AIM: Pterygium is the primitive degenerative and hyperplastic pathology of the conjunctiva. The origin and nature of the primary pterygium lesion is still not fully understood. Surgery is used as a treatment for primary and recurrent pterygium. The surgical techniques applied range from simple surgical removal to complex procedures comprising the treatment of the optical zone and the repair of the conjunctiva. Conjunctival reconstruction may be carried out with free graft of the amniotic membrane.

Electrophysiologic testing of the visual system and ancillary imaging investigations may be crucial aids for correctly diagnosing patients with unexplained visual loss or isolated disc pallor. These patients may harbor a retinal disease process and not an optic neuropathy. A diverse range of retinal conditions may present in this manner, and misdiagnosis may lead to unnecessary neurologic investigation and subsequent delay in therapeutic intervention.

We describe a new instrument, the Reiss punctal punch, which is specifically designed to performa "wedge" punctoplasty. The Reiss punctal punch was used alone to treat three eyelids of three patients, and in conjunction with horizontal tightening of the eyelid to treat 35 eyelids of 18 patients with symptomatic epiphora related to punctal ectropion with secondary punctal stenosis. After a mean follow up of 12 months, 36/38 (95%) of the eyelids demonstrated a patent punctum, and symptomatic relief was achieved in 35/38 (92%) of the eyes.

Diabetic retinopathy is a leading cause of visual impairment and blindness. The disease is more likely in patients with insulin-dependent diabetes mellitus, and the incidence is higher in diabetes of long duration. Gonioscopy and goniophotography were performed on 137 eyes with the Optiflex angle supported implant. At the time of gonioscopy the lenses had been in situ for a median of 20 months, with a range of 3-38 months. The dimensions and topographical anatomy of 26 eyes from 14 neonates and infants were measured and photographed with special attention to the insertional position of the extraocular muscles. Additional measurements were made in histologic preparations of 12 normal infant eyes. Although the diameters of the neonatal eyes were found to be about 70% of the adult eyes, the volumes of the globes were only about half, and the surface areas even less. Corneal sensitivity was measured before and after exposure to an electric welding arc. The ultraviolet radiation produced in the exposed eye a rapid and large decrease in corneal sensitivity with an equally rapid recovery, all occurring within 4 h.
