Incidence of TB in inmates with latent TB infection: 5-year follow-up.

BACKGROUND: Inmates are a high-risk population for tuberculosis (TB) control efforts, including treatment for latent tuberculosis infection (LTBI). Completion of therapy after release has been poor. The goal of this study was to evaluate therapy completion and active disease over 5 years in a cohort of inmates. METHODS: The sample was from a completed randomized trial in 1998-1999 of education or incentive versus usual care to improve therapy completion after release from the San Francisco County Jail. Records from the jail, the County Tuberculosis Clinic, and the California TB Registry were

A Rorschach investigation of defensiveness, self-perception, interpersonal relations, and affective states in incarcerated pedophiles.

Rorschach protocols of 60 incarcerated men who met the Diagnostic and Statistical Manual of Mental Disorders (4th ed. [DSM-IV]; American Psychiatric Association, 1994) criteria for Pedophilia were compared to those of 60 incarcerated men with no history of sex offenses (matched for age, education, and race). Comprehensive System Rorschach variables (Exner, 1991) were selected based on both psychodynamic and cognitive-behavioral models of pedophilia. Pedophiles' Rorschachs (a) contained significantly more responses and were more likely to reveal signs of (b) anxiety and helplessness, (c) painfu

Serostatus for vaccine-preventable diseases in residents at Melbourne Juvenile Justice Centre.

There are concerns in Australia about inadequate rates of childhood immunisation, an important preventive measure to reduce infectious diseases. The population passing through the Melbourne Juvenile Justice Centre (MJJC) comes from a background at high risk for inadequate immunisation and outbreaks can occur in residential institutions. MJJC residents were invited to participate in a study by completing a medical officer-administered risk behaviour questionnaire and/or giving a blood sample. Ninety residents completed the questionnaire; 83 gave blood samples. Sera were tested for measles, mump

Implications of directly observed therapy in tuberculosis control measures among IDUs.

Tuberculosis (TB) is a rapidly growing problem among injecting drug users (IDU), especially those infected with human immunodeficiency virus. The authors review IDUs' responses to current TB control strategies and discuss the implications of their findings for the proposed implementation of directly observed therapy (DOT), a method for ensuring that patients take prescribed medication. Field workers carried out 210 ethnographic interviews with 68 IDUs in a Brooklyn, NY, community during 1990-93. Case studies suggested that many IDUs are uninformed about TB and often misinformed about their per

An exploration of the violent victim experiences of female offenders.

Female offenders are often portrayed as victims of violence, yet few studies have systematically examined such victimization. Violent experiences may both contribute to, and result from, crime--early family abuse may help propel a young woman into crime, and a criminal lifestyle may increase her risk of assault by strangers and intimate partners. Using data from 83 inmates, this study examined violent childhood and adult experiences, and explored possible linkages between victimization and offending. Although many women reported violent experiences, most striking was the high rate of assault b

Negative correlation between level of self-reported antisocial personality disorder symptoms and neural responsiveness to looming threats.

OBJECTIVE: Individuals with antisocial personality disorder (ASPD) exhibit an elevated risk for both threat/frustration-based reactive aggression and goal-directed aggression. This heightened risk may be associated with atypical neural responses to environmental threats. The objective of this study was to investigate neural responsiveness to threatening facial stimuli among individuals with varying levels of self-reported ASPD. METHODS: Fifty-four male participants were recruited, comprising 27 violent offenders incarcerated in a judicial police hospital and 27 individuals from the community.

Evaluation of the sensitivity of the Health Screening of People in Police Custody (HELP-PC) tool in Northumbria Police, UK.

This paper presents an evaluation of a novel health screening tool implemented in 2016 by Northumbria Police, in the North East of England, United Kingdom. The aim of the study was to determine the sensitivity of the tool in detecting physical and mental health conditions. Secondary aims were to produce useful data on the prevalence of these conditions and onward referrals within custody. The tool was compared to a clinical interview carried out within the custody suite by research psychiatrists. Researchers spent a total of 64 days within the custody suite and interviewed 177 participants. Th

Changing patterns and predictors of opioid use disorder and treatment of adults released from state detention in Pennsylvania.

INTRODUCTION: There is increased effort among states to enhance access to pharmacological treatment, a best practice for those diagnosed with opioid use disorder (OUD), within carceral settings to lower risk of overdose fatalities after release. This study examines trends in OUD and medications for opioid use disorder (MOUD) received by individuals released from state correctional institutions (SCIs) in Pennsylvania. METHODS: Using prison data, this retrospective cohort study analyzes annual rates of substance use disorder by sub-type, focusing specifically on OUD and subsequent treatments, a

Structural Determinants of Health Literacy Among Formerly Incarcerated Individuals: Insights From the Survey of Racism and Public Health.

