Comparison of shoulder internal rotation test with the elbow flexion test in the diagnosis of cubital tunnel syndrome.

PURPOSE: To compare the shoulder internal rotation test-a new, provocative test-with the elbow flexion test in the diagnosis of cubital tunnel syndrome (CubTS). METHODS: Twenty-five patients with CubTS were examined before and after surgery with 10 seconds each of the elbow flexion and shoulder internal rotation tests. Fifty-four asymptomatic individuals and 14 neuropathy patients with a diagnosis other than CubTS were also examined as control cases. For the shoulder internal rotation test, the patient's upper extremity was kept at 90° abduction, maximum internal rotation, and 10° flexion at

Whipple's disease: staging and monitoring by cytology and polymerase chain reaction analysis of cerebrospinal fluid.

BACKGROUND & AIMS: Diagnostic procedures in Whipple's disease usually focus on the intestine, but symptomatic central nervous system involvement is a major threat for patients. The aim of this study was to determine the diagnostic value of cerebrospinal fluid (CSF) analysis. METHODS: A total of 39 CSF samples and 2 brain biopsy specimens that were obtained from 24 patients with Whipple's disease at various intervals after diagnosis were examined. Five patients presented with neurological symptoms, 3 of them as relapses after therapy. Thirty-two CSF samples were examined by polymerase chain re

Totally cystic schwannoma of the lumbar spine.

A schwannoma is a benign tumor arising from a schwann cell and occurs mainly in the nerve sheath in the intradural extramedullary region. Schwannomas have been well described as occurring in the lumbar spine, but total cystic degeneration of schwannomas is rarely reported. The authors describe the clinical and radiographic evaluations and treatment of a rare case of an intraextradural totally cystic schwannoma on the lumbar spine. Two patients reported a history of 6 to 12 months of pain accompanied by weakness in the lower extremities. On examination, 1 patient had bilateral lower-extremity mu

An Ocular Manifestation of a Systemic Disease With Encephalitozoon Pogonae in a Juvenile Central Bearded Dragon (Pogona vitticeps).

OBJECTIVE: Encephalitozoon pogonae, a recently identified microsporidian species, has been associated with systemic infections in Central bearded dragons (Pogona vitticeps) manifesting as granulomatous inflammation and vasculitis. Despite the species similarity to Encephalitozoon cuniculi, which causes ocular, neurologic, and renal pathology in rabbits, ocular manifestations of E. pogonae in bearded dragons are underreported. This case report aims to explore the ocular manifestations of E. pogonae in a clinical case and highlight the challenges in diagnosis and treatment of microsporidial infe

Mercury poisoning as a cause of intracranial hypertension.

Mercury poisoning is a rare but fatal toxicologic emergency. Neurologic manifestations involving the central nervous system are seen usually with chronic mercury intoxication. The most commonly seen complaints are headache, tremor, impaired cognitive skills, weakness, muscle atrophy, and paresthesia. Here, we present a male patient who was chronically exposed to elemental mercury and had papilledema and intracranial hypertension without parenchymal lesion in the central nervous system. A 12-year-old male patient was referred to our emergency room because of severe fatigue, generalized muscle p

[Severe cardiac failure in Kearns-Sayre syndrome].

Mitochondrial disorders are a group of diseases that can affect virtually all organ systems. A 19 year old man was seen in 1993 with neurologic abnormalities consisting of impaired function of muscles, diplopia, progressive loss of vision, impaired phonation and swallowing, during the last 10 years. Physical examination disclosed moderate wasting of the four limb muscles, mild motor weakness of neck muscles, symmetrical hyporeflexia, cerebellar dysfunction, severe external ophtalmoplegia and ptosis. Fundii oculi examination showed retinitis pigmentosa. The electromyogram demonstrated myopathic

Prevalence of neurocysticercosis and its characteristics among people with epileptic seizures and progressively worsening severe headaches in 60 villages in three provinces of Burkina Faso.

PURPOSE: Neurocysticercosis (NCC) is a common cause of epilepsy in low- and middle-income countries (LMICs). Few studies have described neuroimaging findings in individuals with headaches in addition to epilepsy. Our aim was therefore to describe the types of NCC lesions seen on cerebral computed tomography (cCT), among participants with progressively worsening severe headaches (PWSH) and epilepsy living in 60 villages in Burkina Faso, and to determine the prevalence of NCC in these groups,. METHODS: Data from a screening questionnaire for epilepsy and PWSH and subsequent neurological examina

Posterior reversible encephalopathy syndrome (PRES) in a patient with moyamoya disease: A case report.

INTRODUCTION: Moyamoya disease (MMD) and posterior reversible encephalopathy syndrome (PRES) share similar pathophysiological characteristics of endothelial dysfunction and impaired cerebral autoregulation. However, there have never been any published studies to demonstrate the relationship between these 2 rare diseases. PATIENT CONCERNS: A 26-year-old Asian man presented with a throbbing headache, blurred vision, and extremely high blood pressure. We initially suspected acute cerebral infarction based on the cerebral computed tomography, underlying MMD, and prior ischemic stroke. However, th

Tarsal Tunnel Syndrome.

Tarsal tunnel syndrome is paresthesia and pain in the foot and ankle caused by entrapment and compression of the tibial nerve within the fibro-osseous tarsal tunnel beneath the flexor retinaculum. The most helpful diagnostic criteria are a positive Tinel sign at the ankle and objective sensory loss along the distribution of the tibial nerve. Treatment is designed to reduce the compression of the nerve, and surgical nerve release is indicated with failure of conservative options. It is important to identify the causative factor of the nerve compression and eliminate it to obtain excellent resul

Maternal Milk and Relationships to Early Neurobehavioral Outcome in Preterm Infants.

The purpose of this study was to (1) define medical and sociodemographic factors related to maternal milk feedings and (2) explore relationships between maternal milk feeding and early neurobehavioral outcome. Ninety-two preterm infants born ≤ 32 weeks gestation had maternal milk feeding and breastfeeding tracked in this retrospective analysis. At 34 to 41 weeks postmenstrual age (PMA), neurobehavior was assessed with the NICU Network Neurobehavioral Scale. Maternal milk feeding was often delayed by the use of total parenteral nutrition, administered for a median of 11 (7-26) days, impacting t
