HSP47 inhibitor Col003 inhibits glial scar formation and improves neurological function in ischemic stroke in rats.

BACKGROUND: Glial scarring is a major obstacle for axonal regeneration and neural repair in the late stage of ischemic stroke. Reactive astrocytes are the main component of the glial scar. Heat shock protein 47 (HSP47) is significantly expressed in reactive astrocytes and remains present in the glial scar. However, the role of HSP47 in glial scar formation in ischemic stroke remains unclear. METHODS: HSP47 was identified in the peri-infarct region of rats at 1, 7, and 14 days post middle cerebral artery occlusion (MCAO). The rats received daily tail vein injections of the HSP47 inhibitor Col0

Direct evidence implicating the asymmetrical dimethylarginine (ADMA)-dimethylarginine dimethylaminohydrolase 1 (DDAH1) pathway in cerebrovascular dysfunction and stroke pathogenesis in mice.

BACKGROUND AND PURPOSE: Asymmetrical dimethylarginine (ADMA) is an endogenous inhibitor of nitric oxide synthase (NOS), whereas dimethylaminohydrolase 1 (DDAH1) is primarily responsible for its metabolism. However, the roles of DDAH1 in cerebrovascular regulation are poorly understood. Furthermore, despite evidence that elevated ADMA levels are associated with worse ischaemic stroke outcomes, there is a paucity of data showing causality. Therefore, this study aimed to address these knowledge gaps. EXPERIMENTAL APPROACH: We examined the effect of an inhibitor of DDAH1 (L-257) on endothelial-ni

Alterations of cerebrospinal fluid dynamics in post-stroke depression: A longitudinal multimodal imaging study.

BACKGROUND: Altered cerebrospinal fluid (CSF) dynamics is probably related to post-stroke depression (PSD) through different mechanism. However, temporal changes in perivascular CSF dynamics after stroke remain poor understood. This study investigated the temporal relationship between CSF dynamics and PSD recovery using multimodal neuroimaging strategy. METHOD: We enrolled 308 stroke patients underwent baseline neuropsychological assessment and magnetic resonance imaging scan, and 110 completed 6-month follow-up evaluations. CSF dynamics were assessed through three imaging indicators, includi

NCF1 aggravates cerebral ischemia-reperfusion injury by amplifying NOX2-Dependent oxidative stress and inflammatory responses.

Cerebral ischemia-reperfusion injury (CIRI) is a major contributor to poor outcomes after ischemic stroke, yet the mechanisms linking oxidative stress and inflammation remain incompletely understood. Neutrophil cytosolic factor 1 (NCF1), a key organizer subunit of the NOX2 NADPH oxidase complex, is involved in reactive oxygen species (ROS) generation and immune activation. Here, we investigated the expression pattern, pathological role, and mechanistic significance of NCF1 in CIRI. A public spatial transcriptomic dataset of ischemic mouse brain tissue was reanalyzed to assess the spatial distr

Bridging Evidence Gaps in Severe Carotid Stenosis: A Systematic Review of Pre-Revascularization Risk Stratification, Clinical Predictors, and High-Risk Subgroups.

OBJECTIVE: Severe carotid artery stenosis (CAS) impairs cerebral blood flow and cognitive function. Revascularization may improve outcomes, but patient selection and timing remain challenging. This review synthesizes evidence on pre-revascularization risk factors. This systematic review synthesizes pre-revascularization evidence on CAS-related cognitive decline and evaluates timing, restenosis, high-risk subgroups (dialysis, radiation-induced, and sex), bilateral stenosis, hyperperfusion, and risk for stroke/death/MI outcomes. METHOD: A systematic search of PubMed, Scopus, EBSCO, Web of Scien

Identification of structural disconnection subnetworks in acute and subacute post-stroke depression.

BACKGROUND: Post-stroke depression (PSD) is common after acute ischemic stroke, yet its neural substrates remain unclear. This study identified structural disconnection subnetworks associated with PSD and evaluated their association with depression severity and topology. METHODS: We retrospectively analyzed 615 first-ever acute ischemic stroke patients. Depressive symptoms were assessed with HAMD-17 at baseline and 3 months. Individual 135 × 135 structural disconnection matrices were constructed from acute DWI lesions (MRI median 3 days post-stroke [1-4]) and HCP-842 normative connectome. Net

Exercise-induced Sesn2 mediates autophagic flux to alleviate neural damage after ischemic stroke in mice.

BACKGROUND: We previously demonstrated that exercise pretreatment can suppress oxidative stress and neuroinflammation following ischemic stroke. However, the specific mechanisms underlying these effects are uncertain. Sestrin2 (Sesn2), a stress-responsive protein, has been reported to reduce neuroinflammation and protect against ischemic cerebral injury. Hence, this study aimed to verify whether Sesn2 can mediate the antineuroinflammatory and antioxidative effects of exercise pretreatment and explore the potential downstream mechanisms involved. METHODS: To assess infarction volume and neuron

Inhibition of class IIa HDACs reduces neuroinflammation via NEU1-LAMP1 regulation and promotes M2 macrophage polarization in ischemic stroke.

Neuroinflammation plays a crucial role in ischemic injury. Macrophages contribute to post-stroke inflammation through activation and polarization into pro-inflammatory (M1) or anti-inflammatory (M2) phenotypes. Class IIa histone deacetylases (HDACs) have been implicated in inflammatory regulation. Neuraminidase 1 (NEU1) serves as a negative regulator of lysosomal-associated membrane protein 1 (LAMP1). This study investigates whether class IIa HDACs inhibition mitigates neuroinflammation by modulating macrophage polarization and lysosomal exocytosis via the NEU1-LAMP1. A total of 74 adult male

Oxytocin may promote hippocampal neurogenesis in ischemic stroke rats via a pathway related to DNMT1-mediated Wnt3a/β-catenin.

