Effect of pain on activity and participation in individuals with unilateral lower-extremity amputation based on the International Classification of Functioning, Disability, and Health.

BACKGROUND: Amputation is a major life-changing event and is regarded as one of the loading causes of permanent disability. However, amputation often causes changes in painful and nonpainful sensory experiences. Pain after amputation has been shown to increase functional limitations caused by limb loss and lower quality of life. OBJECTIVE: This study aimed to examine the effect of pain on activity and participation in individuals with unilateral below-knee amputations according to the International Classification of Functioning, Disability, and Health. STUDY DESIGN: Cross-sectional study. M

The use of forearm free fillet flap in traumatic upper extremity amputations.

BACKGROUND: Complete traumatic upper extremity avulsions are an infrequent but devastating injury. These injuries are usually the result of massive blunt trauma to the upper limb. Intact issue from amputated or nonsalvageable limbs may be transferred for reconstruction of complex defects resulting from trauma when the indications for replantation are not met. This strategy allows preservation of stump length or coverage of exposed joints, and provides free flap harvest for reconstruction without additional donor-site morbidity. METHODS: A retrospective review at São João Hospital was performe

Anatomical and functional changes in the organization of the cuneate nucleus of adult rats after fetal forelimb amputation.

A previous study has shown that fetal forelimb removal in the rat results in an increase in the size of the hindlimb representation in primary somatosensory cortex and suggested that this anomalous cortical organization may have resulted from alterations in the primary afferent innervation of the dorsal column nuclei (Killackey and Dawson, 1989). The present study used both anatomical and electrophysiological techniques to examine the effects of fetal forelimb amputation on the dorsal column nuclei. Rats sustained forelimb removals on embryonic day 16 and were used in terminal experiments when

Retrograde Trans-Amputation Embolectomy of Common, Superficial and Deep Femoral Arteries for Inflow Revascularisation During Above-Knee Amputation: A Case Report and Review of the Literature.

BACKGROUND: Primary above-knee amputation (AKA) may at times be the only option for unsalvageable acute lower limb ischemia. However, occlusion of the femoral arteries may result in poor inflow and contribute to wound complications such as stump gangrene and sepsis. Previously attempted inflow revascularisation techniques include surgical bypass and percutaneous angioplasty and/or stenting. CASE PRESENTATION: We present a case of a 77-year-old lady with unsalvageable acute right lower limb ischemia secondary to cardioembolic occlusion of the common (CFA), superficial (SFA) and deep (PFA) femo

Is skin pressure a relevant factor for socket assessment in patients with lower limb amputation?

BACKGROUND: Prosthetic rehabilitation improves the overall quality of life of patients, despite discomfort and medical complications. No quantitative assessment of prosthesis-patient interaction is used in routine protocols and prosthesis quality still results from the manufacturer's know-how. OBJECTIVE: Our objective is to investigate whether pressure can be a relevant factor for assessing socket adequacy. METHODS: A total of 8 transtibial amputee volunteers took part in this experimental study. The protocol included static standing and 2 minutes walking tests while the stump-to-socket inte

Reconstruction of fingertip and stump using a composite graft from the hypothenar region.

Finger amputation with bone exposure when replantation is not feasible requires a procedure for closure and padding of the stump. To preserve the length and provide adequate coverage, various flaps are used. A situation may occur in which local flaps are not sufficient and distant flaps are preferred. However, distant flaps often require a two-stage procedure, prolonged immobilization, and skin grafts. Thus, a simpler approach for fingertip or stump reconstruction while maintaining the padding effect of a flap was designed. The composite of glabrous skin and subcutaneous fat provided such padd

Peripheral Interventions for Painful Stump Neuromas of the Lower Limb: A Systematic Review.

OBJECTIVES: Painful stump neuromas in lower limb amputees are a significant burden on a person's quality of life due to interference with wearing prostheses and therefore the ability to walk. Treating painful stump neuromas is a challenge perhaps reflected by the lack of clinical guidelines. MATERIALS AND METHODS: A systematic review was conducted to evaluate the efficacy of all treatments tried in the management of symptomatic neuromas in the lower limb amputation stump in order to establish whether one treatment is superior. RESULTS: Twenty-two studies were included in the final review whi

A finite element analysis of load distribution during donning and orthostatic posture in the ITOP hybrid subischial socket.

