Nonarthroplasty treatment of glenohumeral cartilage lesions.

Treatment of young, active persons with symptomatic cartilage lesions of the glenohumeral joint represents a significant challenge. Diagnosis of glenohumeral chondral defects is not always straightforward and effective treatment requires familiarity with a number of techniques. Low-demand individuals may accept palliative therapy in the form of arthroscopic debridement as a temporizing solution. However, younger, high-demand individuals require a careful, stepwise approach that includes reparative, restorative, and reconstructive strategies. Reparative strategies use marrow-stimulation techniq

Successful treatment of pustulotic arthro-osteitis with bimekizumab: a case report.

Pustulotic arthro-osteitis (PAO) is an inflammatory osteoarticular disorder associated with palmoplantar pustulosis (PPP). Whilst anti-TNF-α agents and IL-23 inhibitors have shown efficacy in PAO, the therapeutic potential of bimekizumab (BKZ), a dual IL-17A and IL-17F inhibitor, remains unestablished. A 53-year-old Japanese man with a history of PPP developed fever and polyarthritis involving the distal interphalangeal joints, shoulders, and knees. Laboratory investigations revealed elevated inflammatory markers, including C-reactive protein and matrix metalloproteinase-3. Imaging studies sho

Further delineation of facioaudiosymphalangism syndrome: Description of a family with a novel NOG mutation and without hearing loss.

Mutations in the NOG gene give rise to a wide range of clinical phenotypes. Noggin, the protein encoded by this gene is a secreted modulator of multiple pathways involved in both bone and joint development. Proximal symphalangism is commonly observed in patients bearing mutations in this gene, however secondary symptomes are often found including typical facies with hemicylindrical nose with bulbous tip, hyperopia, reduced mobility of multiple joints, hearing loss due to stapes fixation, and recurrent pain from affected joints. With large variation of the phenotype both within and between affe

Comparison of internal fixations for distal clavicular fractures based on loading tests and finite element analyses.

It is difficult to apply strong and stable internal fixation to a fracture of the distal end of the clavicle because it is unstable, the dis … because the arm plate did not interfere with the acromioclavicular joint, suggesting that favorable shoulder joint function can be achieved. The stability of fixation with the LCP clavicle hook plate and the scorpion was similar, and plate fixations were stronger than fixation by tension band wiring.

Biomechanical energetic analysis of technique during learning the longswing on the high bar.

Biomechanical energetic analysis of technique can be performed to identify limits or constraints to performance outcome at the level of joint work, and to assess the mechanical efficiency of techniques. The aim of this study was to investigate the biomechanical energetic processes during learning the longswing on the high bar. Twelve male, novice participants took part in a training study. Kinematic and kinetics data were collected during swing attempts in eight weekly testing sessions. Inverse dynamics analysis was performed from known zero forces at the toes. Joint work, total energy, and ba

[Hip arthroscopy--indications, surgical technique and implementation].

Hip arthroscopy was first described by Burman in 1931. During the 1980's this procedure was performed by a limited group of orthopedic surgeons. Implementation and development of the surgical procedure received most attention during those years, together with the formulation of specific indications to perform hip arthroscopy. In contrast to other large joints (knee, shoulder), where accurate diagnosis is feasible using standard imaging procedures such as plain radiographs, computed tomography and magnetic resonance imaging, the hip joint often presents as a diagnostic anomaly. In specific case

Elbow and wrist injuries in sports.

Any disabling injury of the elbow or wrist should be studied roentgenographically for evidence of fracture which may not be otherwise evident but which may cause permanent disability unless the joint is immobilized for healing."Tennis elbow" may be treated with physical therapy and analgesic injection but may require splinting or tendon stripping. Elbow sprain can occur in the growing epiphysis but is rare in adults. A jarring fall on the hand may cause fracture or dislocation at the elbow. Full extension of the joint should be restored gradually by active exercise rather than passive or forci

Long-term outcomes of arthroscopic Bankart repair: risk of recurrence and osteoarthritis at 11-20 years of follow-up.

BACKGROUND: Arthroscopic Bankart repair (ABR) is a commonly performed treatment for anterior shoulder instability. Risk factors for recurrence of dislocation after this procedure have been investigated previously at short- and mid-term follow-up. Recurrence of dislocation in the long term, as well as the incidence and risk factors for the development of osteoarthritis (OA) after this procedure require further investigation. METHODS: In this single-institution retrospective study, patients who had received ABR for structural anterior instability of the shoulder with a history of at least 1 doc

Arthroscopic knotless anterior labral stabilization using labral tape and wide awake anaesthesia-short term results.

BACKGROUND: The shoulder is the least constrained of all joints of the body and is more susceptible to injury including dislocation. The rate of recurrent instability following primary stabilization procedure at 10 years of follow-up ranged from 3.4 to 20%. There is a lack of evidence in the literature regarding use of labral tape and anchors for anterior stabilization despite the growing market for this product. We describe the outcomes of 67 patients who underwent knotless arthroscopic anterior stabilisation under awake anaesthesia using 1.5 mm LabralTape with 2.9 mm Pushlock anchors for pri
