The role of nasopharyngectomy in the management of nasopharyngeal carcinoma.

PURPOSE OF REVIEW: Recently, endoscopic nasopharyngectomy (ENPG) has become an effective treatment for locally recurrent nasopharyngeal carcinoma (NPC). This article reviews recent publications on ENPG and specifically addresses the surgical anatomy of the nasopharynx and discusses several important issues regarding ENPG. RECENT FINDINGS: The surgical techniques for ENPG have been previously described in several studies. The latest published data revealed good outcomes of ENPG compared with intensity-modulated radiation therapy (IMRT) in recurrent NPC. In addition, ENPG avoids severe reirradi

Cardiac MR imaging and MR angiography for assessment of complex tetralogy of Fallot and pulmonary atresia.

Breath-hold electrocardiographically gated cardiac magnetic resonance (MR) imaging and contrast material-enhanced MR angiography are emerging as ideal techniques for the evaluation of complex congenital heart disease. Tetralogy of Fallot is the most common cause of cyanotic congenital heart disease and, in its classic form, is associated with varying degrees of hypoplasia of the central and peripheral pulmonary arteries, with valvar pulmonary atresia and collateral aortopulmonary vessels occupying the extreme end of the spectrum. Accurate assessment of the size and anatomy of the pulmonary art

Frame-Based Stereotactic Endoscopic Third Ventriculostomy-Toward Improved Precision and Minimizing Morbidities.

BACKGROUND: Endoscopic third ventriculostomy (ETV) is a widely accepted treatment for obstructive hydrocephalus. For most practitioners, this procedure will be performed without navigation guidance. Without such guidance, the complications associated with the procedure have ranged from 1% to 8%. We hypothesized that the discrepancy between the surface bony anatomy and internal cranial anatomy contributes to the morbidities associated with ETV. We tested this hypothesis by comparing the position of the entry point defined by the classic Kocher's point relative to the ideal entry point that woul

Exposure of the V1 Segment of the Vertebral Artery: Stepwise Cadaveric Surgical Simulation.

BACKGROUND: Surgical exposure of the V1 segment of the vertebral artery (V1-VA) at the lower neck may be necessary to address intravascular (atherosclerotic) and extravascular (external compression by neoplastic or degenerative) pathologies. The adjacent anatomy at the lower cervical region is complex and relatively unfamiliar to neurosurgeons compared to that of upper cervical levels. High-quality cadaveric images simulating the surgical approach to V1-VA are important for learning the relevant anatomy. OBJECTIVE: To provide a brief stepwise depiction of the exposure of the V1-VA using a cad

The use of 3-dimensional (3D) printing in teaching musculoskeletal oncology for medical undergraduates.

INTRODUCTION: The approach to integrating relevant anatomy in the medical curriculum has been debated for many years. Current literature has explored the broad impact of 3D printing in medical education. However, there is little evidence on 3D printing for the teaching of musculoskeletal oncology (MSO). This is a self-controlled case series (SCCS) study that aims to analyse the effectiveness of 3D printed models in MSO in enhancing the learning experience, engagement and understanding of clinical and surgical anatomy for medical students. METHOD: A cross-sectional cohort study involving 75 cl

Do active learning techniques promote higher academic performance in an online graduate anatomy course?

Human anatomy is a foundational course in graduate health professional programs. Given the increased enrollment in anatomy courses, along with the development of new technologies, institutions are increasingly compelled to provide alternative course formats, including online learning. Similarly, higher education is increasingly transitioning from traditional lecture-based learning to methods that allow students to actively participate in their own learning process (i.e., active learning). Active learning has been shown to have many benefits, including enhanced academic performance. Few studies

Lateral Interbody Fusion at L4/5: Management of the Transitional Psoas.

OBJECTIVE: Managing retraction of the lumbar plexus is critical to safely perform lateral lumbar interbody fusion (LLIF) via the transpsoas approach. Occasionally, a transitional psoas is encountered at L4/5 and has been postulated to be a contraindication to transpsoas LLIF. A case series of patients with transitional psoas who underwent L4/5 LLIFs is presented. METHODS: This retrospective review assessed 79 consecutive patients who underwent L4/5 LLIF during a 24-month period. Preoperative imaging was reviewed, and patients were classified into 2 groups: normal psoas or transitional psoas.