INTRODUCTION: Pressure and shear stresses applied to the stump of a transfemoral amputee wearing a newly designed prosthetic socket have been analyzed by a finite element modeling approach. METHODS: The new socket was developed by the Istituto Tecnico Ortopedico Preneste, and it was named the "hybrid subischial socket." This work aimed at understanding the loads' distribution on the stump surface in 2 operative conditions: at the end of the wearing phase and during the orthostatic posture. The model of the stump was composed of 4 different materials: the femoral bone, the muscle tissue, the f

A Sensor-Based Decision Support System for Transfemoral Socket Rectification.

A decision support system (DSS) was developed that outputs suggestions for socket-rectification actions to the prosthetist, aiming at improving the fitness of transfemoral prosthetic socket design and reducing the time needed for the final socket design. For this purpose, the DSS employs a fuzzy-logic inference engine (IE) which combines a set of rectification rules with pressure measurements generated by sensors embedded in the socket, for deciding the rectification actions. The latter is then processed by an algorithm that receives, manipulates and modifies a 3D digital socket model as a tri

A sensor to monitor limb depth in transtibial sockets with locking pin suspension: a technical note.

BACKGROUND: Monitoring of limb depth in transtibial sockets may provide useful information toward patient education and care. OBJECTIVE: The objective was to develop a sensor to detect the depth of a locking pin in the shuttle lock of a transtibial socket and to monitor the small motions between ratchet clicks during ambulation. STUDY DESIGN: Controlled bench testing and single-participant study. METHODS: A copper wire coil positioned beneath the socket shuttle lock was used with an inductive sensing chip to monitor locking pin depth. A custom jig was used to calibrate the sensor and bench

Sockets for Limb Prostheses: A Review of Existing Technologies and Open Challenges.

In the prosthetics field, one of the most important bottlenecks is still the human-machine interface, namely the socket. Indeed, a large number of amputees still rejects prostheses or points out a low satisfaction level, due to a sub-optimal interaction between the socket and the residual limb tissues. The aim of this paper is to describe the main parameters (displacements, stress, volume fluctuations and temperature) affecting the stump-socket interface and reducing the comfort/stability of limb prostheses. In this review, a classification of the different socket types proposed in the literat

Motion compensation in a tomographic ultrasound imaging system: Toward volumetric scans of a limb for prosthetic socket design.

Current methods of prosthetic socket fabrication remain subjective and ineffective at creating an interface to the human body that is both comfortable and functional. Though there has been recent success using methods like magnetic resonance imaging and biomechanical modeling, a low-cost, streamlined, and repeatable process has not been fully demonstrated. Medical ultrasonography, which has significant potential to expand its clinical applications, is being pursued to acquire data that may quantify and improve the design process and fabrication of prosthetic sockets. This paper presents a new

Skin problems in lower limb amputees: an overview by case reports.

The stump in lower limb amputees is prone to skin problems because it is exposed to several unnatural conditions (shear and stress forces and increased humidity) when a prosthesis is used. This study reviews the literature on case reports of lower limb amputees with skin problems on the stump. In total, 56 reports comprising 76 cases were identified in the literature. The main disorders are acroangiodermatitis, allergic contact dermatitis, bullous diseases, epidermal hyperplasia, hyperhidrosis, infections, malignancies and ulcerations.

Shape and volume change in the transtibial residuum over the short term: preliminary investigation of six subjects.

A preliminary investigation was conducted to characterize the magnitude and distribution of volume change in transtibial residua at two time intervals: upon prosthesis removal and at 2 week intervals. Six adult male unilateral transtibial amputee subjects, between 0.75 and 40.0 years since amputation, were imaged 10 times over a 35-minute interval with a custom residual limb optical scanner. Volume changes and shape changes over time were assessed. Measurements were repeated 2 weeks later. Volume increase on socket removal for the six subjects ranged from 2.4% to 10.9% (median 6.0% +/- standar

Bone overgrowth in the adult traumatic amputee.

Bone overgrowth of the residual limb after an amputation is a well documented complication in the pediatric amputee population. Bone overgrowth can cause pain, problems with skin breakdown, and poor prosthetic fit. There have been few reports of bone overgrowth in the adult amputee. Two cases of traumatic transfemoral amputations after extensive tissue damage are presented. Both patients successfully completed an in-patient amputee rehabilitation program and achieved functional ambulation with their prostheses. However, each developed distal residual limb pain within a year after their amputat
