Primary Monoblock Inset Reverse Shoulder Arthroplasty Resulted in Decreased Pain and Improved Function.

BACKGROUND: The first-generation, lateral-center-of-rotation reverse shoulder arthroplasty (RSA) modular design has demonstrated durable early-, mid-, and long-term outcomes. The second-generation monoblock implant shares a similar design but eliminates the modular junction and facilitates inset placement within the metaphysis to avoid humeral-sided junctional failures and facilitate metaphyseal press-fit. However, no paper has specifically examined the radiographic findings and improvements in pain and function after the use of this next generation design. QUESTIONS/PURPOSES: (1) After secon

Function-preserving reduction and fixation of unstable Jefferson fractures using a C1 posterior limited construct.

STUDY DESIGN: This is a retrospective, clinical, and radiologic study of posterior reduction and fusion of the C1 arch in the treatment of unstable Jefferson fractures. OBJECTIVE: The aim of the study was to describe a new motion-preserving surgical technique in the treatment of unstable Jefferson fracture. SUMMARY OF BACKGROUND DATA: The management of unstable Jefferson fractures remains controversial. The majority of C1 fractures can be effectively treated nonoperatively with external immobilization unless there is an injury to the transverse atlantal ligament (TAL). Conservative treatment

Surgical treatment of the severe sequelae of infantile septic arthritis of the hip.

We retrospectively reviewed 45 hips in 43 patients with severe sequelae of infantile septic arthritis of the hip to compare the efficacy of various hip reconstructive and salvage surgeries, and to propose an algorithmic treatment protocol for the different types. Ten hips were classified as Choi Type IIIA, three as Type IIIB, 14 as Type IVA, and 18 as Type IVB sequelae. A total of 78 hip surgeries and 18 limb-length equalizations (three contralateral femoral epiphysiodesis and 15 ipsilateral femoral and/or tibial lengthenings) were done. The first surgical reconstructions were done at an avera

[Malpractice in the treatment of supracondylar humeral fractures in children - experience of the arbitration office of the Northern German Medical Boards].

INTRODUCTION: Arbitration offices ("Schlichtungsstellen") in Germany are expert panels for the extrajudicial resolution of malpractice claims. The performance of arbitration panel proceedings ("Schlichtungsverfahren") is based on the German medical and insurance jurisdiction. In Germany, and in the United States likewise, malpractice claims involving children concern in most cases fracture treatment followed by appendicitis. Out of 242 panel proceedings with the background of fracture treatment in children malpractice was confirmed in 144 cases (60%). The overall ratio: number of confirmed mal

Somatic versus sympathetic mediated chronic limb pain. Experience and treatment options.

It has been helpful in our practice to separate somatic from sympathetic-mediated peripheral nerve pain. We would recommend application of the new nomenclature of type I complex regional pain (sympathetic dystrophy) and type II complex regional pain (causalgia) (see Table 1). We believe it is essential that both of these conditions be separated into their early and late phases and that the treatment alternatives be customized for the individual patient and the peripheral nerve involved. If a cast, pin, or external fixation apparatus is associated with peripheral nerve pain, the offending appar

Modified Dunn Procedure for Open Reduction of Chronic Slipped Capital Femoral Epiphysis.

BACKGROUND: Abnormal femoral head anatomy following moderate-to-severe slipped capital femoral epiphysis (SCFE) can lead to femoroacetabular impingement and premature osteoarthritis4-10. Surgical correction at the deformity site through capital reorientation has the potential to fully ameliorate this but has traditionally been associated with high rates of osteonecrosis11-15. The modified Dunn procedure has the potential to restore anatomy in hips with SCFE while protecting the blood supply to the femoral head. DESCRIPTION: A surgical dislocation of the hip is performed according to the techn

Arthroscopic Reduction and Internal Fixation of an Osseous Bankart Lesion.

UNLABELLED: Operative treatment of anterior glenohumeral instability is challenging, particularly with the presence of an anterior glenoid rim fracture, also called an "osseous Bankart lesion." Successful reduction and fixation of the lesion has been shown to greatly reduce the risk of recurrent dislocations while achieving osseous union and normalization of glenoid anatomy1. DESCRIPTION: The current surgical video article outlines a technique for an osseous Bankart repair in a patient with a displaced fracture as well as substantial pain and instability. First, the amount of bone loss is mea

[What is your diagnosis? Hypertrophic olivary degeneration].

BACKGROUND: Stroke is a leading cause of disability, often accompanied by unilateral spatial neglect (USN), which severely impairs recovery. Traditional assessments like paper-pencil tests provide limited insights into behaviors and eye-hand coordination during real-world tasks. Advances in hand pose estimation and eye tracking in combination with augmented reality (AR) offer potential for data-driven assessments of naturalistic interactions. OBJECTIVE: This proof-of-concept study presents and evaluates a multimodal behavioral tracking system that captures gaze, body, and hand movements durin

Clinical Outcomes and Complications of the Suzuki Frame in Digital Fractures: A Systematic Review and Meta-Analysis.

