Evaluation of Cardiovascular and Hepatic Changes in Myocardial Infarction Patients Post-Covid-19 Vaccination.

INTRODUCTION: The global COVID-19 vaccination campaign has significantly reduced severe illness and mortality; however, emerging evidence raises concerns regarding its potential cardiovascular effects, particularly myocardial infarction (MI). METHODS: This study investigates the relationship between COVID-19 vaccination and MI incidence among first-time MI patients in Saudi Arabia. Post-COVID-19 vaccination within six months postvaccination accounted for potential confounding factors, such as pre-existing health conditions, age, and lifestyle. A total of 102 MI patients, with a male predomina…

Effects of moderate physical training program in post-myocardial infarction patients with arterial hypertension.

The clinical effectiveness of physical training in a Cardiac Rehabilitation Program (CRP) was assessed in hypertensive (Arterial Hypertension, AH), post-Myocardial Infarction (MI) patients. 206 patients were randomized into a physically trained group (PhTG, n=102) and an untrained, control group (CG, n=104). All patients received standard drug therapy. PhTG patients performed mild callisthenic exercises and moderately intensive bicycle exercise three times/week for one year. Compared to control patients, PhTG patients had significant changes in exercise capacity (duration +38%, p<0.001; total …

Left Ventricular Remodeling After Myocardial Infarction-Pathophysiology, Diagnostic Approach and Management During Cardiac Rehabilitation.

Despite the improvement in prognosis in patients with acute myocardial infarction (AMI), a significant proportion of survivors still experience heart failure (HF)-related adverse outcomes. Adverse left ventricular remodeling (LVR), which refers to a progressive dilation of left ventricular (LV) end-diastolic and end-systolic volumes, usually accompanied by a deterioration in LV systolic function, occurs frequently and underlies most cases of HF development after AMI. In this review, we discuss the current definitions of post-AMI LVR, the most appropriate imaging modalities for its detection, a…

Segmentation of the right ventricular myocardial infarction in multi-centre cardiac magnetic resonance images.

Right ventricular myocardial infarction (RVMI) is associated with higher in-hospital morbidity and mortality. Cardiac magnetic resonance (CMR) imaging provides crucial pathological information for diagnosis and/or treatment of RVMI. Segmentation of RVMI in CMR images is significant but challenging. This is because, to the best of our knowledge, there is no publicly available dataset in this field. Furthermore, the severe class imbalance problem caused by mostly less than 0.2 % proportion and the extreme intensity overlap between RVMI and the background bring challenges to the design of segment…

Plasma-based Raman spectroscopy for early detection of acute myocardial infarction in murine models.

Acute myocardial infarction (AMI) is a leading cause of cardiovascular mortality. Current diagnostic dilemma suffers from limited sensitivity, insufficiently timely and effective specificity. To address this dilemma, Raman spectroscopy, a rapid and non-invasive technique with significant potential for plasma metabolic profiling, although AMI involves rapid metabolic alterations, diagnostic approaches based on plasma metabolites remain underexplored. In murine AMI model induced by coronary artery ligation, we acquired Raman spectra from ultrafiltered plasma samples 8 h post-surgery. We identifi…

Predictors and Impact of Cardiogenic Shock in Oldest-Old ST-Elevation Myocardial Infarction Patients.

Background: Cardiogenic shock (CS) is the most frequent cause of in-hospital mortality after ST-elevation myocardial infarction (STEMI). Data about CS in very elderly (age ≥ 85 years) STEMI patients are scarce. We sought to assess the prognostic factors and the short- and mid-term impact of CS in this population. Methods: Consecutive very elderly STEMI patients undergoing invasive treatment were included in a retrospective multicenter registry. Results: Among 608 patients, 72 (11.8%) fulfilled experienced CS. Peripheral artery disease (PAD) (OR: 2.25, 95% CI: 1.29-3.92, p < 0.01) and cardiac a…

Ideal Timing of Coronary Artery Bypass Grafting After Non-ST-Segment Elevation Myocardial Infarction.

