[Children orbital floor fracture: retrospective study, about 34 cases].

INTRODUCTION: Orbital fractures represent 30 % of children facial fractures. Nausea and vomiting are more predictive of entrapment than local trauma stigmatisms. Entrapment and diplopia are more frequent in adults. Delay for surgery is unclear in literature varying from 6 hours to days. The aim of this study is to summarise the aspects of orbital floor fractures in children with regard to clinical and radiological presentation, management, and outcomes. PATIENTS AND METHODS: We conducted a retrospective study including 34 children presenting isolated orbital floor fracture. Clinical, radiolog…

Evidence implicating Thamnostylum lucknowense as an etiological agent of rhino-orbital mucormycosis.

In this report, we present a case of rhino-orbital mucormycosis in a 57-year-old female with poorly controlled diabetes mellitus. The only mold cultured at 25°C, 37°C, and 40°C from a specimen of the nasal crust was identified phenotypically and independently using nuclear ribosomal DNA sequence data as Thamnostylum lucknowense. To our knowledge, this report presents the first data implicating this mucoraceous fungus as a mycotic agent of human infection.

Orbital metastasis secondary to pulmonary adenocarcinoma treated with gefitinib: a case report.

INTRODUCTION: Orbital metastases of lung cancer are rare. However, because the number of patients diagnosed with lung cancer is increasing, the probability that a physician will see a patient with an orbital metastasis is also increasing. Unfortunately, the clinical course and response of these patients to cytotoxic chemotherapy are generally poor and keeping a patient's quality of vision is difficult. In recent years, gefitinib, an epidermal growth factor receptor tyrosine kinase inhibitor, has brightened the outlook for patients with advanced non-small cell lung cancer, especially for those …

Percutaneous embolization of an orbital hemangiopericytoma with Onyx facilitates its surgical excision.

This is a case report on the surgical treatment of orbital hemangiopericytoma. The patient initially underwent an attempted excision of the tumor with significant hemorrhage elsewhere, which precluded its complete excision. Due to continued growth and pain, the patient underwent percutaneous embolization with Onyx-18 under fluoroscopic radiographic guidance. The patient continued to have pain, progressive enlargement of the tumor, and underwent surgical excision. The excision was noted to have minimal bleeding and was relatively easy to dissect and excise compared with the typical hemangioperi…

Optic nerve thickening and infarction as the first evidence of orbital involvement with mucormycosis.

We report a 53-year-old female with uncontrolled diabetes mellitus who presented with decreased vision in the right eye for a few hours duration. Orbital computed tomography and magnetic resonance imaging were performed at presentation and showed a thickening and infarction of the right optic nerve with no other orbital abnormalities. A few days later, the patient developed necrosis in the region of the right medial canthus and nasal mucosa. Tissue biopsy confirmed the diagnosis of mucormycosis.

Two cases of feline orbital aspergillosis due to Aspergillus udagawae and A. viridinutans.

Aspergillus udagawae and A. viridinutans are members of the section Fumigati; both cause invasive aspergillosis in humans. These two Aspergillus species are discriminated from A. fumigatus by molecular methods. Herein, we report two cases of feline orbital aspergillosis, one caused by A. udagawae and the other by A. viridinutans. To our knowledge, Case 1 represents the first reported case of treatment of A. udagawae with a high dosage of itraconazole, and Case 2 represents the first reported case of A. viridinutans infection associated with sarcoma. Identification of the etiologic agents of th…

Radiofrequency ablation of the supra-orbital nerve in the treatment algorithm of hemicrania continua.

Hemicrania continua (HC) is an uncommon primary headache disorder in which the diagnosis centers on unilaterality and its absolute responsiveness to indomethacin. We describe 3 patients with a long standing history of headache diagnosed as hemicrania continua. There was profound response to indomethacin which was limited by side effects. In one patient the therapy with indomethacin was limited secondary to co-morbidities. Initial diagnostic blockade provided significant relief of symptoms based on which radio-frequency ablation of the supraorbital nerve was performed with substantial improveme…

Orbital invasion by squamous cell carcinoma arising in multiple epidermoid cysts.

Epidermal or epidermoid cysts usually are benign, solitary-growing masses located in the mid- or lower dermis. They are believed to derive from pilosebaceous units and are lined with an epidermis-like epithelium including a granular cell layer. The occurrence of multiple epidermal cysts on the scalp of nonsyndromic patients is extremely rare. Although the presence of squamous cell carcinoma in the wall of an isolated epidermoid cysts is well documented in the dermatological literature,, the authors are not aware of any article in the English literature describing orbital invasion by a carcinom…

Orbital Doppler evaluation of blood flow velocities in patients with diabetic retinopathy.

