[Anemia: new developments and diagnostic and management strategies].

Anemia is a major public health problem that affects approximately 25% of the world's population. Its prevalence is increased in certain populations: it affects 40% of pregnant women, 42% of children under 5 years old and it increases with age from 50 years old. Anemia can be an emergency in case of hemorrhage or acute hemolysis, but it is most often chronic. Anemia can be constitutional or acquired. In the second case, iron or vitamin deficiencies are the most frequent causes. Anemia can also be a diagnostic pointing to hypothyroidism, inflammatory disease, or cancer. In this article, we prov

Regulation of iron absorption in infants.

BACKGROUND: Iron programs in low- and middle-income countries often target infants and young children. Limited data from human infants and mouse models suggest that homeostatic control of iron absorption is incomplete in early infancy. Excess iron absorption during infancy may have detrimental effects. OBJECTIVES: Our aims were to 1) investigate determinants of iron absorption in infants aged 3-15 mo and assess whether regulation of iron absorption is fully mature during this period and 2) define the threshold ferritin and hepcidin concentrations in infancy that trigger upregulation of iron a

Multiple micronutrient supplements versus iron-folic acid supplements and maternal anemia outcomes: an iron dose analysis.

Antenatal multiple micronutrient supplements (MMS) are more effective than iron and folic acid (IFA) supplements in reducing adverse pregnancy outcomes. Questions remain, however, about the ability of MMS to prevent anemia as effectively as IFA, especially at a lower dose of daily iron and in areas of high anemia prevalence. Analyzing data from 11 trials from a recent Cochrane review, we compared MMS to IFA, delivering either 30 or 60 mg of iron, in sustaining hemoglobin and preventing third trimester anemia and iron deficiency anemia (IDA), accounting for daily iron dose, total supplemental i

Immediate intravenous iron administration improves anaemia recovery following total knee arthroplasty: A propensity-matched analysis.

BACKGROUND AND OBJECTIVES: Patients who undergo total knee arthroplasty (TKA) have a risk of postoperative anaemia. This observational study evaluated whether single-dose intravenous ferric carboxymaltose (FCM) administered immediately after TKA facilitates the correction of anaemia. MATERIALS AND METHODS: We retrospectively analysed 722 patients who underwent primary TKA. The FCM group receiving 1000 mg intravenous FCM within one postoperative hour was compared with the non-FCM group that did not receive the medication. A propensity score matching with multiple logistic regression analysis w

Tolerable upper intake level of iron damages the intestine and alters the intestinal flora in weaned piglets.

Iron supplementation has been an intervention to improve iron storage and prevent iron deficiency anemia in weaned piglets and the recommended nutrient intake (RNI) and tolerable upper intake levels (UL) of iron have been established. The purpose of this study is to investigate the potential harm of UL iron to the gut and microbes of weaned piglets. Thirty 23 day old weaned piglets were assigned to three dietary treatments: a basal diet supplemented with 100 (RNI), 300, and 3000 (UL) mg FeSO4 per kg diet for 28 days. Then, we used the intestinal porcine epithelial cell line (IPEC-1) as a cell

Clinical relevance of erythrocyte ferritin in microcytic anemias.

BACKGROUND: Erythrocyte ferritin (EF) reflects the balance between iron supply and its utilization for hemoglobin synthesis. This balance is altered in microcytosis. We aimed to evaluate the diagnostic value of both EF and the ratio (FRR) plasma ferritin (PF)/EF in these disorders. METHODS: A total of 231 subjects participated in the study. Samples from 93 adult patients with different causes of microcytosis, 57 healthy subjects and 81 full-term newborns were analyzed to determine EF and PF concentrations and other hematological parameters. RESULTS: In patients with iron deficiency, and in c

Effects of daily iron supplementation in primary-school-aged children: systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Anemia is an important public health and clinical problem. Observational studies have linked iron deficiency and anemia in children with many poor outcomes, including impaired cognitive development; however, iron supplementation, a widely used preventive and therapeutic strategy, is associated with adverse effects. Primary-school-aged children are at a critical stage in intellectual development, and optimization of their cognitive performance could have long-lasting individual and population benefits. In this study, we summarize the evidence for the benefits and safety of daily iro

Advances in understanding the molecular basis for the regulation of dietary iron absorption.

PURPOSE OF REVIEW: The number of newly identified genes participating in the regulation of iron homeostasis has continued to expand at a remarkable pace. The roles for many have begun to be elucidated and there is an increasing indication that hepatocytes play a central role in determining the level of intestinal iron absorption. Total body iron homeostasis is dependent upon carefully regulated absorption of dietary iron, thus these genes are of fundamental importance in understanding of pathophysiology of such common disorders as hereditary hemochromatosis (HH) and the anaemia of chronic dise

[Treatment of disorders of iron metabolism].

When prescribing oral iron preparations, consideration must be given in particular to the question of bioavailability. In this connection, ferrous sulfate preparations are to be preferred. The simultaneous ingestion of tea, coffee or dairy products reduces the absorption of iron, while ascorbic acid increases it. Gastrointestinal side effects of oral iron substitution are usually dose-dependent. If absorption or mobilization disorders, gastrointestinal diseases or signs of intolerance to oral administration are present, iron must be administered via the parenteral route. To avoid toxicity, acc

[Fe and Cu content in the liver, spleen, kidney and heart of growing rats with variable Fe and Cu supply].

