Intrabiliary rupture of hydatid cyst of the liver.

Infestation with Echinococcus granulosus is common in Iraq, where a close relationship exists between dogs, the carnivorous definitive hosts, and sheep, the herbivorous hosts of the parasite. Plants contaminated by eggs of the tapeworm passed in the dog's faeces may be ingested by man, giving rise to hydatid disease. Of 136 cases of hydatid disease affecting various tissues and organs studied and treated during a 3-year period, the liver was involved in 94; intrabiliary rupture occurred in 15. Pain, hectic fever, and obstructive jaundice were invariable in these 15 cases but biliary obstructio…

Liver fluke infection in horses and ponies.

Thirty eight cases of Fasciola hepatica infection in horses with associated clinical signs are reported. A method of examining large amounts of faeces for fluke is described. A safe method of treatment for infected horses is given which involves oral medication with oxyclozanide at a dose rate of 15 ml/50 kg body weight.

Bronchobiliary fistula due to echinococcosis of the liver.

In Greece, which has the second highest incidence of echinococcosis in the world, there has been an overall decrease in the frequency of this disease and its complications during the past 15 years. Between 1920 and 1960, approximately 46 patients per year were operated on for echinococcosis at the Hellenic Red Cross Hospital, and 1.8% of 1037 hepatic hydatid cysts had ruptured into the bronchi. From 1960 to 1975, the number of patients requiring operation decreased to approximately 25 per year, and incidence of rupture into the bronchi declined to 1% of 288 liver cysts. The reduction has resul…

The familial liver donor in renal transplantation.

A survey is made of the 20-year experience with more than 300 living donors in renal transplantation at the Peter Bent Brigham Hospital. Details are given pertaining to the first transplantation of the kidney from one living person to another in December 1954. The surgical technique, the complications and the results of renal transplantation from a living donor are summarized. The philosophic and psychologic problems encountered are described. A comparison of results summarized by Murray of living, related donor transplants to cadaveric donor transplants is included. This paper constitutes a s

Aspects of paediatric liver disease in Karachi.

Seventy children with hepatomegaly, between the ages of 3 months and 13 years, were investigated including a liver biopsy in 60, to study the general pattern of liver disease in children. Thirty percent had acute viral hapatitis, 20 percent cirrhosis, 17.6 percent pulmonary tuberculosis, 18 percent hereditary diseases and 14 percent miscellaneous diseases involving the liver. None of the cases met the criteria for Indian childhood cirrhosis. It was concluded that in Karachi pulmonary tuberculosis was a common case of hepatosplenomegaly in children and that the aetiology of cirrhosis was probab…

Liver involvement in the syndrome of mixed cryoglobulinemia.

A study of liver abnormalities in 36 patients with mixed cryoglobulinemia in the absence of underlying infectious, connective tissue, or lymphoproliferative disorders revealed clinical or biochemical evidence of liver dysfunction in 84%. Hepatomegaly was detected in 77%, splenomegaly in 54%, and abnormalities in bilirubin, alkaline phosphatase, or serum glutamic oxalacetic transaminase in 77%. Only four of the patients had overt liver disease. Of 15 biopsies from 12 patients, there was normal tissue structure in two, minimal nonspecific changes in one, portal fibrosis in three, chronic persist…

Polycystic disease of the liver. Angiographic diagnosis.

Three cases of the syndrome of polycystic disease of the liver and kidneys are present. The definite diagnoses were established by means of angiography. Exploratory laparotomy was avoided. The incidence and symptoms of the syndrome and the diagnostic potentials of the methods of examination are reviewed. It is concluded that detection of either polycystic liver or kidney disease should initiate examination of the other component of the syndrome as well.

Ultrastructure of hepatocellular carcinoma in a cirrhotic liver.

Hepatoma cells, though sharing many similarities with non-neoplastic hepatocytes, showed conspicuous alterations--increased nuclear/cytoplasmic ratio, intranuclear invapinations of cytoplasm, lipoid material, high mitotic activity, and the presence of abundant small fat droplets and glycogen in the cytoplasm. "Capillarization" of sinusoids was a constant feature with the appearance of junctions between endothelial cells, deposition of fibrillary material in Disse's spaces, flattening of hepatocyte microvilli and deposition of basement-membrane-like material. Abundant microtubules and the prese

Abnormal liver function in chronic hypervitaminosis A.

A case of chronic hypervitaminosis A is reported in a 57-year-old woman who took vitamin preparations for alopecia. Liver biopsy of the patient showed both an increase in the number and size of fat-storing cells on light microscopy and rapidly fading green autofluorescence of vitamin A. Electron microscopy confirmed the presence of engorged fat-storing cells in the space of Disse and minor toxic changes in hepatocytes. This case illustrates the early hepatic changes of chronic hypervitaminosis A and indicates a need for restriction of sales of vitamin preparations.

[Liver abscess. Diagnosis by the method of opacification].

A 15 min infusion of a 75% iodized solution at 2 cc/k-w produces opacification of the more vascularized organs; hypovascular, necrotic or cystic regions are thus of a different radiological density. In order to show the usefulness of this method in diagnosing a hepatic abscess quickly and topographically, it was applied in 13 cases which had been diagnosed as invading amoebiasis. Hepatography proved to be the most valuable. Positive findings are observed 5 to 7 min after infusion has been completed. The greater density at the edge of the abscess enables its diameter to be measured and the liqu

Surgical treatment of hydatid disease of the liver.

Hydatid disease in southern Ontario is becoming more frequent because of infection in the large immigrant population. Review of 25 cases of hydatid disease of the liver has clarified the clinical features, diagnostic approach and methods of treatment. The commonest presenting symptoms were pain, mass or hepatomegaly, and jaundice. Diagnosis was confirmed, and cysts were localized, by Casoni skin testing, hepatic scanning and angiography. Surgical treatment consisted either of resection, excision or enucleation, and evacuation of the cyst. Two patients were treated by left lateral segmental res…
