OBJECTIVE: Safety and effectiveness of acoustic reflex tests are important issues because these tests are widely applied to screen for retrocochlear pathology. Previous studies have reported moderately high sensitivity and specificity for detection of acoustic neuroma. However, there have been reports of possible iatrogenic hearing loss resulting from acoustic reflex threshold (ART) and decay (ARD) tests. This study assessed safety and clinical performance of ART tests for detection of acoustic neuroma.
DESIGN: We report a case in which ARD testing resulted in a significant bilateral permanen

BACKGROUND: Microsurgery and single-fraction radiosurgery for acoustic neuromas are associated with high rates of control, but can result in facial palsy and trigeminal neuropathy. To reduce the morbidity of treatment for acoustic neuromas while maintaining efficacy, we explored fractionated stereotactic radiosurgery (FSR).
METHODS: We reviewed data for 31 acoustic neuromas in 30 patients treated with 25 Gy (linear accelerator) given in 5 consecutive daily fractions. The minimum follow-up was 6 months (6-44 months). The mean tumor volume was 1.1 cm(3) (0.1-8.74 cm(3)).
RESULTS: All tumors re

In this report, we present five cases of acoustic neuroma in children; one of them concern a case of neurofibromatosis 2. The most common symptom is a deafness, but it can also be revealed by a tinnitus, a vertigo, a facial nerve paralysis or headache. Diagnosis is confirmed by T1 and T2-weighted MRI with intravenous infusion of gadolinium. The deals of the treatment are tumor's control and preservation of the hearing and facial nerve functions. The means used are either microsurgery with a preference for posteriors approaches to translabyrinthic one, or radiosurgery with gamma knife which see

Intra-operative neurophysiologic monitoring (IOM) is rapidly evolving as an important adjunct during acoustic neuroma surgery to reduce the incidence of neurologic deficits. Monitoring alerts the surgeon to ongoing changes in neural function. The benefit of facial nerve monitoring in reducing the incidence of facial palsy during acoustic tumor resection appears clear and is now recommended by the National Institutes of Health (Consensus Development Conference on Acoustic Neuroma, 1991). Auditory monitoring is not as effective as facial monitoring but hearing preservation can be enhanced partic

Alterations of the p53 tumor suppressor gene in 21 cases with acoustic neuroma were investigated by polymerase chain reaction-restriction fragment polymorphism (PCR-RFLP) and single strand conformation polymorphism (PCR-SSCP). Neither mutation nor deletion was found. In 13 informative cases, no loss of heterozygosity (LOH) was confirmed. Thus our results further substantiate the scant contribution of p53 gene tumorigenesis and cell proliferation to acoustic neuromas.

AIM OF THE STUDY: Postoperative sensory component of the facial nerve after acoustic tumor surgery has received little attention in the literature. The object of the present investigation was to review this specific topic analyzing the postoperative frequency of taste and lacrimation (crocodile tears or dry eye) abnormalities.
MATERIAL AND METHODS: 54 patients who underwent acoustic tumor removal were selected for this study. Each of these patients was recalled and pre and postoperative evolution of the sensory dysfunction were assessed. The latters were correlated with the facial function ev

Surgical series of hearing preservation in acoustic neuroma usually emphasize a team's results with one particular technique. This report reviews acoustic neuroma outcomes of individualizing the surgical approach to patient and tumor characteristics. This study reviews 60 consecutive hearing preservation acoustic neuroma surgeries in a total series of 330 acoustic neuromas. Tumor sizes ranged from 0.3 to 4 cm in patients ranging from 23 to 74 years of age. Middle fossa surgery was performed in 57%, retrosigmoid in 43%. Overall, measurable hearing was preserved in 77%, and useful hearing in 67%

A 22-year-old male with unilateral facial, cochlear and glossopharyngeal cranial nerve dysfunction and ataxia that was slowly progressive over one year is described. Although clinically it was considered he probably had an acoustic neuroma, surgery revealed a pontine glioblastoma exophytic into the cerebellopontine angle and internal acoustic meatus. Clinicoradiological features that distinguish this extremely rare lesion of the hindbrain from acoustic neuroma are discussed.