BACKGROUND: Patients with non-ST-segment elevation myocardial infarction (NSTEMI) benefit from revascularization, but guidelines are vague regarding optimal timing. This study aimed to identify the ideal timing of coronary artery bypass grafting (CABG) after NSTEMI. METHODS: Study investigators examined patients with NSTEMI who underwent isolated CABG within 30 days of cardiac catheterization between July 2011 and July 2023 in a multicenter regional collaborative. Patients were stratified into 3 groups: ≤2 days, 3 to 7 days, and 8 to 30 days. Multivariable logistic regression analysis was per…

Non-Invasive Diagnosis of Chronic Myocardial Infarction via Composite In-Silico-Human Data Learning.

Myocardial infarction (MI) continues to be a leading cause of death worldwide. The precise quantification of infarcted tissue is crucial to diagnosis, therapeutic management, and post-MI care. Late gadolinium enhancement-cardiac magnetic resonance (LGE-CMR) is regarded as the gold standard for precise infarct tissue localization in MI patients. A fundamental limitation of LGE-CMR is the invasive intravenous introduction of gadolinium-based contrast agents that present potential high-risk toxicity, particularly for individuals with underlying chronic kidney diseases. Herein, a completely non-in…

Renal Function and its Time-Sensitive Influence on Survival Rates of Acute Myocardial Infarction.

BACKGROUND: Primary percutaneous coronary intervention is the preferred treatment for acute myocardial infarction. However, in patients with chronic kidney disease (CKD), the use of contrast media can exacerbate renal dysfunction, often necessitating alternative strategies. The impact of CKD on acute myocardial infarction prognosis, particularly in the context of percutaneous coronary intervention, is not fully understood. METHODS AND RESULTS: This study utilized real-world registry data from the OACIS (Osaka Acute Coronary Insufficiency Study) to evaluate prognosis across different CKD grade…

Phosphorylation of CRYAB induces a condensatopathy to worsen post-myocardial infarction left ventricular remodeling.

Protein aggregates are emerging therapeutic targets in rare monogenic causes of cardiomyopathy and amyloid heart disease, but their role in more prevalent heart-failure syndromes remains mechanistically unexamined. We observed mislocalization of desmin and sarcomeric proteins to aggregates in human myocardium with ischemic cardiomyopathy and in mouse hearts with post-myocardial infarction ventricular remodeling, mimicking findings of autosomal-dominant cardiomyopathy induced by the R120G mutation in the cognate chaperone protein CRYAB. In both syndromes, we demonstrate increased partitioning o…

S100A12 triggers NETosis to aggravate myocardial infarction injury via the Annexin A5-calcium axis.

Neutrophil extracellular traps (NETs) play a critical role in acute myocardial infarction (AMI) and the externalization of S100 family members. Here, we show the effects of S100A12 on NETs formation and myocardial injury following AMI. S100A12 expression increases rapidly in neutrophils and peaks on day 1 after AMI, promoting NETs production and exacerbating myocardial injury. DNase I, an inhibitor of NETs, reduces apoptosis of cardiomyocytes induced by S100A12. Mechanistically, the interaction of S100A12 and Annexin A5 (ANXA5) enhances calcium influx and promotes NETs formation. Blockage of A…

Podoplanin-positive cell-derived small extracellular vesicles contribute to cardiac amyloidosis after myocardial infarction.

Cardiac amyloidosis is a secondary phenomenon of an already pre-existing chronic condition. Whether cardiac amyloidosis represents one of the complications post myocardial infarction (MI) has yet to be fully understood. Here, we show that amyloidosis occurs after MI and that amyloid fibers are composed of macrophage-derived serum amyloid A 3 (SAA3) monomers. SAA3 overproduction in macrophages is triggered by exosomal communication from cardiac stromal cells (CSCs), which, in response to MI, activate the expression of a platelet aggregation-inducing type I transmembrane glycoprotein, Podoplanin…

Bioactive Peptide-Based Composite Hydrogel for Myocardial Infarction Treatment: ROS Scavenging and Angiogenesis Regulation.

After myocardial infarction (MI), the affected area of the myocardium falls into a state of ischemia and hypoxia, and subsequently, cardiomyocytes undergo a series of pathological changes and eventually transform into scar tissue. Therefore, restoring blood perfusion and reducing reactive oxygen species (ROS) are essential to promote the repair process of damaged myocardium. Here, the MMP12 (YWDAW) peptide which has a good antioxidant effect in deep-sea fish muscle, and the KRX (MRPYDANKR) peptide which shows a pro-angiogenesis effect from mammalian endothelial genes, were utilized collaborati…
