocarcinoma (CCA) is a deadly and heterogeneous type of cancer characterized by a spectrum of epidemiologic associations as well as genetic and epigenetic alterations. We seek to understand how these features inter-relate in the earliest phase of cancer development and through the course of disease progression. For this, we studied murine models of liver injury integrating the most commonly occurring gene mutations of CCA - including Kras, Tp53, Arid1a and Smad4 - as well as murine hepatobiliary cancer models and derived primary cell lines based on these mutations. Among commonly mutate

West Nile virus triggers intestinal dysmotility via T cell-mediated enteric nervous system injury.

Intestinal dysmotility syndromes have been epidemiologically associated with several antecedent bacterial and viral infections. To model this phenotype, we previously infected mice with the neurotropic flavivirus West Nile virus (WNV) and demonstrated intestinal transit defects. Here, we found that within 1 week of WNV infection, enteric neurons and glia became damaged, resulting in sustained reductions of neuronal cells and their networks of connecting fibers. Using cell-depleting antibodies, adoptive transfer experiments, and mice lacking specific immune cells or immune functions, we show th

A Predictive Model for the Early Death of Breast Cancer With Synchronous Liver Metastases: A Population-Based Study.

BACKGROUND: Breast cancer liver metastasis (BCLM) is a severe condition often resulting in early death. The identification of prognostic factors and the construction of accurate predictive models can guide clinical decision-making. METHODS: A large sample of data from the Surveillance, Epidemiology, and End Results (SEER) database was analyzed, including 3711 patients diagnosed with de novo BCLM between 2010 and 2015. Predictive models were developed using histograms, and stepwise regression addressed variable collinearity. Internal validation was performed, and results were compared to simil

Predicting cutaneous malignant melanoma patients' survival using deep learning: a retrospective cohort study.

BACKGROUND: Cutaneous malignant melanoma (CMM) has the worst prognosis among skin cancers, especially metastatic CMM. Predicting its prognosis accurately could direct clinical decisions. METHODS: The Surveillance, Epidemiology, and End Results database was screened to collect CMM patients' data. According to diagnosed time, patients were subdivided into three cohorts, train cohort (diagnosed between 2010 and 2013), validation cohort (diagnosed in 2014), and test cohort (diagnosed in 2015). Train cohort was used to train deep learning survival model for cutaneous malignant melanoma (DeepCMM).

Sarcopenia, osteoporosis and frailty.

Muscles and bones are intricately connected tissues displaying marked co-variation during development, growth, aging, and in many diseases. While the diagnosis and treatment of osteoporosis are well established in clinical practice, sarcopenia has only been classified internationally as a disease in 2016. Both conditions are associated with an increased risk of adverse health outcomes such as fractures, dysmobility and mortality. Rather than focusing on one dimension of bone or muscle mass or weakness, the concept of musculoskeletal frailty captures the overall loss of physiological reserves i

Effects of boosting and waning in highly exposed populations on dengue epidemic dynamics.

Sequential infection with multiple dengue virus (DENV) serotypes is thought to induce enduring protection against dengue disease. However, long-term antibody waning has been observed after repeated DENV infection. Here, we provide evidence that highly immune Nicaraguan children and adults (n = 4478) experience boosting and waning of antibodies during and after major Zika and dengue epidemics. We develop a susceptible-infected-recovered-susceptible (SIRS-type) model that tracks immunity by titer rather than number of infections to show that boosts in highly immune individuals can contribute to

Model-Based Estimation of Expected Time to Cholera Extinction in Lusaka, Zambia.

The developing world has been facing a significant health issue due to cholera as an endemic communicable disease. Lusaka was Zambia's worst affected province, with 5414 reported cases of cholera during the outbreak from late October 2017 to May 12, 2018. To explore the epidemiological characteristics associated with the outbreak, we fitted weekly reported cholera cases with a compartmental disease model that incorporates two transmission routes, namely environment-to-human and human-to-human. Estimates of the basic reproduction number show that both transmission modes contributed almost equal

Modeling COVID-19 transmission between age groups in the United States considering virus mutations, vaccinations, and reinfection.

The in-depth understanding of the dynamics of COVID-19 transmission among different age groups is of great interest for governments and health authorities so that strategies can be devised to reduce the pandemic's detrimental effects. We developed the SIRDV-Virulence (Susceptible-Infected-Recovered-Dead-Vaccinated-Virulence) epidemiological model based on a population balance equation to study the effects virus mutants, vaccination strategies, 'Anti/Non Vaxxer' proportions, and reinfection rates to provide methods to mitigate COVID-19 transmission among the United States population. The influence of landscape structure on the dispersal pattern of yellow fever virus in the state of São Paulo.

Man-made changes to the landscape play a crucial role in altering the epidemiologic patterns of infectious diseases, mainly as a result of pathogen spillover. Sylvatic yellow fever is ideally suited to modeling of this phenomenon as the risk of transmission of the disease as well as its circulation and dispersal are associated with forest fragmentation. In this study we investigated the temporal dispersal pattern of yellow fever virus (YFV) by means of confirmed cases of epizootics in non-human primates in municipalities in the state of São Paulo where there was no recommendation for vaccinati
