The purpose of this study was to ablate atrioventricular (AV) conduction in dogs with an argon laser using a transvascular approach. Six dogs were anesthetized and underwent a right thoracotomy and atriotomy. A bipolar #7 French lumen catheter containing a 400 u silica fiber was used to map the region of the AV junction. We tested the feasibility of photocoagulating placental vascular communications with a fetoscopically delivered neodymium:yttrium-aluminum-garnet laser in 12 pregnant monkeys in the second trimester. The technique was successful in eight cases. One ended in spontaneous labor 2 weeks after occlusion and one stillbirth occurred at term. The neodymium-yttrium-aluminium-garnet laser has proved to be a useful therapeutic tool for the management of endobronchial lesions. Between October 1, 1986 and October 31, 1987, 31 patients received 41 laser treatments at the Peter MacCallum Cancer Institute mainly for bronchial obstruction, haemoptysis or persistent cough. Good symptomatic relief was obtained in the majority of patients with no operative mortality and a moderate morbidity. When the endobronchial lesion is malignant, concurrent radiotherapy is needed to prevent the early recurrence of the cancer.

We present the results obtained in the adrenal gland of white rats (average weight 220 g) following irradiation of the pineal gland with laser light. Using a hand-held optical fiber, a surgical neodymium:yttrium, aluminum, and garnet laser was used transsclerally on the left ciliary body of 25 dogs. Dogs were assigned to groups and were given low (100 J) and high (238 J) energy levels. In 12 dogs given 100-J energy, the intraocular pressure decreased a mean value of 6 mm of Hg below that in the untreated right eye. The decision to treat retinal breaks and degenerations prophylactically should be made with an awareness of the incidence of complications associated with argon laser photocoagulation (ALP). An unusual presentation of acquired immunodeficiency syndrome (AIDS)--a case of Kaposi's sarcoma involving the glans penis and fossa navicularis--was treated palliatively with neodymium:YAG (Nd:YAG) laser photoablation. A single outpatient treatment removed the two lesions and restored the patient's ability to void normally. The Nd:YAG laser proved extremely useful, providing effective outpatient therapy with minimal morbidity and excellent local results.

Although laser is effective in the treatment of genital condylomata, patients with extensive lesions frequently require more than one treatment. Argon Laser Trabeculoplasty (ALT) have been used in the treatment of open angle glaucoma since 1982. During a five-year period, 1982-1986, the annual rate of glaucoma filtering operations has decreased steadily from 65 to 8. The pressure reducing effect is good both in simple and capsular glaucoma. In secondary glaucoma ALT failed to control the intraocular pressure in about 50%. Simple and capsular glaucoma respond well to repeated ALT, but in secondary glaucoma repeated ALT seems to be useless.

The intra-ocular pressure in 76 patients, undergoing Neodymium: YAG laser surgery was measured just before treatment, and one and two hours afterwards. The fellow eye served as a control. Serious elevation of the intra-ocular pressure was demonstrated, especially in capsulotomy in the aphakic eye, to a lesser extent in pseudophakic eyes. Intra-ocular pressure elevation of more than 20 mm was seen in iridectomies. Timolol 1/2% eyedrops given before Neodymium: YAG laser treatment minimized the elevation of the intra-ocular pressure.
