Antenatal phenotype associated with PAK2 pathogenic variants: bilateral pleural effusion as a warning sign.

Fetal pleural effusions can arise in various contexts with different prognosis. They have been reported in fetuses presenting with hereditar … ms to also be associated with cardiac and respiratory damage in addition to the Knobloch syndrome 1 phenotype. PAK2 is a ubiquitous protein with a major implication in regulation and remodeling of the cytoskeleton and numerous other cellular pathways. Knobloch-associated variants seem to cause a loss of the kinase function of the protein. Even if the ophtha …

Adult-onset distal myopathy with predominant hand involvement as a rare phenotype of plectinopathy.

Classic phenotypes of plectinopathies include epidermolysis bullosa simplex and muscular dystrophy with proximal distribution, associated or not with skin blistering. However, in recent years, patients manifesting new muscular phenotypes including lower-limb distal weakness have been described. We aim to expand the phenotypic spectrum of plectinopathies by reporting a case presenting with a pure skeletal myopathy with predominant upper-limb distal weakness. We describe this distal myopathy phenotype providing comprehensive clinical, genetic, myopathological and radiological data. Genetic studi…

Gene-environment interactions modulate the phenotype severity in mouse models of congenital craniofacial syndromes.

Birth defects are the leading cause of infant mortality, and most inborn errors of development are multifactorial in origin resulting from complex gene-environment interactions. Definition of specific gene-environment interactions in the etiology and pathogenesis of congenital disorders is critically needed in the absence of genotype-phenotype correlation but is challenging. This is particularly true for congenital craniofacial anomalies, which account for approximately one-third of all birth defects, as they typically exhibit considerable inter- and intrafamilial variability. A classic exampl…

Serologic investigation and management of an antibody screen negative para-Bombay phenotype during pregnancy.

BACKGROUND: Individuals with Bombay and para-Bombay red blood cell (RBC) phenotypes produce anti-H antibodies associated with acute haemolytic transfusion reactions. The rarity of compatible donor units creates unique logistical challenges, especially during pregnancy as the onset of labour and fetal distress can occur without warning. CASE REPORT: A 24-year-old female at 38 weeks gestation presented with breech fetal position and reported anti-H antibodies. Critically, the in-house antibody screen was negative by automated solid-phase red cell adherence (SPRCA), while manual testing subseque…

Intrauterine phenotype, genetic analysis, and pregnancy follow-up of fetuses with the 16p12.2 microdeletion.

Reports on the intrauterine phenotype of the 16p12.2 microdeletion are few. A retrospective analysis of the clinical data, genetic testing results, and neonatal prognoses of fetuses with the 16p12.2 microdeletion was conducted to provide a basis for their clinical management. The research participants were pregnant women who underwent prenatal diagnoses between November 2016 and June 2024. Among them, 12,000 cases were selected for karyotype analyses and single-nucleotide polymorphism (SNP) array testing. In the SNP array, 13 out of 12,000 fetuses (0.1%) had the 16p12.2 microdeletion, which in…

CohortDiagnostics: Phenotype evaluation across a network of observational data sources using population-level characterization.

OBJECTIVE: This paper introduces a novel framework for evaluating phenotype algorithms (PAs) using the open-source tool, Cohort Diagnostics. MATERIALS AND METHODS: The method is based on several diagnostic criteria to evaluate a patient cohort returned by a PA. Diagnostics include estimates of incidence rate, index date entry code breakdown, and prevalence of all observed clinical events prior to, on, and after index date. We test our framework by evaluating one PA for systemic lupus erythematosus (SLE) and two PAs for Alzheimer's disease (AD) across 10 different observational data sources. …

Expanding the Genotype and Phenotype Diversity in a Chinese Cohort With TRPV4-Related Dysplasia.

Dominant mutations in the calcium permeable ion channel TRPV4 (transient receptor potential vanilloid 4) typically result in skeletal dysplasia or peripheral neuromuscular disease. However, the full spectrum of TRPV4-related phenotypes remains incompletely defined. This study systematically reviewed the clinical and genetic features of 10 Chinese patients harboring various TRPV4 variants. In the cohort, six patients were diagnosed with spondylometaphyseal dysplasia Kozlowski type (SMDK) and four patients with metatropic dysplasia (MD). The most common features involved spinal deformity (platys…

Comparative assessment of human chondroprogenitor viability and molecular phenotype in various parenteral transport media.

Chondroprogenitors derived from articular cartilage offer a promising approach for treating cartilage pathologies owing to their high chondrogenic and low hypertrophic potential. Optimizing holding conditions and parenteral solutions for transporting these cells from the processing to the transplantation site is crucial to enable their clinical application. This study assessed the viability, molecular phenotype maintenance, and differentiation potential of human fibronectin adhesion assay-derived chondroprogenitors (FAA-CPs) suspended in five parenteral solutions-(a) normal saline (NS), (b) pl…

IgA2 ACPA Drives a Hyper-Inflammatory Phenotype in Macrophages via ATP Synthase and COX2.

IgA can form immune complexes (ICs) and activate myeloid cells via Fc alpha receptor-mediated signalling to secrete pro-inflammatory cytokines. It was previously described that of the two IgA subclasses (IgA1 and IgA2), IgA2 is more inflammatory than IgA1. However, the mechanisms underlying this differential pro-inflammatory potential remain poorly defined. Using anti-citrullinated protein IgA1 and IgA2 antibodies (ACPA) that are commonly found in rheumatoid arthritis (RA) patients and linked to chronic inflammation, we show here that, in macrophages, IgA2-ICs boost TLR-induced TNF and IL6 sec

Bony alignment decisions affect patient-specific laxity phenotype patterns significantly, independent of the deformity.