In the course of a series of experiments on factorial Fe and Cu supply (0, 25, 250 and 625 micrograms Fe/g; 0, 10, 100 and 250 micrograms Cu/g), t effects on Fe and Cu contents of liver, spleen, kidney, and heart of growing rats were examined. Total contents as well as concentrations in these organs depended not only on the supply with the respective trace element but also on interactions between both trace elements. Iron doses of 250 and 625 micrograms/g resulted in an accumulation of Fe in the liver of Cu-deficient animals, but in reduced Fe contents in spleen, kidney, and heart. Following s

Effect of metal ions in the culture medium on the stearoyl-coenzyme A desaturase activity of Mycobacterium phlei.

A particulate fraction prepared from Mycobacterium phlei grown in a metal-deficient medium exhibited a greatly reduced activity of stearoyl-CoA desaturase compared to that from normally grown cells. Metal deficiency, however, had no effect on the FAD-dependent NADPH-cytochrome C reductase activity, which has been suggested to participate in the desaturation process. When the cells were grown in the deficient medium supplemented with both Fe2+ and Mg2+, the desaturase activity was restored to the normal level. Supplementation with Mg2+ alone promoted growth but did not restore the desaturase ac

Comparison of the effects of high dietary iron levels on bone microarchitecture responses in the mouse strains 129/Sv and C57BL/6J.

Iron is an essential nutrient for all living organisms. Both iron deficiency and excess can be harmful. Bone, a highly metabolic active organ, is particularly sensitive to fluctuations in iron levels. In this study, we investigated the effects of dietary iron overload on bone homeostasis with a specific focus on two frequently utilized mouse strains: 129/Sv and C57BL/6J. Our findings revealed that after 6 weeks on an iron-rich diet, 129/Sv mice exhibited a decrease in trabecular and cortical bone density in both vertebral and femoral bones, which was linked to reduced bone turnover. In contras

Iron regulatory protein two facilitates ferritinophagy and DNA damage/repair through guiding ATG9A trafficking.

Trace elemental iron is an essential nutrient that participates in diverse metabolic processes. Dysregulation of cellular iron homeostasis, both iron deficiency and iron overload, is detrimental and tightly associated with disease pathogenesis. IRPs-IREs system is located at the center for iron homeostasis regulation. Additionally, ferritinophagy, the autophagy-dependent ferritin catabolism for iron recycling, is emerging as a novel mechanism for iron homeostasis regulation. It is still unclear whether IRPs-IREs system and ferritinophagy are synergistic or redundant in determining iron homeost

Preparation and Performance Evaluation of Polysaccharide-Iron Complex of Eucommia ulmoides.

An innovative iron supplement crucial for treating iron-deficiency anemia was developed in this study. Polysaccharide was extracted from Eucommia ulmoides leaves using a microwave-assisted hot water method, and subsequently, the polysaccharide-iron complex was synthesized through co-thermal synthesis with FeCl3. The physicochemical properties, structure, and thermal stability of the complex were analyzed using FE-SEM, SEC-MALLS, FT-IR, XRD, and DSC techniques. Furthermore, the antioxidant activity of the polysaccharide-iron complex was evaluated through an experiment in vitro. The results reve

Role of Helicobacter pylori virulence factors for iron acquisition from gastric epithelial cells of the host and impact on bacterial colonization.

EVALUATION OF: Tan S, Noto JM, Romero-Gallo J, Peek RM Jr, Amieva MR. Helicobacter pylori perturbs iron trafficking in the epithelium to grow on the cell surface. PLoS Pathog. 7(5), E1002050 (2011). The effects of Helicobacter pylori virulence factors on gastric epithelial cells are topics open to many studies. Major virulence factors, cytotoxin-associated gene A (CagA) and vacuolating cytotoxin (VacA), predict severe infection outcomes in many countries. H. pylori possesses various proteins for iron transport/storage, however, mechanisms of iron acquisition are not fully evaluated. The study

Postmenopausal Deep Infiltrating Endometriosis of the Colon: Rare Location and Novel Medical Therapy.

We report an uncommon case of deep infiltrating endometriosis of the colon presenting as iron deficiency anemia nine years after hysterectomy with bilateral salpingo-oophorectomy. The endometrial implant was found at the hepatic flexure, an exceedingly rare location for endometriosis invasion with no cases distinctly reported in the literature. Additionally, the presentation of gastrointestinal endometriosis as iron deficiency anemia is not well documented in the literature. Instead of surgery, we prescribed a novel medical therapeutic approach using conjugated estrogen-bazedoxifene to antagon

Trichotillomania in celiac disease patient refractory to iron replacement.

Trichotillomania is characterized by recurring hair pulling behavior and repeated attempts to decrease or stop the behavior. This behavior can cause a trichobezoar, which is a mass of hair found in the stomach. Patients with diagnosed celiac disease may have an urge to swallow their hair due to iron or folat deficiency, which is called pica. We report a case of an 11-year old girl with celiac disease who has had trichotillomania for a duration of 2 years, and required an operation due to residual gastric hair mass. In this case trichotillomania was more likely caused by behavioral disorders se
