Classical conditioning model of panic disorder: response to Wolpe and Rowan.

We refute Wolpe and Rowan's (1989) classical conditioning theory of panic disorder based upon several lines of evidence presented above. While there is no doubt that hyperventilation and conditioning are important factors in the production of panic attacks, Wolpe and Rowan's (1989) hypothesis is inconsistent with recent data concerning the limited role of hyperventilation and the importance of cognitive mediation in the production of panic attacks.

Biology of anxiety.

Our understanding of the biological basis of anxiety is far from complete, although our knowledge of both the neuropharmacologic and molecular basis of anxiety has increased. This article reviews our current knowledge of the possible biological basis of generalized anxiety disorder and panic disorder.

Panic attacks during relaxation and relaxation-induced anxiety: a hyperventilation interpretation.

This paper explains how a hyperventilation theory of panic disorder accounts for panic attacks during relaxation and relaxation-induced anxiety. The explanation is based on the observation that chronic hyperventilators maintain a steady state of low pCO2 (arterial carbon dioxide tension) and are, therefore, sensitive to relatively small increases in ventilation when metabolism is low and to relatively sudden reductions in metabolism when ventilation is relatively constant. Thus, if minute volume of air breathed remains constant while the metabolic production of CO2 decreases, as in the case of

Mitral valve prolapse, panic states, and anxiety. A dilemma in perspective.

The inter-relationship of mitral valve prolapse and psychiatric symptoms as a model for the approach to illness for psychiatrists is considered. The clinical usefulness of the concept of mitral valve prolapse is discussed and placed in perspective to consultation-liaison psychiatry. The special role of an integrative, multidimensional model of all illness is emphasized.

Psychotherapeutic approaches to the treatment of panic attacks, hypochondriasis and agoraphobia.

Various theoretical orientations suggest different treatment approaches to the agoraphobic syndrome. In this paper a self-psychology formulation is applied to the treatment of panic attacks, hypochondriasis and agoraphobia. When these symptoms are viewed as manifestations of self-fragmentation, their treatment can be conceptualized as a process of improving self-cohesiveness. The initial management of symptoms sets in motion a self-object transference which is the foundation of treatment. As therapy proceeds, the defences against affect which are associated with a primary defect in anxiety reg

Is panic disorder a distinct diagnostic entity? A critical review of the borders of a syndrome.

The evidence supporting the existence of panic disorder as a distinct clinical entity is critically examined, as are the current criteria for panic disorder in DSM-III. It is argued that the current definition of a panic attack is imprecise and that the borders and overlap of panic disorder with other psychiatric disorders raise broader questions as to what is meant by a distinct psychiatric disorder. DSM-III "panic disorder" defines an ideal type that may be more relevant for research purposes than clinical. In defining fairly homogeneous "pure" cases, it overlooks the prevalence and importan

Reduction in hypochondriasis with treatment of panic disorder.

Hypochondriasis was assessed in 60 patients with panic disorder and agoraphobia using the Illness Behavior Questionnaire. Before treatment, IBQ hypochondriasis scores were similar to those of a group of hypochondriacal psychiatric patients. In patients who improved with treatment, significant reductions in somatic preoccupation, disease phobia, and disease conviction occurred. Hypochondriasis appears to be a prominent feature of panic disorder and agoraphobia, and responds to treatment of the primary conditions. Our findings underscore the importance of providing adequate treatment and thereby

Effects of public education about breast cancer and breast self-examination.

In a field experiment severity of and susceptibility to breast cancer were varied in four different persuasive messages about breast cancer and breast self-examination (BSE). The purpose of this study was to find out whether such health messages in a real-life setting had an effect on knowledge about symptoms, attitudes and behavior relating to BSE. The second goal of this study was to investigate whether fear, aroused by these persuasive communications, had a significant role in influencing the recommended behavior (BSE). In spite of a successful manipulation of seriousness and susceptibility…

A review of blood donor motivation and recruitment.

Sixty English-language articles on the motivation and recruitment of blood donors and nondonors are reviewed and evaluated. Apparently researchers have been finding essentially the same results for approximately twenty years. Motivations to donate are: altruism/humanitarian, personal or family credit, social pressure, replacement and reward. Motivations not to donate are: fear (of needle, sight of blood, weakness, finger or ear pricking), medical excuses, reactions, apathy, and inconvenience. Deeper analysis of motivation suggests that donors may unconsciously desire a "pat-on-the-back" for th…

Vocalization in juvenile crocodilians.

Evidence is presented that the grunts and distress calls of juvenile alligators (Alligator mississipiensis) are examples of graded calls, which are given in different form depending largely on contest. Representative sonagrams of the distress calls of three other species of crocodilians (Caiman crocodilus, Crocodylus niloticus, and Crocodylus siamensis) are presented and compared with sonagrams of alligator distress calls. These findings are discussed in light of current knowledge of the bioacoustics of other vertebrates.

Sleep Disturbances in Survivors of Childhood Acute Lymphoblastic Leukemia: A Cross-Sectional Study.

OBJECTIVES: Survivors of childhood cancer are at risk of sleep disturbances, an often, neglected aspect of holistic health. This study was carried out to determine the prevalence of sleep disturbances in survivors of pediatric acute lymphoblastic leukemia (ALL) and to identify any risk factors for the same. METHODS: Survivors of ALL aged 6-18 years, who had completed their treatment, at least two or more years ago, were enrolled. The Child Sleep Habits Questionnaire (CSHQ) was used to assess the sleep problems. RESULTS: Out of 89 childhood survivors of ALL, 42.7% had sleep disturbances. Fear…

Pro-Vax, Anti-Vax, or Shades of Gray? Segmenting Consumers Based on Attitudes to Vaccination.

Vaccination hesitancy remains a prominent global health concern, as recognized by the World Health Organization (WHO). This quantitative study, conducted on a representative sample of the Belgian population (n = 1,030), explores consumer segmentation concerning vaccination attitudes during the COVID-19 pandemic in 2021, identifying five distinct clusters. While 39.6% of the surveyed consumers broadly accepted the COVID-19 vaccine (Convinced Pro-Vaxxers), our analysis revealed that 17.1% were skeptical (Skeptics), 12.7% were fearful of the inoculation procedure (Fearful Doubters), and 7.1% were…

[Distrust of vaccination: Why?].

Vaccines have saved millions of lives and reduced the severity of many infections. A reduction in vaccination coverage is now reflected in the re-emergence of epidemics of mumps, pertussis, measles and chickenpox. Many people do not recognize the effectiveness of vaccination and fear the side effects. The main concern is the safety of vaccines. Lack of information weighs less than lack of trust in health authorities. The greater responsibility of the individual and the respect for his free will, may lead the authorities to a less vigourous promotion of the "vaccination duty" which is also a so…

The dynamics of the production of AIDS-related stigma among pregnant women living with HIV/AIDS in Rio de Janeiro, Brazil.

The study analyses the relationship between AIDS-related stigma and the processes of discrimination prior to diagnosis among pregnant women … ability and autonomy to exercise sexual and reproductive rights. Being diagnosed with HIV triggered fear of moral judgment and of breakdown in social and family support networks. Given these fears, pregnant women living with HIV/AIDS opt for concealment of the diagnosis. It is necessary for health services, NGOs and government agencies to work together …