PURPOSE: While bony alignment phenotype reconstruction became an important part of personalised knee arthroplasty, the knowledge on laxity phenotypes (LPs) is still limited. This study aimed to calculate individual LPs and assess their changes based on bony decisions from different alignment workflows. METHODS: Radiographs and computer-assisted surgery data of 86 knees were imported into a validated knee alignment simulator. Individual bony parameters (medial proximal tibial angle, lateral distal femoral angle and posterior condyle axis) were first introduced. By that, the patient-specific bo…

Radiographic phenotype-driven clustering in lumbar decompression: comparative study of outcome and reoperation risk.

BACKGROUND CONTEXT: Lumbar spinal canal stenosis (LSCS) presents with various radiographic findings, often including concurrent degenerative changes. Prior studies have investigated the effects of individual radiographic findings and parameters separately using conventional methods such as logistic regression. However, applying these independent effects to real-world patients remains challenging due to an unknown interaction effect among multiple degenerative radiographic findings. PURPOSE: To identify distinct patient phenotypes based on preoperative radiographic findings using unsupervised …

Clinicopathological and Endoscopic Features of Non-Ampullary Duodenal Epithelial Tumors with Gastrointestinal Mixed Phenotype.

UNLABELLED: <p>Introduction: Non-ampullary duodenal epithelial tumors with a gastrointestinal mixed phenotype (mixed-type NADETs) have not been thoroughly analyzed. We aimed to elucidate the clinicopathological and endoscopic characteristics of mixed-type NADETs. METHODS: A total of 229 NADETs from 218 patients collected from February 2010 to December 2023 were analyzed. Based on immunohistochemistry for MUC5AC, MUC6, MUC2, and CD10, the NADETs were classified into gastric phenotype (GP), gastric predominant mixed phenotype (GPP), intestinal predominant mixed phenotype (IPP), and intestinal p…

Transcatheter aortic valve implantation in elderly patients with severe aortic stenosis and rheumatic phenotype.

Significant fibrosis is characteristic of rheumatic aortic valve disease that affects anchoring and deployment of the trans-catheter heart valve. Our goal was to determine whether trans-catheter aortic valve implantation (TAVI) is an effective treatment option for severe aortic stenosis patients with rheumatic valvular disease. This study is a prospective cohort study that includes a total of 54 rheumatic severe aortic stenosis patients who were referred for TAVI. The age ranged from 65.00 to 83.00 years (mean age 72.75 ± 5.86 years). Notably, 63% of the patients had undergone preimplantation

Intra-putaminal muscarinic receptor agonist infusion induces a dystonic phenotype in non-human primates.

Dystonia is a debilitating motor disorder of unclear pathophysiology, for which no specific pharmacological treatment is available in most cases. So far, several genetic models of dystonia suggesting a potential dysregulation of the intrastriatal cholinergic system have been obtained in rodents. However, in most cases, these animals did not exhibit a clear phenotype of dystonia but less specific motor impairments. In this study, we engineered an original model of dystonia in non-human primates by increasing cholinergic tone in the sensorimotor striatum. Chronic infusion of oxotremorine, a non-…

Further Clinical and Biochemical Phenotype of GLA p.A143T: A Fabry Disease Newborn Screening Experience.

BACKGROUND: In 2015, Illinois added Fabry disease to the newborn screening (NBS) panel, and numerous individuals who have the controversial p.A143T GLA variant were identified. Ann & Robert H. Lurie Children's Hospital of Chicago identified 80 individuals with this variant. SUMMARY: Of the 80 individuals, 34/80 were identified by NBS, 2/80 were identified by gene panel testing, and 44/80 were identified by cascade testing. These individuals were from 36 families and ranged in age from 7 months to 71 years. Most individuals identified by NBS were male (90.9%) and 35.96% had the p.A143T variant

Low SVEP1 in intrahepatic cholangiocarcinoma mediates phenotype switching-driven metastasis by Jag2/Notch1/Hes5.

Intrahepatic cholangiocarcinoma (ICC) is a distinct and increasingly prevalent subtype of cholangiocarcinoma arising from the epithelial cel … n in vitro, as well as tumor growth and lung metastasis in vivo. These effects were mediated by EMT phenotype switching through the activation of the Jag2/Notch1/Hes5 pathway. Our findings enhance the understanding of the novel mechanisms driving ICC progression and metastasis, suggesting that SVEP1 is a potential biomarker for ICC diagnosis.

Identification of nonsense variants in the ATM gene mimicking SCID phenotype: a brief report.

Severe combined immunodeficiency (SCID) represents a life-threatening inborn error of immunity, necessitating rapid diagnosis and intervention to prevent fatal outcomes. While SCID is characterized by profound T-cell lymphopenia, it may overlap with other conditions like ataxia-telangiectasia (AT), which also presents with T-cell deficiencies. This study examines two cases of suspected SCID in infants, later identified as AT due to pathogenic variants in the ATM gene. Despite initial negative results from SCID-targeted gene panels, further genetic testing revealed nonsense mutations (p.Y2036X
