X-linked adrenoleukodystrophy (ALD) is a severe neurodegenerative disorder. ALD is characterized by progressive demyelination within the central and peripheral nervous system, adrenal insufficiency (Addison's disease) and accumulation of very-long-chain fatty acids (VLCFA) in plasma, fibroblasts and tissues. The overall incidence of ALD is 1:17,000 including hemizygotes and heterozygotes who are frequently symptomatic. There are two main ALD phenotypes: 1) a cerebral demyelinating form which affects boys between 5-12 years, but also 35% of adult males; 2) a form that mainly involves the spinal

X-Linked Adrenoleukodystrophy or the Management of Rare Diseases in the General Practice Abstract. We report on a 48-year-old patient with X-linked adrenoleukodystrophy with slowly progressive leg-accentuated spastic paraparesis, vegetative dysfunction with bladder and sexual dysfunction, and primary adrenal insufficiency. The diagnosis of adrenomyeloneuropathy, the adult form of adrenoleukodystrophy, and Addison's disease was made at the age of 20 by evidence of an increased concentration of long-chain fatty acids in the plasma. The therapy is symptom-oriented.

Autoimmune polyendocrinopathy candidiasis ectodermal dystrophy (APECED) syndrome is a rare monogenic autosomal recessive disorder caused by biallelic mutations in the AIRE (autoimmune regulator) gene. Patients with APECED present with heterogeneous endocrine and non-endocrine manifestations. In this study, we report an Iranian patient who presented with Addison disease, chronic mucocutaneous candidiasis, alopecia totalis, keratopathy and asplenia treated as an isolated endocrinopathy for 25 years. In the adulthood, the diagnosis of APECED was made by genetic analysis which demonstrated homozyg…

Schmidt's syndrome, also known as polyglandular deficiency syndrome, is the presence of Addison's disease and hypothyrodism in a single patient. It is usually associated with other autoimmune disorders like vitiligo, diabetes mellitus, myasthenia gravis. A rare case of an 18-year-old girl having Schmidt's syndrome and vitiligo who presented with puberty menorrhagia is reported. A brief review of the literature is also given.

A 42-year-old man presenting with symptomatic hypercalcemia was successfully treated with corticosteroids. Initially he was thought to suffer from Addison's disease. A thyrotoxic state was, however, disclosed during the treatment. Evidence suggests that the hypercalcemia was caused by thyrotoxicosis. The effects of thyroid and adrenocortical hormones on calcium metabolism are discussed. Corticosteroids seem valuable in differentiating thyrotoxic hypercalcemia from coincidental hyperparathyroidism.

BACKGROUND: Autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy syndrome (APECED) is an autosomal recessive disease due to mutations of the autoimmune regulator (AIRE) gene. Typical manifestations include candidiasis, Addison's disease, and hypoparathyroidism. Type 1 diabetes, alopecia, vitiligo, ectodermal dystrophy, celiac disease and other intestinal dysfunctions, chronic atrophic gastritis, chronic active hepatitis, autoimmune thyroid disorders, pernicious anemia and premature ovarian failure are other rare associated diseases although other conditions have been associated with

A 58-year-old man presents with spontaneous brown discolorations of his mouth and hands. Our differential diagnosis included Peutz-Jeghers syndrome, Laugier-Hunziker syndrome or Addison's disease. There were no polyps in a previously performed colonoscopy and no other systemic symptoms. We made the diagnosis Laugier-Hunziker syndrome, a benign skin disorder that doesn't require treatment, confirmed by skin biopsy.

Recovery of the ventricular function in a patient with cardiomyopathy is very rare. Autoimmune polyendocrine syndrome is also very rare. We herein report a case of reversed cardiomyopathy associated with autoimmune polyendocrine syndrome type II (Schmidt's syndrome) composed of Addison's disease, vitiligo and Hashimoto's thyroiditis. The ventricular function and size were reversed following the administration of suitable hormone replacement therapy for polyendocrine syndrome.