BACKGROUND: Formerly incarcerated individuals (FIIs) encounter difficulties with covering the cost of dental and medical care, adhering to medication regimens, and receiving fair treatment from health care providers. Yet, no published research has examined modifiable pathways to increase FIIs' health literacy (HL), which is essential for addressing the health needs of this vulnerable population. OBJECTIVE: The aim of this article is to examine neighborhood characteristics (neighborhood deprivation, racial and economic polarization, and residential segregation) and public assistance program en

The Long Shadow of Incarceration: The Association of Incarceration History With Self-Reported Health Among Older Adults.

BACKGROUND: The United States has one of the highest incarceration rates in the world. Prior incarceration is associated with adverse health effects. While the era of "mass incarceration" began in 1973, little work has focused on older adults, whose lives have spanned the five decades of mass incarceration. METHODS: We conducted a cross-sectional analysis using data on adults 50 or older from the nationally representative Family History of Incarceration Survey to test the independent association between prior incarceration and self-reported physical and mental health. In logistic regression m

Sustainability of Treatment Programs Utilizing Medications for Opioid Use Disorders in Incarcerated Young Adults.

The epidemic of opioid overdose brought a major health crisis to the front line of public health in the United States. Early efforts have focused on the prevention of production, distribution, and consumption of the drugs. However, there is little information about youth populations at risk for opioid overdose and their response to targeted treatment plans. The San Bernardino County Youth Opioid Response (SBCYOR) coalition in collaboration with the San Bernadino County (SBC) Probation Department organized a safety net system for at-risk youth by improving communication among county resources.

Family Medicine Resident Education About Health Disparities Associated With Incarceration: A CERA Research Study.

BACKGROUND AND OBJECTIVES: We submitted research questions to the Council of Academic Family Medicine Educational Research Alliance (CERA) to assess the format of family medicine resident education about health disparities associated with incarceration and the perceived efficacy of efforts to prepare graduates for competent care of formerly incarcerated patients in practice. We think this is a universal problem, and current efforts are insufficient. METHODS: We evaluated data as part of the fall 2022 CERA survey of program directors (PDs). We reviewed descriptive statistics, generated compara

Partner incarceration, maternal substance use, and the mediating role of social support: A longitudinal analysis using the future of families and child wellbeing study.

INTRODUCTION: The United States is responsible for the highest incarceration rate globally. This study aimed to explore the impact of partner incarceration on maternal substance use and whether social support mediates the relationship between partner incarceration and maternal substance use. METHODS: Using data from the Future of Families and Child Wellbeing Study, a longitudinal cohort following new parents and children, this analysis quantifies the relationship between paternal incarceration and maternal substance use (N = 2823). We analyzed maternal responses in years 3 (2001-2003), 5 (200

Paternal Incarceration and Adolescent Delinquency: Role of Father Engagement and Early Child Behavior Problems.

Paternal incarceration is an important predictor of teen delinquency, but the factors that may explain this relationship-such as early child problem behaviors and level of father engagement-have not been adequately explored. The current longitudinal study examined paternal history of incarceration as a predictor of teen self-reported delinquency over a 15-year gap, considering early child problem behaviors and father engagement as mediators. Sex differences in these relationships were also evaluated. This four-wave longitudinal study included an analytic sample of 4897 teens who participated i

Infectious Disease Education in a Carceral Setting: Insights from a Jail-Based Medical Student-Led Health Literacy Initiative.

Incarceration is associated with adverse health outcomes, including higher rates of infectious diseases. Poor health literacy in this population is a likely contributing factor, making health education a potential tool to help address these health disparities. This article aims to describe the implementation of a novel medical student-led educational initiative in the jail setting as a tool to promote health literacy and decrease the infectious disease burden in the correctional setting. A four-part infectious disease health education curriculum was developed and tailored to a carceral context

Mandible Fracture Outcome Disparities in the Incarcerated Patient Population.

INTRODUCTION: Inmates represent a vulnerable health care population within the United States, with inconsistent access to surgical specialty care. Facial fractures are common among the incarcerated population, comprising approximately 14% of all traumatic injuries. However, few studies have evaluated facial fracture outcomes in this population. The aim of this study is to identify disparities in mandible fracture outcomes in the incarcerated population. METHODS: A retrospective review of all isolated traumatic mandible fractures at a single academic medical center from 2018 to 2023 was conduc
