Shared Decision-Making in Curative Uro-Oncology: A Systematic Review.

OBJECTIVES: To synthesize current evidence on shared decision-making (SDM) processes that guide the choice of curative-intent treatment among patients with prostate or bladder cancer. METHODS: Following PRISMA 2020 guidelines, we searched seven databases from inception to November 2024 for randomised, observational, and qualitative studies evaluating SDM in treatment selection for localized prostate or bladder cancer. Two reviewers independently screened, extracted data, and appraised quality using the Mixed-Methods Appraisal Tool. RESULTS: Ten studies (n = 11,506) met inclusion criteria: ei

Isolated Vaginal Recurrence of Urothelial Bladder Cancer on FDG PET/CT.

A 58-year-old woman who underwent partial cystectomy for a high-grade invasive urothelial carcinoma of the left lateral bladder wall 11 months ago was referred of FDG PET/CT scan to evaluate a possible vaginal malignancy detected on a follow-up pelvic MRI 5 days ago. FDG PET/CT showed intense activity of the vaginal mass indicating malignancy and no other lesion suspicious for malignancy. A diagnosis of metastatic high-grade urothelial carcinoma of the vagina was made after removal of the mass.

FDG PET/CT in a Case of Inflammatory Myofibroblastic Tumor of the Bladder.

Inflammatory myofibroblastic tumor of the bladder is rare. We described contrast-enhanced CT and FDG PET/CT findings in an inflammatory myofibroblastic tumor of myxoid-vascular pattern involving the bladder muscular layer and adventitia with extensive edema of the adjacent mucosa. The tumor showed hyperenhancement on postcontrast CT and high-grade activity with SUV max of 31 on PET/CT. The thickened edematous mucosa showed isoenhancement and isometabolism relative to the normal bladder wall. This case indicates inflammatory myofibroblastic tumor should be included in the differential diagnosis

Surrogacy of Intermediate Clinical Endpoints for Overall Survival in Patients With Localized Muscle-Invasive Bladder Cancer.

BACKGROUND: Although intermediate clinical endpoints (ICEs) may expedite completion of randomized controlled trials (RCTs) evaluating perioperative systemic treatments for localized muscle-invasive bladder cancer (MIBC), no validated surrogate for overall survival (OS) has been established. We aimed to assess the surrogacy of pathologic complete response (pCR), pathologic objective response (pOR), and disease-free survival (DFS) for OS. PATIENTS AND METHODS: We analyzed 4,828 patients with MIBC (cT2-T4N0M0) who underwent radical cystectomy (RC) with or without neoadjuvant chemotherapy (NAC) a

Transvascular endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) to confirm urothelial carcinoma metastasis to the pulmonary artery.

Tumours within the pulmonary arteries provide a diagnostic challenge due to their similar appearance to pulmonary emboli. Often, the decision to sample is multidisciplinary and the best approach to take is often unknown. Here we describe a case of urothelial carcinoma which metastasised to the mediastinum with evidence of tumour burden within the pulmonary artery. Sampling of this by endovascular and thoracoscopic means was unsuccessful; however, we were able to provide a diagnosis using direct visualization with endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA). Thi

Corneal stromal metastasis of urothelial carcinoma in a dog.

A 15-year-old intact male Miniature Dachshund presented with severe hematuria. Clinical evaluations of this dog raised the suspicion of a malignant bladder tumor, and corneal stromal opacity was observed in the left eye. Because of the critical condition, the dog died on the day of presentation, after which a necropsy was performed. Necropsy and histopathological examinations confirmed urothelial carcinoma (UC) in the bladder. Moreover, the corneal stromal opacity represented a metastatic lesion of UC, and similar metastatic lesions were identified at 13 other sites. The UC in this dog was con

Urine-Stable Aptamer-Conjugated Gold Nanorods for the Early Detection of High-Grade Bladder Cancer Residual Disease.

The limitations of current diagnostic imaging techniques and therapies for bladder cancer are associated with and responsible for the recurrence and progression of residual disease, with an impact on social costs and quality of life of patients. This study delivers a cost-effective solution for the detection of bladder cancer residual disease, which is represented by the early detection of bladder cancer lesions < 1 mm. Urine-stable 34-mer SH-terminated 2'F-Py-RNA aptamer that recognizes the integrin α5β1, expressed by 81% of human high-grade non-muscle invasive bladder cancer, is developed. T

Individualizing first-line treatment for advanced urothelial carcinoma: A favorable dilemma for patients and physicians.

