PTH Treatment Increases Cortical Bone Mass More in Response to Compression than Tension in Mice.

Parathyroid hormone (PTH) is an anabolic osteoporosis treatment that increases bone mass and reduces fracture risk. Clinically, the effects of PTH are site-specific, increasing bone mass more at the spine than the hip and not increasing bone mass at the radius. Differences in local loading environment between the spine, hip, and radius may help explain the variation in efficacy, as PTH and mechanical loading have been shown to synergistically increase bone mass. We hypothesized that differences in loading mode might further explain these variations. Owing to the curvature of the mouse tibia, c

Mechanical strength and gastric residence time of expandable fibrous dosage forms.

Expandable, gastroretentive dosage forms are promising for precise control of drug concentration in blood. So far, however, short gastric residence times and safety considerations have limited their use. To mitigate such limitations, in this work expandable fibrous dosage forms were investigated for mechanical strength and gastric residence time in dogs. The fiber formulation comprised ibuprofen drug; water-absorbing, high-molecular-weight hydroxypropyl methylcellulose (HPMC) excipient; fiber-strengthening, enteric methacrylic acid-ethyl acrylate excipient; and barium sulfate, a gastrointestin

Bone loss and reduced osteoblast function in primary biliary cirrhosis.

The association of bone loss with primary biliary cirrhosis is poorly understood. In 15 premenopausal female patients, only 2 of whom had fractures, mean bone mineral density was reduced at the lumbar spine but not at the midradius or distal radius. Bone loss was not statistically related to the duration or severity of liver disease. Urinary hydroxyproline excretion, an index for bone resorption, was not different from that of 15 age-matched normal women, but the serum concentration of bone Gla-protein (osteocalcin), a specific marker for bone turnover, was decreased (p less than 0.001). Bone

Impact of ulnar fracture level on radial head dislocation: A Monteggia fracture study.

BACKGROUND: Monteggia fracture is defined as a combination of a proximal ulnar fracture with radial head dislocation. Radial head dislocation may be misdiagnosed under an elbow radiograph, leading to further complications. This study aimed to compare the ulnar fracture pattern characteristics between Monteggia fracture and an isolated proximal ulnar fracture in terms of how close the fracture site is to the coronoid and what type of fracture pattern is more likely to result in radial head dislocation. METHODS: This single-center retrospective study, conducted from January 2014 to June 2022, i

Elbow Fracture-Dislocations: Determining Treatment Strategies.

Elbow dislocations represent common injuries. A quarter of these injuries involve at least 1 fracture. The sequel of elbow fracture-dislocations can be fraught with complications, including recurrent instability, posttraumatic arthritis, elbow contracture, and poor functional results. The 3 main patterns of injury are valgus posterolateral rotatory instability, varus posteromedial rotatory instability, and transolecranon fracture-dislocation. This article discusses each pattern individually, including the anatomy, the typical injury pattern, and treatment strategies. It also discusses common c

Is there a need for standardized postoperative radiographs after operative treatment of wrist or ankle fractures?

PURPOSE: The purpose of this study was to evaluate the frequency of changes in treatment plan due to standardized postoperative radiographs. A secondary aim was to compare our results with a national benchmark. METHODS: This is a single-center retrospective case series of 167 consecutive patients, operated with open reduction and internal fixation (ORIF) for distal radius or ankle fractures in 2014. Changes in the treatment protocol were defined as additional CT-imaging, reoperation or other changes as stated in the postoperative instructions. In addition, a national survey was conducted asse

Open radiocarpal fracture-dislocations.

Ten open radiocarpal fracture-dislocations in nine patients were treated by wound debridement, open reduction, and maintenance of reduction by casting supplemented with internal or external fixation. Eight patients had associated fractures or dislocations of the ipsilateral extremity: elbow fracture or dislocation (three patients), shoulder fracture (one patient), fractured humerus (two patients), and phalangeal fractures (three patients). In seven of ten cases the median and/or ulnar nerve was severely contused and required immediate decompression. Six patients were followed at least 15 month

Long-term patient-reported outcomes after nonoperative treatment of distal radial fractures: what CT-based gaps and step-offs can be accepted?