DNA methylation plays a pivotal role in neuroprotection after ischemic stroke. Oxytocin, a neuropeptide renowned for its involvement anti-inflammatory and antioxidant activities, emerges as a promising candidate for the treatment of stroke. Here, we employed a transient middle cerebral artery occlusion (tMCAO) rat model to explore the underlying mechanisms of oxytocin's therapeutic potential. We utilized the Morris Water Maze, novel object recognition, and intruder tests to assess spatial memory, cognitive memory, and social memory abilities in rats, respectively. RNA sequencing and Western bl

Immune modulatory and vascular protective effects of Ibudilast in post-stroke inflammation.

Ischemic stroke induces sustained microvascular dysfunction and inflammation, contributing to futile recanalization and poor tissue recovery despite successful large-vessel reopening. Neutrophil adhesion and capillary stalling are key drivers of this microvascular failure. While Ibudilast, a small-molecule phosphodiesterase (PDE3/4) inhibitor with vasodilatory and anti-inflammatory properties, has shown promise in modulating neuroinflammation, its impact on post-stroke microvascular flow and neutrophil-mediated obstruction remains poorly understood. In this study, we investigated the effects o

Generalizable self-supervised learning for brain CTA in acute stroke.

Acute stroke management involves rapid and accurate interpretation of CTA imaging data. However, generalizable models for multiple acute stroke tasks able to learn from unlabeled data do not exist. We propose a linear probed self-supervised contrastive learning utilizing 3D CTA images and the findings section of radiologists' reports for pretraining. Subsequently, the pretrained model was applied to four disparate tasks: large vessel occlusion (LVO) detection, acute ischemic stroke detection, acute ischemic stroke, intracerebral hemorrhage classification, and ischemic core volume prediction. Predictive value of left atrial function for latent paroxysmal atrial fibrillation as the cause of embolic stroke of undetermined source.

BACKGROUND: In patients with embolic stroke of undetermined source (ESUS), paroxysmal atrial fibrillation (AF) is often diagnosed, however, the risk of paroxysmal AF in ESUS has not been well described. Several studies have suggested a linkage between left atrial (LA) functional parameters and risk of AF in stroke patients. The aim of this study was to assess the role of LA functional parameters as predictors of latent paroxysmal AF in ESUS on admission. METHODS: Between January 2015 and December 2019, consecutive stroke patients with suspected ESUS at admission were prospectively included in

Mendelian Randomization Study of Obesity and Cerebrovascular Disease.

OBJECTIVE: To systematically investigate causal relationships between obesity and cerebrovascular disease and the extent to which hypertension and hyperglycemia mediate the effect of obesity on cerebrovascular disease. METHODS: We used summary statistics from genome-wide association studies for body mass index (BMI), waist-to-hip ratio (WHR), and multiple cerebrovascular disease phenotypes. We explored causal associations with 2-sample Mendelian randomization (MR) accounting for genetic covariation between BMI and WHR, and we assessed what proportion of the association between obesity and cer

Neuroprotective effect of secreted factors from human adipose stem cells in a rat stroke model.

Objectives Recent evidence shows that stem cells exert neuroprotective effect through the secretion of immune modulatory, neurotrophic factors. We aimed to assess the neuroprotective effect of selected recombinant factors (RFs) detected in human adipose stem cell (hASC)-conditioned medium (CM), in a rat ischemic stroke model. Methods Ischemic stroke was induced in Sprague-Dawley rats using 2 h transient middle cerebral artery occlusion (MCAO). One hour after reperfusion, the vehicle (Dulbecco's modified Eagle medium; DMEM), concentrated CM, and selected RFs mixed with DMEM were administered in

Metallothionein I as a direct link between therapeutic hematopoietic stem/progenitor cells and cerebral protection in stroke.

Stroke continues to be a leading cause of death and disability worldwide, yet effective treatments are lacking. Previous studies have indicated that stem-cell transplantation could be an effective treatment. However, little is known about the direct impact of transplanted cells on injured brain tissue. We wanted to help fill this knowledge gap and investigated effects of hematopoietic stem/progenitor cells (HSPCs) on the cerebral microcirculation after ischemia-reperfusion injury (I/RI). Treatment of HSPCs in I/RI for up to 2 wk after cerebral I/RI led to decreased mortality rate, decreased in

Functional improvement of focal cerebral ischemia injury by subdural transplantation of induced pluripotent stem cells with fibrin glue.

Ischemic stroke is the leading cause of disability in the world. Cell transplantation has emerged in various neurological diseases as a potential therapeutic approach in the postacute stroke phase. Recently, inducible pluripotent stem (iPS) cells showed potential for multilineage differentiation and provide a resource for stem cell-based therapies. However, whether iPS transplantation could improve the function of stroke-like model is still an open question. The aim of this study is to investigate the therapeutic effects of subdural transplantation of iPS mixed with fibrin glue (iPS-FG) on cer

Complications of carotid stenting during live transmissions.

OBJECTIVES: We sought to examine the acute and subacute results of carotid stenting performed during live transmissions. BACKGROUND: Teaching courses focusing on live demonstrations of carotid interventions have been the key educational facility for physicians interested in learning state-of-the-art interventional techniques of carotid stenosis treatment. However, starting with the very first live demonstration of interventional procedures, there has been an ongoing discussion whether patients treated during live transmissions are at higher risk. METHODS: Between March 1, 2001, and June 30,