To improve the therapeutic effect of acoustic neuroma, 158 patients with acoustic neuroma treated in our hospital were retrospectively reviewed. Among them, 11 recurrent acoustic neuroma were found. The clinical analysis demonstrated that the recurrence of acoustic neuroma had close relationship with the size of the tumor, operative approach and extent of excision. All recurrent acoustic neuroma was large-sized when first operated. The more the tumor was excised, the lower the recurrent rate was. The recurrent rates were 19.4% for partial resection, 13.2% for near-total resection, 0% for total

Previous studies have shown that acoustic rhinometry (AR) is well suited to describe the nasal airway dimensions in healthy infants. The technique is quick to perform, non-invasive, without potential hazards and requires minimal cooperation. Due to the small dimensions of the infant nasal airways, the optimized miniprobe (Rhinometrics, Lynge, Denmark) provides abilities superior to those of adult probes. The equipment is portable and examinations can be performed in the maternity ward, out-patient department or at home. Measurements on models simulating the pre- and post-operative nasal geomet

Pre-, per- and post-operative ECoG findings obtained from a 43-year-old woman with an acoustic neuroma were studied. Although CAP presented a normal waveform in response to click during the whole process of the operation, it was transformed into an abnormally broadened negative waveform in shape with a mild hearing exacerbation, and has remained unchanged up to the present. This broad response was considered to be a receptor potential, because it showed no adaptational amplitude reduction. Additionally, postoperative ECoG to tone burst stimuli demonstrated an increase in DC potential following

Cystic acoustic neurinomas (ANs) are less frequent and are different from solid ANs in clinical and radiological features. We had 14 cystic ANs (13.5% of 104 cases) in the last 17 years. Computerized tomographic or magnetic resonance images allowed for the classification of these cystic ANs into three types: Type A being large single cysts with a thin tumourous wall (7 cases); type B single cysts with a thick tumourous wall (3 cases); type C multicystic (4 cases). Half of the cystic ANs were not accompanied by enlargement of the internal auditory canal, despite the largeness of the cysts. The

Electrocochleographic cochlear microphonics (ECochG-CM) and Evoked Otoacoustic Emissions (EOAE) were recorded for 17 patients with acoustic neuromas to assess their cochlear functions. ECochG-CM was recorded by a transtympanic needle electrode technique. The acoustic stimuli of EOAE were short tone bursts and thresholds of detection of the slow components were determined. In 13 of the 17 patients, elevated detection thresholds of ECochG-CM and/or abnormal interaural differences in detection thresholds of the slow components of EOAE were observed. The patients with hearing loss and an elevated

The management of hydrocephalus in association with a cerebellopontine angle tumor is controversial. There is a widely held belief that initial therapy should always be directed toward treatment of hydrocephalus before definitive surgery. The potential problems of cerebrospinal fluid (CSF) shunting and drainage have to led to an evolution in the management of hydrocephalus at St. Vincent's Hospital. There is growing evidence that complete removal of cerebellopontine angle tumors will result in resolution of hydrocephalus without requiring other methods of CSF decompression. The authors present

Third-octave sound analysis was performed on the snoring sounds of nine subjects with obstructive sleep apnoea (OSA) and 18 with simple snoring. Both groups demonstrated a large low frequency peak in linear sound levels at around 80 Hz. However, the OSA group displayed a substantially larger high frequency sound component. We utilized this fact in the development of an acoustic index (Hawke Index: HI) which describes the ratio between the overall A-weighted and linear sound levels for the recorded snoring sound of each subject [HI = dB(A)/dB(SPL) for Lmax]. There was a significant positive cor

Both closing dynamics and the mechanical condition of a Björk-Shiley Convexo-Concave (BSCC) valve are significant in assessing the risks of outlet strut fracture. Risk of fracture increases with the presence of a pre-existing fracture in one of the two strut legs and with magnitude and frequency of loading. Recent analyses of in vivo data collected in clinical studies, and in vitro data from a computer-controlled pulse duplicator, indicate that the condition of an outlet strut can be evaluated by non-invasive passive acoustic measurement. The technique utilizes heuristic methods to identify fe
