Surgical treatment of the axilla in breast cancer patients is a controversial issue, as not only staging must be ensured, but also regional disease control. Twenty seven histologically verified axillary recurrences encountered over a period of 20 years were analysed according to primary tumours and their axillary metastases. The median age of the 27 investigated patients was 57 years. Axillary recurrences occurred in 7 node negative patients (26%), 10 (37%) and 6 (22%) patients with 1-3 and more than 3 positive nodes, respectively. The number of histologically assessed lymph nodes was less tha

INTRODUCTION: Retroperitoneal lymph node dissection (RPLND) is the most accurate method for evaluating the presence of retroperitoneal metastases from clinical Stage I nonseminomatous testicular tumors. We have introduced laparoscopy to reduce the morbidity of open RPLND, which is too high for a diagnostic procedure. As the next step, we replaced open surgery with the laparoscopic approach for Stage IIb tumors to remove residual tumor after chemotherapy.
TECHNICAL CONSIDERATIONS: One prerequisite for laparoscopic RPLND is free access to the retroperitoneum, which can be achieved by wide disse

Carcinoma of unknown primary (CUP-syndrome) is taken into consideration when the patient has histologically confirmed metastases but the primary focus remains unknown or it's diagnosis is delayed. Metastases of unknown origin constitute from 5% to 31% of all neck metastases. Squamous cell carcinoma and anaplastic carcinoma metastases in the neck lymphatics suggest that location of primary focus can be found in head and neck region. Diagnostic procedures applied in detection of primary foci have been described. The knowledge of lymphatic metastases spreading routes is helpful in the primary foc

PURPOSE: This is a report of 3 cases of extraneural metastasis of glioblastoma after interstitial radiation and assessment of pertinent literature addressing concern over an increased risk of these events with this therapy.
METHODS AND MATERIALS: In a series of 82 patients treated with (125)I brachytherapy for primary malignant brain tumors over a 7-year interval, 3 cases of extraneural glioblastoma were identified. The multicatheter technique for delivery of (125)I sources was utilized in all. Extraneural metastases were documented by imaging studies or biopsy. Over the same period, 310 pati

AIMS: To document the incidence of popliteal lymph node involvement by metastatic melanoma and to consider the implications of this information for clinical management.
METHODS: From the computerized database of the Sydney Melanoma Unit, all patients with primary melanomas located at or distal to the knee were identified and their records were examined. Experience with those patients who developed popliteal node metastases was then reviewed.
RESULTS: Thirteen of 4262 patients (0.31%) with primary melanomas of the distal lower limb developed popliteal node metastases. Six of the 13 patients h

Traditionally, cutaneous malignant melanoma is regarded as a radioresistant tumor. Recently, however, an increasing number of clinical studies have refuted this notion. The authors examined the role of radiation therapy in the palliative and/or adjuvant treatment of cutaneous malignant melanoma. The records of 69 patients with cutaneous malignant melanoma were reviewed. Twenty-five patients with extensive regional lymph node involvement received adjuvant radiation therapy after primary surgical treatment, and the remainder received palliative radiation therapy. The therapeutic significance of

Currently, it is routine practice to carry out axillary lymph node dissection at the time of surgical removal of a malignant primary breast tumour. As breast cancer is being diagnosed at an earlier stage in a growing percentage of cases, this procedure is proving unnecessary in a proportion of patients. Axillary lymph node dissection carries a risk of side effects and ideally we should identify the patients who actually require the procedure. The concept of the sentinel node is not new and was developed over twenty years ago. The technique of lymphoscintography was initially used to identify t

In a prospective study of 29 patients with clinically node-negative breast cancer, sampling of rib marrow was superior to sampling of iliac-crest marrow for detection of micrometastases. Combination of marrow micrometastatic analysis with sentinel lymph-node mapping may improve accuracy of staging in breast cancer.

