Acute and subacute polyhydramnios in singleton pregnancies.

Over a 10-year period when 51,022 singleton infants were delivered, 19 pregnancies (1 in 2,685) were complicated by acute polyhydramnios 17 (1 in 3,000) by subacute polyhydramnios and 501 (1 in 102) by chronic polyhydramnios. The incidence of major congenital malformations in singleton pregnancies associated with acute polyhydramnios was 63% and the perinatal mortality rate was 74%. When subacute polyhydramnios occurred in singleton pregnancies, the incidence of major congenital malformations was 65%, similar to acute polyhydramnios, but the perinatal mortality rate was only 35%. The comparabl

The prevalence at birth of congenital malformations in communities near the Hanford site.

The authors examined the prevalence of congenital malformations among births in Benton and Franklin counties, in southeastern Washington State, from 1968 through 1980. The Hanford Site is in this area and serves as a major employer. In addition, various agriculturally and chemically related activities are in the area. Hospital and vital records were used to identify 454 malformation cases among 23,319 births; this yielded a malformation rate of 19.6 per 1,000 births, a rate similar to those reported in other studies. The rates of specific malformations ascertained during the first year of life

Incidence of anencephalus and spina bifida in Greece.

A cross-sectional national survey of 11,048 births in the whole of Greece in April 1984 identified 7 cases of anencephalus and 9 of spina bifida. There was thus an incidence of 1.45 neural tube defects per 1,000 total births. This is the first geographically defined population study from southern Europe. In comparison with hospital-based data from the 2 countries on its borders, the Greek rates are rather similar to those of Yugoslavia but much lower than reports from Turkey.

Pulmonary edema in severe preeclampsia-eclampsia: analysis of thirty-seven consecutive cases.

During approximately a 9-year period, 37 severe preeclamptic-eclamptic patients had pulmonary edema for an incidence of 2.9%. The incidence was significantly higher in older patients (p less than 0.0001) and in multigravid patients (p less than 0.05). Eleven (30%) had antepartum edema with 10 (90%) of the 11 having preexisting chronic hypertension. Twenty-six (70%) had postpartum edema with an average onset of 71 hours post partum. The majority of these patients had excessive colloid and crystalloid infusions for various medical, surgical, and obstetric complications. There were four maternal

The effect of placenta previa localization upon maternal and fetal-neonatal outcome.

The present study evaluated the association between maternal and fetal-neonatal outcome in women with placenta previa (PP) and the site of PP, i.e. low lying, marginal, partial or total. The study group was composed of 154 women (159 newborns) and the control group of 300 woman (305 newborns). Of the various parameters examined, the following 3 were associated with PP localization: advanced maternal age was associated with major types of PP, while focal accretion of the placenta and neonatal mortality in cases of vaginal delivery were associated with minor types of PP. No connection was found

Malnutrition and gastroenteritis among Jamaican children.

In response to Resolution No. 7 of the Fifth Caribbean Health Minsters Conference of February 1973, the Executive Secretary convened a meeting of a stering committee comprising members of staff of CFNI, the Faculty of Medicine, U.W.I., PAHO, WHO and FAO. Arising from this, it was decided to produce the Strategy and Plan of Action of Combat Gastroenteritis and Malnutrition in children under two years of age. The implementation of that programme was launched in Jamaica in January 1976 by Dr. A.W. Patterson, the Principal Medical Officer of the Ministry of Health and Environmental Control. This p

Is it safer to intubate premature infants in the delivery room?

OBJECTIVES: Early nasal continuous positive airway pressure (ENCPAP) has recently emerged in neonatal units as an acceptable alternative to routine intubation and mandatory ventilation. The risks and benefits of ENCPAP have yet to be established. In this study, we aimed to examine variables that influenced the decision to initiate ENCPAP in the delivery room (DR). We also explored potential harmful effects of early intubation and examined whether unsuccessful ENCPAP attempts might subject infants to any unforeseen morbidity. METHODS: All inborn very low birth weight (VLBW) infants admitted to

Sex differences in early and late neonatal all-cause mortality and disease burden in Kazakhstan, 2010-2023: A population-based analysis of Global Burden of Disease estimates.

