Real-world effectiveness and safety of elexacaftor/tezacaftor/ivacaftor in cystic fibrosis patients with Phe508del -gating and -residual function genotypes.

BACKGROUND: Cystic fibrosis (CF) is a genetic disease caused by CFTR protein dysfunction. Elexacaftor/tezacaftor/ivacaftor (ETI) shows promise in improving outcomes for people with CF (pwCF) with Phe508del combined with gating (F/G) or residual function (F/RF) mutations, though real-world evidence remains scarce. OBJECTIVES: This study evaluated the effectiveness and safety of ETI therapy in pwCF with F/G and F/RF genotypes in real-world settings. METHODS: A multicentre, retrospective study enrolled 78 pwCF (24 F/G and 54 F/RF) receiving ETI after prior CFTR modulator treatments (ivacaftor a

The treatment with Elexacaftor/Tezacaftor/Ivacaftor significantly increases serum bilirubin and decreases blood platelets in children and adolescents with cystic fibrosis homozygous or double heterozygous for the F508del CFTR variant.

The Elexacaftor/Tezacaftor/Ivacaftor (ETI) combination of cystic fibrosis transmembrane regulator modulators is safe and effective even in children with at least one F508del variant. However, cases of liver damage have been reported, and we observed an increase of serum bilirubin and alanine aminotransferase (ALT) in Cystic Fibrosis (CF) adults homozygous or compound heterozygous for the F508del after one year of ETI treatment. In the present study we followed 106 people with CF (pwCF) aged 8-15 years, who were homozygous or compound heterozygous for the F508del variant and treated with ETI, m

Effectiveness of nirmatrelvir/ritonavir and molnupiravir in reducing the risk of short-term and long-term cardiovascular complications of COVID-19: a target trial emulation study.

While treatment with nirmatrelvir/ritonavir or molnupiravir is effective in lowering the rate of severe COVID-19, the effectiveness of these antivirals in reducing the risk of cardiovascular outcomes, especially among the hospitalized population, remains largely unknown. In this study, we assessed the real-world effectiveness of nirmatrelvir/ritonavir and molnupiravir on short- and long-term cardiovascular complications of COVID-19 using a target trial emulation design. Two target trials of COVID-19 antivirals were emulated by using a territory-wide, population-based, retrospective cohort of h

Pioneering 1,3,5-triaryl pyrazolines as promising dual inhibitors of COX-2 and 15-LOX endowed with potent anticancer activity: design, synthesis, and in silico study.

Despite selective COX-2 inhibitors existing, they do not fully meet the demand for safer anti-inflammatory drugs. COX-2 and 15-LOX dual inhibition offers a promising avenue for developing safer NSAIDs and may play a crucial role in the fight against cancer. Thus, we designed and synthesized new 1,3,5-triaryl pyrazolines as dual COX-2/15-LOX inhibitors with potent anticancer activity. The synthesized compounds were evaluated in vitro as dual COXs/15-LOX inhibitors and for anticancer activity against human colorectal cancer cell lines HT-29 and HCT116. Peripheral Muscle Function and Body Composition in People With Cystic Fibrosis on Elexacaftor/Tezacaftor/Ivacaftor: A Cross-Sectional Single-Centre Study.

BACKGROUND: People with cystic fibrosis (pwCF) often have multifactorial peripheral muscle abnormalities attributed to, for example, malnutrition, steroid use, altered redox balance and, potentially, CF-specific intrinsic alterations. Malnutrition in CF now includes an increasing prevalence of overweight and obesity, particularly in those receiving CF transmembrane conductance regulator (CFTR) modulator therapy (CFTRm). We aimed to characterise peripheral muscle function and body composition in pwCF on Elexacaftor/Tezacaftor/Ivacaftor (ETI) CFTRm, compared to healthy controls. METHODS: Fiftee

Evaluation of gaps in direct oral anticoagulants (DOACs) management in an outpatient underserved clinic: a cross-sectional study.

DOACs are widely used for stroke prevention in non-valvular atrial fibrillation and treatment of thromboembolism, but gaps in their management can lead to adverse outcomes. In outpatient, underserved clinics, challenges such as inappropriate dosing, lack of monitoring, and improper drug combinations may be more pronounced, necessitating evaluation to optimize patient care and reduce risks. This study assessed the appropriateness of DOAC use and dosing per FDA guidelines, considering renal function, hepatic function, and drug-drug interactions. Secondary objectives focused on evaluating inappro

Loss-of-function mutations in PLD4 lead to systemic lupus erythematosus.

Monogenic lupus offers valuable insights into the underlying mechanisms and therapeutic approaches for systemic lupus erythematosus (SLE)1-3. Here we report on five patients with SLE carrying recessive mutations in phospholipase D family member 4 (PLD4). Deleterious variants in PLD4 resulted in impaired single-stranded nucleic acid exonuclease activity in in vitro and ex vivo assays. PLD4 loss-of-function mutations led to excessive activation of Toll-like receptor 7 (TLR7) and TLR9. Downstream inflammatory signalling pathways, especially type I interferon signalling, were hyperactivated in pat

Acquired resistance to vemurafenib restrains thyroid cancer stem cell self-renewal by suppressing STAT3 activation.

Vemurafenib (PLX4032) is a B-Raf kinase-specific inhibitor that has been approved for treating BRAF-mutated melanoma but is ineffective in treating thyroid cancer. Cancer stem cells (CSCs) play an important role in drug resistance (DR). Our present study aims to determine the status of CSCs in thyroid cancer cells that have acquired adaptive drug resistance to PLX4032. We first established a DR anaplastic thyroid cancer (ATC) cell line by culturing SW1736 cells in the media containing gradually increasing concentrations of PLX4032 (0.1-4 μM) for 6 months. We found that HER3, a member of the Er

An oral lichen planus-like mouse model driven by IFN-γ signaling and cytotoxic CD8+ T cells.

Oral lichen planus (OLP) is a recalcitrant inflammatory disease with potential for malignant transformation, involving a cytotoxic CD8+ T cell-mediated basal keratinocyte apoptosis. However, it lacks an appropriate mouse model for study. Here we developed an OLP-like mouse model using topical oxazolone to induce a delayed-type hypersensitivity-mediated oral lichenoid reaction. Histological and ultrastructural analysis confirmed hallmark pathological features of OLP, including band-like CD8+ T cell infiltration and basal cell damage as well as the presence of Civatte bodies.