PURPOSE: Nonoperative treatment of distal radial fractures is recommended for intra-articular gaps or step-offs under 2 mm; a threshold based on 40-year-old radiographic data, which is still referred to despite the transition toward Computer Tomography (CT)-imaging for assessing displacement. Controversy about treatment exists when displacement slightly exceeds 2 mm. We assessed the association between CT-based gaps and step-offs and patient-reported outcomes after nonoperative treatment, and established CT-based thresholds for fracture displacement. METHODS: A multicenter retrospective cohort

Combining radiomics of X-rays with patient functional rating scales for predicting satisfaction after radial fracture fixation: a multimodal machine learning predictive model.

BACKGROUND: Patient satisfaction after one year of distal radius fracture fixation is influenced by various aspects such as the surgical approach, the patient's physical functioning, and psychological factors. Hence, a multimodal machine learning prediction model combining traditional rating scales and postoperative X-ray images of patients was developed to predict patient satisfaction one year after surgery for personalized clinical treatment. METHODS: In this study, we reviewed 385 patients who underwent internal fixation with a palmar plate or external fixation bracket fixation in 2018-202

A retrospective review of 101 operatively treated radial neck fractures in children and analysis of risk factors for functional outcomes.

INTRODUCTION: Radial neck fracture is a less common injury in childhood and many factors are associated with the final functional outcomes. This study aimed to describe the clinical and radiological results of the patients with radial neck fractures treated surgically and assess the potential risk factors for poor functional outcomes. PATIENTS AND METHODS: This study enrolled 101 children with radial neck fracture and treated surgically at our department. Patients were followed up at least three years and assessed clinically and radiographically. The primary outcome of this study was the func

Procedural Sedation With Ketamine Versus Propofol for Closed Reduction of Pediatric Both Bone Forearm Fractures.

Effective treatment of pediatric both bone forearm fractures consists of timely restoration of anatomic alignment with manipulation and immobilization, often accomplished with the aid of procedural sedation in the emergency department setting. The current lack of consensus regarding a safe and optimal regimen may result in inadequate sedation, compromised quality of reduction, or patient harm. The current study was conducted to answer the following questions for pediatric both bone forearm fractures treated with closed reduction with either ketamine or propofol procedural sedation: (1) Is ther

Three-screw versus two-screw fixation of distal fragment in fifth metacarpal neck fractures stabilized with locking plate.

Fifth metacarpal neck fracture commonly requires open reduction and internal fixation. Locking plate was widely adopted in the treatment of fifth metacarpal neck fracture as first-line choice for fixation. Patients with fifth metacarpal neck fracture receiving locking plate fixation were included for analysis. Features of internal fixation including number of distal and proximal locking screws, diameter of the screws and usage of lag screws were recorded. Clinical and radiographic outcomes included final volar angulation, grip strength, Michigan Hand Outcomes Questionnaire (MHQ) and range of m

The yield of subsequent radiographs during nonoperative treatment of radial head and neck fractures.

OBJECTIVES: After diagnosis of an isolated radial head or neck fracture and selection of nonoperative treatment, the value of subsequent radiographs is uncertain. This study tested the null hypothesis that there are no patient, surgeon, or injury factors associated with alteration in patient management based on subsequent radiographs. Secondarily, we tested the null hypothesis that the use of subsequent radiographs is not associated with patient, surgeon, and fracture characteristics. METHODS: We identified 415 adult patients with nonoperative treatment for isolated Broberg and Morrey modifie

Functional outcomes after fixation of "terrible triad" elbow fracture dislocations.

Historically, the published literature on "terrible triad" injuries has shown a high rate of unacceptable results. The use of systematic treatment protocols may improve functional outcome. The authors performed a retrospective study of all patients aged 18 years or older who underwent surgical treatment for "terrible triad" elbow fracture dislocation at their institution over a period 7 years. Surgical treatment involved fixation or replacement of the radial head, repair of the anterior capsule or coronoid fracture in most cases, and repair of the lateral collateral ligament. Outcomes included

Newly defined fracture pattern specific to Mason III radial head fractures: fracture description, management and outcomes using screw fixation.

BACKGROUND: Radial head fractures are the most common type of elbow fracture and are universally classified under the Mason classification system. Mason type III fractures are comminuted and are the most difficult to treat, generally requiring plating if possible, or more commonly arthroplasty or excision, which gives a variable outcome. We hypothesized that a new and specific fracture pattern of the radial head (Mason III) can be treated successfully with screw fixation. METHODS: Six patients presented to the senior surgeon's clinic with this unusual Mason III fracture pattern. In these pati