The treatment landscape for patients with advanced urothelial carcinoma (UC) has evolved rapidly in recent years. In current guidelines, combination treatment with enfortumab vedotin plus pembrolizumab is the first-line (1L) standard of care, and other recommended 1L treatment options are platinum-based chemotherapy followed by avelumab as switch-maintenance treatment in patients without progression, or combination treatment with nivolumab, cisplatin, and gemcitabine for cisplatin-eligible patients only. Individual patients differ in terms of their health status, disease characteristics, expec

[Current and future treatment stratification of metastatic urothelial carcinoma: a great opportunity for true precision oncology].

For many decades, the treatment of metastatic urothelial carcinoma (mUC) has been dominated by platinum-containing chemotherapies. The latest version is the introduction of immune checkpoint inhibitors, especially in combination with modern antibody-drug conjugates (ADC) and FGFR3 inhibitors, but this therapeutic standard has been overtaken in some indications. Nevertheless, biomarker-guided therapy decisions have rarely been used to date. However, the biomarkers available to date have only helped in isolated therapeutic situations. For ADCs, however, this could change substantially when there

Efficient Training Corpus Retrieval for Large Language Model Fine Tuning: A Case Study in Cancer.

The objective is to create an automated knowledge extraction tool for cancer research that builds high-quality academic corpora for LLM fine-tuning while investigating its effectiveness in interleukin-6 and bladder cancer domains. To address the current gap in knowledge retrieval techniques for cancer research data collection, we propose KnowledgePipeline, a novel automated tool that incorporates diverse aspects of academic papers and metadata. Our tool integrates content, co-citations, and co-authorship networks to construct domain-specific academic corpora suitable for fine-tuning LLMs. We l

Comparative histologic survey and transcriptomic investigation into canine prostate carcinoma.

Dogs share features in prostate gland anatomy, physiology, and pathology with men. However, human and canine prostate carcinoma (PC) have histologic and molecular differences. Particularly, the histogenesis of canine PC (cPC) is unclear. This study investigated the origin of cPC using histopathology and transcriptomics with comparison to men. Prostate glands retrospectively and prospectively collected from 445 dogs (approximately 95 % autopsy samples) were surveyed for early carcinomas and preneoplastic lesions, particularly high-grade prostatic intraepithelial neoplasia (HGPIN) due to its rol

Is organ preserving radical cystectomy in women oncologically safe and does it improve quality of life?

INTRODUCTION: Radical cystectomy in women with bladder cancer traditionally involves removal of reproductive organs, often leading to significant impairment in quality of life. Organ-preserving radical cystectomy (OPRC) has emerged as an alternative approach aimed at maintaining functional outcomes without compromising oncological safety. METHODS: This systematic review evaluates current evidence on oncological, surgical, and functional outcomes of OPRC. A comprehensive literature search was conducted using the PubMed, Scopus, and Cochrane databases up to April 2025. Studies were assessed for

Genetic profiling of upper tract urothelial carcinoma: A necessity for precision medicine.

INTRODUCTION: The urothelium of the upper tract (renal pelvis and ureter) has morphological and functional similarities to the bladder. There are also morphological similarities between the urothelial carcinoma of the upper tract (UTUC) and the bladder (UCB). However, there are differences in the embryological origin, phenotype, disease course, and response to treatment between the upper tract and bladder urothelium and their corresponding malignancies. Comprehensive genomic studies can provide valuable information about the differences between UTUC and UCB, which have diagnostic and therapeut

Age related trends in the utilization of neoadjuvant chemotherapy for muscle invasive bladder cancer.

PURPOSE: The current standard of care for muscle invasive bladder cancer (MIBC) is neoadjuvant chemotherapy (NAC) followed by radical cystectomy (RC). Previous research has shown under-utilization of NAC for treatment of MIBC, especially among the elderly. Our aim was to stratify NAC use by decade of life and analyze trends in use over time along with recording pathologic downstaging and perioperative outcomes. MATERIALS AND METHODS: The National Cancer Database was queried for patients with cT2-4NanyM0 MIBC treated with RC from 2010 to 2016 with urothelial carcinoma. Nineteen thousand nine h

Immunogenicity of avelumab in patients with metastatic Merkel cell carcinoma or advanced urothelial carcinoma.

Like other monoclonal antibodies, immune checkpoint inhibitors may be immunogenic in some patients, potentially affecting pharmacokinetics (PKs) and clinical outcomes. In post hoc analyses, we characterized antidrug antibody (ADA) development with avelumab monotherapy in patients with metastatic Merkel cell carcinoma (mMCC) from the JAVELIN Merkel 200 trial (first-line [1L; N = 116] and second-line or later [≥2L; N = 88] cohorts) or with advanced urothelial carcinoma (aUC) from the JAVELIN Bladder 100 (1L maintenance [N = 350]) and JAVELIN Solid Tumor (≥2L [N = 249]) trials.