BACKGROUND: Neonatal mortality remains a major contributor to child mortality globally, with the greatest burden concentrated in the early neonatal period. Sex differences in neonatal survival are consistently reported, yet population-level evidence on sex-specific patterns of all-cause burden across neonatal subperiods in Central Asia remains limited. This study aimed to quantify temporal trends and sex differences in early (0-6 days) and late (7-27 days) neonatal all-cause mortality and disease burden in Kazakhstan from 2010 to 2023, and to examine projected trajectories to 2030. METHODS: W

The magnitude and population burden of educational inequalities in adverse birth outcomes.

We aimed to describe the magnitude and population burden of inequality in adverse birth outcomes by maternal education (five categories). Data from 639,007 singleton births of gestational age ≥ 24 weeks or, if missing, birthweight ≥ 500 g were included from the National registry of the Netherlands, 2016-2019. One in six Dutch births had an adverse outcome. Each step down the educational ladder was associated with higher adverse outcome rates. Inequalities in stillbirth and neonatal mortality rates were large between the highest (Master's or higher) and lowest (primary) educated groups (rate ra

Machine learning approaches to identify neonates and young children at risk for postdischarge mortality in Dar es Salaam, Tanzania and Monrovia, Liberia.

BACKGROUND: The time after hospital discharge carries high rates of mortality in neonates and young children in sub-Saharan Africa. Previous work using logistic regression to develop risk assessment tools to identify those at risk for postdischarge mortality has yielded fair discriminatory value. Our objective was to determine if machine learning models would have greater discriminatory value to identify neonates and young children at risk for postdischarge mortality. METHODS: We conducted a planned secondary analysis of a prospective observational cohort at Muhimbili National Hospital in Dar

Live births and deaths of neonates born to adolescent mothers: analysis of trends and associations from a population study in a region of a middle-income country.

BACKGROUND: Adolescent pregnancy is associated with adverse outcomes, and although there has been a global decline in the incidence of teenage pregnancies and neonatal deaths, the absolute number remains significant. This study aimed to evaluate temporal trends in live births and neonatal deaths from adolescent mothers, as well as to identify the effect of adolescent pregnancy on neonatal death. METHODS: This is a population-based study of all live births from mothers residing in Sao Paulo state, Brazil, between 2004 and 2020. The Prais-Winsten model was used to analyze annual trends for live

Effects of workforce volatility, determinants of infant deaths and resilience in SUS Primary Care.

This article aims to analyze the effects of the volatility of the Primary Health Care (PHC) workforce in the Unified Health System (SUS) on the resilience of services and the reduction in infant deaths. We used the relationship between the PHC workforce and child-sensitive indicators: vaccination (poliomyelitis and measles) and infant deaths. This ecological time series study adopted indicators relating to the workforce, vaccination coverage, and infant deaths from preventable causes from all Brazilian states between 2017 and 2022, totaling a balanced panel with 162 pieces of information in 11

Quality of neonatal health care in comprehensive specialized hospitals, Amhara Region, Ethiopia: a retrospective study with neonatal death audit.

BACKGROUND: Neonatal health is one of the targets for the sustainable development goals (SDGs) that aim to reduce neonatal mortality to at least as low as 12 per 1 000 live births in 2030. However, the world is not on track to achieve this target. The problem has worsened in many low-income countries, including Ethiopia, due to a fragile health system, as well as health crises such as the COVID-19 pandemic, conflict, food insecurity and climate change. According to the Mini Ethiopian Demographic Health Survey, neonatal mortality is unacceptably high in Ethiopia in general, and in Amhara region

Accountability for maternal and newborn health: Why measuring and monitoring broader social, political, and health system determinants matters.

This article offers four key lessons learned from a set of seven studies undertaken as part of the collection entitled, "Improving Maternal Health Measurement to Support Efforts toward Ending Preventable Maternal Mortality". These papers were aimed at validating ten of the Ending Preventable Maternal Mortality initiative indicators that capture information on distal causes of maternal mortality. These ten indicators were selected through an inclusive consultative process, and the research designs adhere to global recommendations on conducting indicator validation studies. The findings of these