Ethnic Differences in Western and Asian Sacroiliac Joint Anatomy for Surgical Planning of Minimally Invasive Sacroiliac Joint Fusion.

Pain originating in the sacroiliac joint (SIJ) is a contributor to chronic lower back pain. Studies on minimally invasive SIJ fusion for chronic pain have been performed in Western populations. Given the shorter stature of Asian populations compared with Western populations, questions can be raised regarding the suitability of the procedure in Asian patients. This study investigated the differences in 12 measurements of sacral and SIJ anatomy between two ethnic populations by analyzing computed tomography scans of 86 patients with SIJ pain. Univariate linear regression was performed to evaluat

Preoperative evaluation of hilar vessel anatomy with 3-D computerized tomography in living kidney donors.

OBJECTIVES: Digital subtract angiography is the gold standard for anatomic assessment of renal vasculature for living renal donors. However, multidetector-row computerized tomography (MDCT) is less invasive than digital subtract angiography and provides information of kidney stones and other intra-abdominal organs. In this study, preoperative MDCT angiography results were compared with the peroperative findings to evaluate the accuracy of MDCT for the evaluation of renal anatomy. METHODS: From December 2002 to May 2007, all 60 consecutive living kidney donors were evaluated with MDCT angiogra

Lower urogenital tract anatomical and functional phenotype in lysyl oxidase like-1 knockout mice resembles female pelvic floor dysfunction in humans.

Female pelvic floor dysfunction (FPFD) is a complex group of conditions that include urinary incontinence and pelvic organ prolapse (POP). In humans, elastin homeostasis has been implicated in the pathophysiology of FPFD. Lysyl oxidase-like 1 knockout (LOXL1-KO) mice demonstrate abnormal elastic fiber homeostasis and develop FPFD after parturition. We compared the lower urogenital tract (LUT) anatomy and function in LOXL1-KO mice with and without POP. LUT anatomy was assessed in LOXL1-KO mice over 28 wk. Pelvic visceral anatomy in LOXL1-KO was evaluated with a 7-Tesla magnetic resonance imagin

Repositionable and retrievable Lotus Valve System for the treatment of bicuspid aortic stenosis: is it time to treat all valves?

For a long time, a bicuspid valve has been considered a contraindication to transcatheter aortic valve implantation (TAVI) due to worse procedural results. Currently available technologies are changing the scenario, with initial experiences reporting much better results with second-generation devices. Areas covered: This review will analyze the risk and potential pitfalls of TAVI on bicuspid valves and will try ascertain how the use of a new-generation fully repositionable device, the Lotus Valve System (LVS), could improve procedural and clinical results in bicuspid anatomies. Expert commenta

[MINIMALLY INVASIVE SURGERY FOR COLORECTAL CANCER BASED ON VIRTUAL SURGICAL ANATOMY].

For advanced right colon cancer, we perform lymph node dissection exposing the so-called surgical trunk. For the resection of advanced distal sigmoid/rectal cancer, we routinely perform lymph node dissection around the root of the inferior mesenteric artery, preserving the left colic artery. To perform either of these procedures safely, it is important to know the precise vascular anatomy with individual variations. However, there are major issues in laparoscopic surgery, such as a lack of tactile sensation and limited visual field. To overcome these issues and identify the vascular anatomy of

Anatomical relationships between testis and epididymis during the fetal period in humans (10-36 weeks postconception).

OBJECTIVE: To determine the anatomy of the epididymis and its relationship with the testis during the fetal period in normal individuals. METHODS: We studied bilaterally 146 testes and epididymides taken from 73 normal fresh human fetuses ranging in age from 10 to 36 weeks postconception. The epididymal anatomy was classified in six types: type I: the epididymis is connected to the testis by its head and tail and the epididymal body is separated from the testis; type II: the epididymis is completely attached to the testis; type III: the epididymis is attached to the testis only by its head; t
