Cardiac remodeling induced by 13-cis retinoic acid treatment in acne patients.

BACKGROUND: Currently, 13-cis-retinoic acid (13-cis-RA) is the most effective therapy for acne. Isotretinoin, a first-generation synthetic 13-cis-RA compound, is associated with numerous adverse effects. To investigate the cardiac effects of 13-cis-RA, acne patients receiving 13-cis-RA were studied. METHODS: Twenty male patients with acne were enrolled in the study. Patients were treated with a dose of 0.5 mg/kg/d of isotretinoin. All participants were assessed prior to treatment and after 10 weeks of therapy with Doppler-echocardiogram. RESULTS: Patients showed reductions in right atrium ve

Regression of pulmonary arteriovenous malformations following heart transplantation.

Pulmonary arteriovenous malformations (PAVMs) can occur following caval to pulmonary artery connection, Glenn and/or Fontan procedure, leading to severe cyanosis and exercise intolerance. It is unknown whether these abnormalities regress or persist following heart transplantation (HTx). Twenty patients with failed Fontan or Glenn procedures were screened for PAVMs prior to HTx by contrast echocardiography, selective pulmonary angiography, and pulmonary venous desaturation. Age at transplant, diagnosis, previous operations, time from Glenn to transplant, systemic oxygenation, hemoglobin level,

Sjögren's syndrome and pulmonary hypertension: Exploring the intricate link with interstitial lung disease.

BACKGROUND: Primary Sjögren's syndrome (pSS) is a systemic autoimmune disease commonly presenting with sicca symptoms and, in approximately one-third of cases, extra-glandular manifestations, including pulmonary involvement. Pulmonary hypertension (pH) is a recognized, though less frequently studied, complication in pSS, particularly in patients with interstitial lung disease (ILD). This study evaluates the prevalence of PH in pSS and the relationship between ILD and cardiovascular risk in this population. METHODS: This cross-sectional study included 63 patients with pSS, diagnosed according

A multicomponent cardiac rehabilitation programme as a safe, feasible, and beneficial approach, including patients with left ventricular assist devices: a case report.

BACKGROUND: Left ventricular assist devices (LVADs) are a well-established therapy for advanced heart failure (HF) used either as a bridge-to-transplant or destination therapy. While cardiac rehabilitation programmes (CRPs) are guideline-recommended in other HF stages, specific data on their role in LVAD recipients remains limited. We present a case of tailored CRP in a LVAD-supported patient highlighting the importance of understanding the device's intrinsic limitations to ensure safe and effective exercise prescription. CASE SUMMARY: A 56-year-old man with cardiovascular risk factors presen

Premature Ventricular Contraction Coupling Interval Variability Destabilizes Cardiac Neuronal and Electrophysiological Control: Insights From Simultaneous Cardioneural Mapping.

BACKGROUND: Variability in premature ventricular contraction (PVC) coupling interval (CI) increases the risk of cardiomyopathy and sudden death. The autonomic nervous system regulates cardiac electrical and mechanical indices, and its dysregulation plays an important role in cardiac disease pathogenesis. The impact of PVCs on the intrinsic cardiac nervous system, a neural network on the heart, remains unknown. The objective was to determine the effect of PVCs and CI on intrinsic cardiac nervous system function in generating cardiac neuronal and electric instability using a novel cardioneural m

Hemodynamic effects of Purkinje potential pacing in the left ventricular endocardium in patients with advanced heart failure.

BACKGROUND: Various difficulties can occur in patients who undergo cardiac resynchronization therapy for drug-refractory heart failure with respect to placement of the left ventricular (LV) lead, because of anatomical features, pacing thresholds, twitching, or pacing lead anchoring, possibly requiring other pacing sites. The goal of this study was to determine whether Purkinje potential (PP) pacing could provide better hemodynamics in patients with left bundle branch block and heart failure than biventricular (BiV) pacing. METHODS: Eleven patients with New York Heart Association functional cl

Fate of functional tricuspid regurgitation in aortic stenosis after aortic valve replacement.

OBJECTIVE: Functional tricuspid regurgitation (TR) is found not infrequently in conjunction with aortic stenosis. The aim of the present study was to evaluate the changes in TR and to identify the predictors of late progression after aortic valve replacement. METHODS: We evaluated 354 patients who had undergone aortic valve replacement for aortic stenosis from January 1995 to December 2009. Patients with mitral regurgitation were excluded. Of the 354 patients, 54 had TR greater than mild. The mean follow-up duration was 4.4±4.3 years (maximum, 15). The serial echocardiographic and clinical da

Evaluation of the Remodeling of the Tricuspid Annulus and Right Heart in Persistent Atrial Fibrillation Patients With or Without Radiofrequency Ablation via Three-Dimensional Echocardiography.

BACKGROUND: Atrial fibrillation (AF) is linked to tricuspid annulus (TA) and right heart (RH) remodeling. Despite advances in catheter ablation, data regarding its structural impact on the TA and RH chambers remain limited. Therefore, the impact of radiofrequency ablation (RFA) on TA and RH remodeling in persistent AF patients is less clear. Furthermore, the potential of three-dimensional echocardiography (3DE) for evaluating TA and RH remodeling following RFA requires further exploration. This study aimed to characterize TA and RH geometry and function in persistent AF patients before and aft

Rationale and design of the PHOspholamban RElated CArdiomyopathy intervention STudy (i-PHORECAST).

BACKGROUND: The p.Arg14del (c.40_42delAGA) phospholamban (PLN) pathogenic variant is a founder mutation that causes dilated cardiomyopathy (DCM) and arrhythmogenic cardiomyopathy (ACM). Carriers are at increased risk of malignant ventricular arrhythmias and heart failure, which has been ascribed to cardiac fibrosis. Importantly, cardiac fibrosis appears to be an early feature of the disease, occurring in many presymptomatic carriers before the onset of overt disease. As with most monogenic cardiomyopathies, no evidence-based treatment is available for presymptomatic carriers. AIMS: The PHOsph

Impaired biventricular deformation in Marfan syndrome: a strain and strain rate study in adult unoperated patients.

OBJECTIVE: To investigate the presence of any regional myocardial deformation abnormalities in Marfan syndrome (MFS) and determine the benefits of using advanced echocardiography compared to conventional techniques. BACKGROUND: Myocardial dysfunction in MFS may be caused by extracellular matrix remodeling thus, resulting in uniform reduced functionality. However, increased aortic stiffness may cause segmental ventricular abnormalities. Strain rate imaging (SRI) constitutes a validated technique to assess regional deformation in various clinical conditions. With this in mind, we aimed to inves

Minimally invasive anterior thoracotomy approach for complex sinus venosus atrial septal defect with anomalous pulmonary venous return: A case report.

INTRODUCTION: Sinus venosus atrial septal defect (SVASD) is a rare congenital anomaly often associated with partial anomalous pulmonary venous return (PAPVR), leading to right heart volume overload and pulmonary hypertension. While traditionally managed via median sternotomy, minimally invasive approaches are emerging as effective alternatives, offering reduced surgical complication and improved outcomes. CASE PRESENTATION: We report a case of a 59-year-old female with a one-year history of recurrent episodes of spontaneous palpitations. Echocardiographic evaluation revealed a high SVASD with