BACKGROUND: There have been conflicting results in relation to impaired ocular hemodynamics in the orbital vessels of patients with diabetic retinopathy (DR). Clarification of the early signs of retinopathy in diabetic patients is urgently needed. AIMS: We aimed to evaluate orbital blood flow velocities using Doppler and gray-scale sonography in patients with DR, and to compare the results with those of their non-diabetic and diabetic peers without retinopathy. METHODS: Orbital Doppler and gray-scale sonography were performed in 123 patients aged 29-77 who had been divided into 3 groups: non…

Biomechanical mechanisms of orbital wall fractures - a transient finite element analysis.

As the biomechanical mechanisms of orbital wall fractures are still under research, three different fracture mechanisms were tested in a finite element based investigation. In contrast to earlier studies, a finer skeletal model and a transient dynamic simulation were used to test pure hydraulic, pure buckling and a mixed force transmission. Results showed that each set-up led to different orbital fracture patterns, which correlate well with clinical findings. Therefore the conclusion is that different mechanisms may act together explaining the variety of clinical fracture situations. Biomechan…

Computer-assisted navigational surgery improves outcomes in orbital reconstructive surgery.

Orbital reconstruction is a difficult procedure, success of which is dependent on the surgeon's experience. The lack of objective methods requires a fair amount of estimation is its execution. This study evaluates the efficacy of Kolibri (BrainLab, Munich, Germany), an intraoperative navigation device, in improving outcomes. From 2004 to 2009, 58 patients with orbital trauma who underwent surgery at the National University Hospital, Singapore, were included in this prospective matched control trial. Twenty-nine consecutive patients underwent surgery with Kolibri. The control group underwent su…

Simulated fronto-orbital advancement achieves reproducible results in metopic synostosis.

BACKGROUND: Metopic synostosis results in trigonocephaly with bitemporal narrowing. Treatment involves differential fronto-orbital advancement, widening of the bitemporal region, and rounding the forehead. The purpose of this article was to implement three-dimensional surgical planning to treat metopic synostosis, including use of intraoperative templates, and to test the accuracy of the results. METHODS: This is a prospectively analyzed series of retrospective patients with metopic synostosis. Consecutive cases treated by the authors involving a simulated computer-aided planning session were…

A case of orbital hypertelorism with recovery of binocular vision after surgical treatment.

Orbital hypertelorism is defined as an abnormally wide bony interorbital distance. The aims of surgery are both correction of ocular dystopia and cosmetic reconstruction of the nasal crest. Marked improvement of visual function, especially binocular vision, by surgery is not expected. Here we report that surgical treatment unexpectedly resulted in a significant visual improvement for a 13-year-old boy with orbital hypertelorism who also had bilateral cleft lip and palate.

Management of penetrating medial and retro-bulbar orbital shrapnel/bullet injuries.

The presence of shrapnel or a bullet lying in the medial orbital and retrobulbar area is rare. A review of the literature revealed only a few reports. Recently eye/orbital penetrating trauma has increased dramatically, mainly due to IEDs or conventional weaponry in civilian and military casualties. Our experience in the management of these types of injuries prompted this report. Practical techniques for correctly determining the location of retained projectiles in the orbit are described. Surgical techniques for treatment are presented, based on safe and successful practices used in the missil…

Orbital and lacrimal gland progressive transformation of germinal centres--an underdiagnosed entity?

AIMS: Progressive transformation of germinal centres (PTGC) is a reactive process of undetermined aetiology. Although usually seen as nodal disease, it can occur at extra-nodal sites, such as oral cavity, gastrointestinal tract and skin. To determine if PTGC occurs in the orbit, we reviewed cases of 'florid reactive lymphoid hyperplasia' affecting this region. METHODS: The cases were retrieved and were investigated using a panel of immunohistochemical stains and PCR clonality studies. Cases were jointly assessed by a specialist ophthalmic pathologist and a haematopathologist. RESULTS: Eight …

Smart phone as an adjunctive imaging tool to visualize scolex in orbital myocysticercosis.

Orbital myocysticercosis is a helminthic infestation with variable clinical presentations. Computed tomography is considered gold standard in orbital imaging. Evaluation of computed tomography films in out-patient departments (OPD) is important in the management of orbital diseases. In this case report, we highlight the use of a smart phone as an aid in OPD for studying computed tomography images in identifying a small cysticercus cyst of the superior rectus muscle, which was invisible on naked eye examination.