In all patients with NSCLC, systematic nodal dissection was performed. Since 1981, 218 stage IIIA-N2 cases were resected. We registered 2 operative mortalities. Overall survival was 23% while, in completely resected cases, survival amounted to 30%. For long-term survival, favourable prognostic factors were cN2 T1-2 N2M0, single mediastinal node involvement and a tumor 20 mm or less in maximum size. The 5 yr survival rates of stage IIIA N2 patients was, respectively 48.1% with tumor diameter < 20 mm, 27.7% with diameter between 21-30 mm, 31.2% with diameter between 31-50 mm and, finally, 16.7%

PURPOSE: To evaluate the improvement of multislice-spiral CT in the assessment of head and neck tumors.
MATERIAL AND METHODS: 80 patients with suspected tumor in the head and neck region were examined with MSCT (Somatom Plus 4 VZ) after the administration of i.v. contrast material. Slice collimation was 4 x 1 mm with a pitch factor of 6. Additional multiplanar reformations were calculated in each case.
RESULTS: The specific anatomy and pathways of tumor spread is difficult to demonstrate in cross-sectional imaging. Tumor infiltration of the base of the skull or the palate could be depicted o

AIM: To assess the value of the intensive histological work up of axillary sentinel lymph nodes (SLN) to demonstrate regional metastatic disease.
METHODS: From a series of 58 successful lymphatic mapping procedures, 78 SLN were analysed by serial sections (mean of 49 levels/SLN) and by immunostaining to cytokeratin and epithelial membrane antigen, and the results compared with those obtained by assessing the central cross section.
RESULTS: The central cross section would have failed to detect metastases in eight of 26 lymph nodes (31%) in patients with breast cancer metastasising to the SLN

BACKGROUND: This study was undertaken to investigate the patterns of lymph node spread and the frequency of involvement of noncontiguous lymph node stations in patients with nonsmall cell lung carcinoma who had complete surgical resection.
METHODS: All patients who had surgical resection as their sole treatment for nonsmall cell lung carcinoma during the years 1987-1990 were reviewed. All patients were treated similarly. Generally, complete mediastinal lymph node dissection was performed after resection of the primary lesion and N1 lymph nodes. Patients were assessed for patterns of involveme

Thirty patients with oral mucosal cancer were studied in relation to oral mucosal damage and bacterial translocation to the regional lymph nodes in the neck. All 30 patients (gingiva 11, tongue 13, cheek mucosa four, oral floor two) underwent extensive, clean-contaminated, head-and-neck surgery (including neck dissection) with free flap reconstruction. A total of 153 lymph nodes was harvested for microbial and histological examination. Viable bacteria were isolated from 70 lymph nodes (45.8%) from 25 patients (83.3%). Bacterial cells in the nodes were detected by scanning electron microscopy.

In recent years, vulvar cancer management has been revolutionised with a more conservative approach being recommended for the primary lesion, and a more rational approach to the management of the lymph nodes. Treatment has become individualised with consideration being given independently to the optimal approach for the primary lesion and regional lymph nodes. The primary vulvar lesion can be effectively treated by radical local excision, thereby sparing the psychosexual consequences of radical vulvectomy in most patients. Local recurrence occurs in up to 10% cases whether or not radical vulve

Much has been written on micrometastases to the axilla in breast cancer but there is no consensus as to their size. In this study three levels of nodal tumour-load are defined following measurement of nodal tumour area on histology. The two cut-points described are both of clinical interest. The smallest deposits, up to 0.0001 cm2, include embolic growth on the afferent side of the node, which is, as reported previously, of poor prognosis. In such cases post-operative prognosis approaches that in the "node-positive", here defined as cases with an axillary tumour-load of 0.5 cm2 or more. Betwee

BACKGROUND: Axillary lymph node dissection is now no longer considered to be the standard treatment in all patients with invasive breast cancer. We have attempted to identify a sub-group of patients with invasive breast carcinoma who may not need to undergo axillary lymph node dissection.
METHODS: Patients (n = 823) with T1 N0M0 invasive breast cancer treated at our hospital between 1970 and 1994 were studied. We investigated the relationship between positive axillary lymph nodes and the following clinico-pathological factors: patient age, menopausal status, contralateral breast cancer (synch