Intraarticular fractures of the phalanges and thumb base are challenging to manage due to the risks of stiffness and loss of motion. The Suzuki pins and rubbers traction system provides dynamic external fixation that maintains reduction while allowing early mobilisation. No prior systematic review and meta-analysis (SRMA) has comprehensively evaluated its outcomes. This study aimed to summarise radiographic union, functional recovery and complication rates to inform clinical decision-making. In accordance with Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) 2020 gui

Bilateral Stifle Arthrodesis as a Salvage Procedure for End-Stage Bilateral Grade IV Lateral Patellar Luxation in a Dog: A Case Report.

Grade IV lateral patellar luxation is an advanced orthopedic condition that may be associated with severe skeletal deformities, chronic soft-tissue contracture, and irreversible loss of normal stifle function. Although reconstructive procedures are considered the treatment of choice in most dogs, some end-stage cases may not be amenable to conventional correction. This report describes the clinical presentation, surgical management, and long-term outcome of a young dog with severe bilateral grade IV lateral patellar luxation treated with single-stage bilateral stifle arthrodesis. A 1-year-old,

Long-Term Outcomes of Prolonged Atlantoaxial Rotatory Fixation Managed Without Halo Vest Immobilization in a Child.

Atlantoaxial rotatory fixation (AARF) in children usually responds to conservative treatment, including rest and cervical collar immobilization. However, some patients experience persistent or recurrent deformity and require prolonged immobilization or more intensive treatment. Long-term outcomes of prolonged AARF managed without halo vest immobilization have rarely been reported. We report the case of an eight-year-old boy who presented with acute neck pain and torticollis without an identifiable trigger. Initial treatment with a cervical collar at a local clinic failed to improve his symptom

Transient acute kidney injury following femoral fracture in mice : the role of sympathetic activation.

AIMS: Femoral shaft fractures are commonly associated with high-energy trauma, and affect individuals of all ages. Kidney dysfunction frequently complicates fracture healing, and trauma-related acute kidney injury (TRAKI) elevates the risk of adverse outcomes. The mechanisms of post-fracture kidney injury, particularly the involvement of sympathetic activation, remain poorly understood. We aimed to define the impact of femoral fracture on the kidney and investigate mechanistically the potential contribution of sympathetic activation. METHODS: A total of 46 female C57BL/6J mice were subjected

Outcomes of modular diaphyseal vs. nonmodular metaphyseal reverse total shoulder arthroplasty stem for 3- and 4-part proximal humerus fractures.

BACKGROUND: Proximal humerus fractures (PHFs) are increasingly prevalent in the aging population, with reverse shoulder arthroplasty (rTSA) emerging as a preferred surgical treatment for complex fracture patterns. Recent advancements have introduced shorter, metaphyseal-fixation humeral stems as alternatives to traditional modular diaphyseal stems. However, limited data exist comparing the clinical outcomes of these designs following rTSA for PHF. METHODS: A retrospective analysis of 99 patients undergoing rTSA for acute three- or four-part PHFs between 2014 and 2024 was conducted. Patients r

Surgical approach selection algorithm based on fracture patterns for scapular body fractures.

BACKGROUND: Scapular body fractures are uncommon injuries often managed nonoperatively. However, in cases with significant displacement, surgical intervention may be required to restore shoulder function. While various surgical approaches have been described, no standardized method exists for selecting the optimal approach based on fracture morphology. The purpose of this study is to retrospectively evaluate surgical treatment and its outcomes for scapular body fractures, and to validate an appropriate method for selecting surgical approaches according to fracture patterns of the scapular body

First rib resection and corrective clavicle osteotomy using the infraclavicular approach for thoracic outlet syndrome due to clavicle malunion: A case report.

BACKGROUND: Thoracic outlet syndrome can develop following the malunion of a clavicle midshaft fracture. To date, thoracic outlet syndrome complicated by clavicle malunion is typically treated with either first rib resection or corrective clavicle osteotomy; however, there have been no reports of these two procedures being performed simultaneously using the same approach. We present the first documented case of thoracic outlet syndrome caused by clavicle malunion treated by simultaneous first rib resection and corrective clavicle osteotomy through a single infraclavicular approach. Meta-analysis of complications and functional outcomes in cosmetic limb lengthening.

INTRODUCTION: Short stature, defined as height below the 3rd percentile for age and gender, affects approximately 200 million people worldwide. It can have profound psychological effects, influencing self-esteem, social interactions, and overall life satisfaction, particularly during adolescence. These individuals often face challenges in both professional and romantic lives, increasing their risk of depression and suicidal ideation. Surgical interventions such as limb lengthening can provide life-changing results for individuals facing psychological distress due to short stature. This meta-an
