Superior mesenteric artery syndrome in the modern trauma patient.

In 1861, von Rokitansky described obstruction of the third part of the duodenum by external compression of the duodenum by the superior mese … early ambulation, patients are able to avoid prolonged bed rest. The use of parenteral and enteral nutritional support has limited the loss of weight associated with trauma and burn patients, making this syndrome uncommon in this patient population. Recent reports of SMA syndrome focus on the association with corrective surgical procedures for scoliosis and obesity.

Metabolic fuels, neuropeptide Y, and estrous behavior in Syrian hamsters.

Of the various environmental factors influencing reproduction, food availability plays a particularly significant role, and an insufficient supply of oxidizable metabolic fuels inhibits reproduction in female mammals. When ovariectomized, steroid-primed hamsters are food deprived for 48 h, estrous behavior is suppressed. However, the specific neuroendocrine alterations that mediate the suppression of estrous behavior are unknown. Several conditions that inhibit female sexual behavior are thought to be associated with altered neuropeptide Y (NPY) activity in the brain. Intracerebroventricular (

Acute hypermagnesemia: a rare complication of antacid administration after bone marrow transplantation.

INTRODUCTION: We reported a case of hypermagnesemia in whom hypotension, hypothermia, and coma developed after repetitive doses of a seemingly harmless antacid for epigastric pain following bone marrow transplantation. METHODS: For this case, serial electrolytes, blood urea nitrogen, creatinine, calcium, and magnesium were obtained. Issues addressed were the restoration of normal hydration by normal saline, together with forced diuresis to hasten the renal excretion of magnesium, and eventual changes in its levels. RESULTS: The highest measured magnesium concentration was 5.9 mmol/l. She rec

Infant botulism.

Although the worldwide incidence of infant botulism is rare, the majority of cases are diagnosed in the United States. An infant can acquire botulism by ingesting Clostridium botulinum spores, which are found in soil or honey products. The spores germinate into bacteria that colonize the bowel and synthesize toxin. As the toxin is absorbed, it irreversibly binds to acetylcholine receptors on motor nerve terminals at neuromuscular junctions. The infant with botulism becomes progressively weak, hypotonic and hyporeflexic, showing bulbar and spinal nerve abnormalities. Presenting symptoms include

Arterial thrombosis leading to intestinal infarction in a patient with Behçet's disease associated with protein C deficiency.

Behçet's disease may be a possible cause of both occlusive and aneurysmal arterial involvement as well as recurrent venous thrombosis. A case of Behçet's disease complicated with vascular involvement leading to intestinal infarction is presented. A 41-yr-old man suffering from Behçet's disease for 15 yr presented with a 2-day history of severe abdominal pain and bloody diarrhea. Intestinal infarction secondary to thrombosis of the superior mesenteric artery had been diagnosed during surgical exploration 3 yr previously. He was started on anticoagulation with nutritional support. The patient wa…

The use of modular nutrients in pediatrics.

BACKGROUND: Nutrient modules are commonly used by pediatricians and dietitians. There is no readily available current literature to which those using nutrient modules can refer. Therefore, we review nutrient module composition, module use in pediatrics, and complications associated with the use of nutrient modules. METHODS: Using an online database, Meduline, we searched the literature from 1996 through 1995. RESULTS: Nutrient modules are single or multiple nutrients that can be combined with a diet to add nutrients or to change the composition of the diet. These nutrients exist as a food or

[Use of growth hormone during catabolic state in a patient in postoperative intensive care].

Patients in the intensive care unit often enter a catabolic state with significant protein loss, which may increase postoperative complications. Nutritional support alone usually does not reverse a catabolic state and thereby induce a positive protein balance. Recent studies suggest that recombinant human growth hormone (rhGH) stimulates protein synthesis and improves protein balance in critically ill patients. An increase in the level of growth hormone often fails to occur in critical illness and injury. We describe the case of a man aged 45 years who underwent laparoscopic fundoplasty for hi…

Necrotizing fasciitis in childhood.

Necrotizing fasciitis is a rare entity in the pediatric population. Five cases of this soft tissue infection were treated at the authors' institution between January and December 1993. Three of the children were profoundly neutropenic secondary to chemotherapy. All five were treated with aggressive surgical debridement, frequent dressing changes, broad-spectrum antibiotics, and nutritional support. In addition, the patients with neutropenia received a combination of granulocyte-colony stimulating factor and granulocyte transfusions. One child died of overwhelming sepsis and bone marrow graft f…

The relationship between anthropometric measurements and measurements of iron status and susceptibility to malaria in Gambian children.

Anthropometric measurements were made and serum iron and ferritin levels determined in a group of Gambian children at the beginning of the rainy season and these findings were related to the malaria experience of the children during the following malaria transmission season. Susceptibility to malaria was not correlated with prior weight-for-age, height-for-age, weight-for-height or serum albumin, or with serum iron, serum iron binding capacity nor serum ferritin. Thus, our findings do not provide any support for the view that poor nutritional status, as assessed by anthropometric measurements,

Effects of exercise training on human copper and zinc nutriture.

The findings of our three studies indicate no significant decreases in blood biochemical indices of copper and zinc nutritional status among female and male swimmers during physical training when dietary intakes of these trace elements are adequate (greater than or equal to 67% recommended safe and adequate intake). At these intakes, erythrocyte superoxide dismutase activity increased after physical training indicating a biochemical adaptation of human copper metabolism associated with exercise training. It is noteworthy that this increase in superoxide dismutase activity occurred without an a

Stimulation of hematopoiesis as an alternative to transfusion.

Optimal parenteral nutritional support, provided concomitantly with extraordinarily large replacement doses of intravenous iron dextran can be safe, effective, and life-saving for severely anemic patients who cannot or will not accept erythrocyte transfusion. Five patients who had sustained massive acute blood loss and two who had severe chronic anemia received as much as 140 ml of iron dextran intravenously. The average initial hemoglobin value in the patients with acute blood loss was 4.7 gm/dl (range 2.6 to 8.4 gm/dl), increasing to an average of 9.8 gm/dl (range 7.5 to 12.8) in 23.4 days (…

Should supportive care be started before initiating cancer treatment?

PURPOSE: This comment proposes that the diagnostic waiting period (DWP), defined as the interval from the first specialist visit to anticancer treatment initiation, should be recognized as an opportunity for early supportive care in patients with advanced lung cancer. METHODS: We discuss the clinical rationale for initiating low-burden supportive care at the first specialist visit, drawing on prior observations of clinical deterioration before treatment initiation and feasibility data from a home-based exercise program during the DWP. RESULTS: The DWP is often necessary for imaging, tissue s

Adjournment in Community HIV Prevention: Exploring Transitions in Community-Academic Partnerships.

Barbershop-based interventions have been increasingly implemented as a means to support culturally relevant and community-accessible health promotion and disease prevention efforts. Specifically, in neighborhoods of Brooklyn, New York, with high HIV seroprevalence rates, barbers have volunteered to support an initiative to help reduce sexual risk behavior. After implementing the Barbershop Talk With Brothers program for 5 years, we explored how program participation has affected barbers' HIV prevention and counseling skills to promote their clients' health, and assessed their views of next sta

[Evidence-based supportive measures to secure pancreatic anastomoses].

BACKGROUND: Pancreatic anastomosis is the Achilles heel of pancreatic surgery. Despite substantial progress in surgical techniques the rate of postoperative pancreatic fistulas remains very high. For this reason various supportive measures to secure pancreatic anastomoses are of continuing interest. OBJECTIVE: This review presents the newest evidence-based data on supportive measures designed to secure a pancreatic anastomosis.
