Survival of airplane stowaways is rare. Here we report an exceptional case of successful treatment and full recovery. After a transcontinental flight an unconscious stowaway was discovered in a wheel well of a Boeing 747-400F. Airport paramedics confirmed regular respiration and achieved 100% oxygen saturation (pulse oximetry) by high-flow oxygen. Rectal body temperature was 35.5 °C. A 79-year-old woman collided with a cliff in a passenger automobile. The fire department acknowledged an automated collision notification from the D-Call Net (DCN) at 1 min after the accident and called for doctors by helicopter ( "Doctor-Heli" [DH] in Japan) 9 min after the injury. Spontaneous pneumomediastinum (SPM) is a fairly uncommon condition in which free air is present in the mediastinum, often in young patients without injury or serious underlying pulmonary disease. We present a case of SPM in a 36-year-old man with no pertinent medical history who presented with chest pain after a 3-h flight. To the best of our knowledge, this is the first known case in radiology literature of SPM caused by increased intra-alveolar pressure secondary to increased altitude.

Otolaryngologic disease was the third most common medical cause of permanent grounding, accounting for 145 (12.2%) of 1,186 aircrew members permanently grounded for various medical reasons in the General Hospital of the Chinese Air Force, 1961-90. Of the 22 aviators examined, 15 required hard contact lenses for optimal correction of vision. No aircraft accidents or incidents, attributable to visual factors, were documented. We concluded that the majority of aviators with keratoconus are able to continue their flying careers safely with the aid of spectacles or contact lenses.