OBJECTIVE: Co-aggregation of autoimmune diseases is common, suggesting partly shared etiologies. Genetic factors are believed to be important, but objective measures of environmental vs heritable influences on co-aggregation are absent. With a novel approach to twin studies, we aimed at estimating heritability and genetic overlap in seven organ-specific autoimmune diseases.
DESIGN: Prospective twin cohort study.
METHODS: We used a cohort of 110 814 twins to examine co-aggregation and heritability of Hashimoto's thyroiditis, atrophic gastritis, celiac disease, Graves' disease, type 1 diabetes

This report documents a case of isolated adrenal gland cryptococcosis without the often reported component of concomitant meningitis or Addison's disease in an immune competent patient. Furthermore, both the patient's lung cancer and adrenal infection with cryptococcus were incidentally discovered during work-up for syncope in the setting of hyponatremia. This case also underscores the diagnostic value of fine-needle aspiration biopsy in this unusual presentation.

Facial hypermelanosis is a clinical feature of a diverse group of disorders, the most common of which is melasma. Exposure to sunlight, genetic predisposition, the use of cosmetics, and certain drugs are implicated in the pathogenesis of most facial hypermelanoses. A detailed personal and family history and the histopathologic findings are, in most cases, enough for establishing the correct diagnosis. Systemic disorders such as Addison's disease should always be suspected and excluded in clinically relevant cases.

A case of spontaneous premature menopause due to autoimmune ovarian failure is described. This report emphasises that this uncommon condition is important to diagnose because of its association with other autoimmune endocrine disorders including Addison's disease. In addition, in the present case, the marked increase in luteinizing hormone with relatively normal follicle stimulating hormone raises the possibility of a non-steroidal inhibitory feedback of follicle stimulating hormone.

Measurement of plasma cortisol is necessary in assessing the functional state of the adrenal cortex and the availability of other active steroids to the tissues during pregnancy. Coupled with clinical endocrine tests, these determinations can give the physician a good indication of the type of disorder present e.g. Addison's diseases, panhypopituitarism and adrenal hyperfunction. The method used for estimation of venous plasma cortisol when adequately controlled, is very sensitive and accurate.

Hashimoto's thyroiditis is a common autoimmune thyroid disease with preference of female gender. The chronic thyroiditis is characterized by autoantibodies against thyroid peroxidase and thyroglobulin. With manifestation, there is often a subclinical hypothyroidism that finally progresses to a persistent hypothyroidism with typical clinical symptoms and the need of hormonal substitution in succession of the lymphocytic infiltration of the thyroid. The ultrasound of the thyroid shows a hypoechogenic and inhomogeneous parenchyma. Autoimmune thyroiditis is frequently associated with autoimmune di

President John F. Kennedy (JFK) had a complex medical history that is now thought to be an autoimmune polyglandular syndrome type 2 with Addison's disease and hypothyroidism. He also had gastrointestinal symptoms from adolescence, which now fit well with coeliac disease. In addition, he had a chronic back problem, which contributed to a chronic pain syndrome. This review looks at JFK's various diseases and focusses on the history of coeliac disease, as well as its presentation. JFK's Irish ancestry supports the hypothesis of a coeliac disease started early in his youth.

Vitamin B12 deficiency can manifest in various forms, including skin changes in the form of hyperpigmentation. The hyperpigmentation in vitamin B12 deficiency commonly presents over the hands and feet, but presenting over the face and oral cavity is very rare. Here we describe an interesting case where there was hyperpigmentation in the face and oral cavity which was mimicking Addison's disease, but the actual diagnosis was vitamin B12 deficiency. This case stresses that knowing the uncommon manifestations of common disease is more important than knowing the common manifestations of uncommon d

The role of vitamin D in calcium-phosphorus metabolism regulation is the most highlighted, nonetheless there is enormous literature on the extra-skeletal effects of vitamin D, and lately new insight into the role of vitamin D in endocrine disease mechanisms has seen light of day. The present narrative review gives an overview of the proposed roles of vitamin D in the etiology of Hashimoto's thyroiditis, Grave's disease, Addison's disease and primary hyperthyroidism. The implications as pertaining to the routine laboratory practice are readily applicable to this patient group as well, and do no
