Antibacterial activity of fosfomycin against uropathogens.

BACKGROUND: In recent years, the rising rates of resistance to antimicrobial drugs among pathogens have caused great difficulty for clinicians treating infectious diseases. The aim of the study was to assess the curative effect of fosfomycin in treating urinary tract infections (UTIs) in China. METHODS: We collected clinical isolates of UTIs to determine their susceptibility to fosfomycin by the agar dilution method and to analyze extended-spectrum β-lactamase (ESBL)-producing isolates by the double-disc method on Mueller-Hinton agar. Fosfomycin-modifying enzyme analysis was conducted by PCR.

Comparative efficacy of commercially available and emerging antimicrobial urinary catheters against bacteriuria caused by E. coli in vitro.

OBJECTIVES: To compare the efficacy of both commercially available and emerging urinary catheter technologies in relation to their effects on bacteriuria caused by Escherichia coli in vitro. Antiseptic urinary catheters have recently become commercially available and others are in the developmental stage. METHODS: Silver alloy-coated catheters, antibiotic Nitrofurazone (NF)-coated catheters, and nitric oxide (NO)-coated catheters were tested against a noncoated control for their antiseptic ability. Inhibition of bacterial growth, biofilm formation, and the number of live bacteria within the b

Antibiotic treatment of women with uncomplicated cystitis before and after allowing pharmacist-supply of trimethoprim.

Background In 2012, New Zealand reclassified trimethoprim to allow specially trained pharmacists to supply it without a prescription to women with symptoms suggesting uncomplicated cystitis, under strict criteria for supply. Objective To assess how this policy change allowing pharmacist supply of trimethoprim affected overall antibiotic supply. Setting Randomly selected community pharmacies throughout New Zealand. Methods Data were collected in pharmacies before the implementation ('baseline') and 1 year later ('post-implementation'). Pharmacy staff recorded prescription and nonprescription su

Phage- and enzybiotic-coated urinary catheters to prevent recurrent multidrug-resistant bacterial infections.

INTRODUCTION: Catheter-associated urinary tract infections (CAUTIs) are among the most prevalent hospital-acquired infections, posing a serious clinical and economic burden, particularly in the context of rising antibiotic resistance. Biofilm formation on indwelling catheters by multidrug-resistant uropathogens further complicates treatment and prevention, underscoring the urgent need for alternative, non-antibiotic solutions. AREAS COVERED: This article explores the potential of bacteriophage- and enzybiotic-coated urinary catheters as an innovative strategy to prevent recurrent bacterial in

Bladder catheterization increases susceptibility to infection that can be prevented by prophylactic antibiotic treatment.

Catheter-associated urinary tract infections (CAUTI) are the most common hospital-associated infections. Here, we report that bladder catheterization initiated a persistent sterile inflammatory reaction within minutes of catheter implantation. Catheterization resulted in increased expression of genes associated with defense responses and cellular migration, with ensuing rapid and sustained innate immune cell infiltration into the bladder. Catheterization also resulted in hypersensitivity to Enterococcus faecalis and uropathogenic Escherichia coli (UPEC) infection, in which colonization was ach

[Do general practitioners follow the national guidelines for treating urinary tract infections with antibiotics?].

BACKGROUND: Incorrect use of antibiotics is a major public health concern both nationally and globally due to the development of antibiotic resistance. The goal of this study was to see if prescription of antibiotics for urinary tract infections in general practice was in accordance with national guidelines. MATERIAL AND METHODS: We combined two sets of data from February and March 2003: prescriptions of antibiotics redeemed in pharmacies, and electronic billing cards collected from the National Insurance Agency from 145 general practitioners in Vestfold county. We analysed all consultations

[The Dutch College of General Practitioners' guideline on urinary tract infections: response from the viewpoint of internal medicine].

In 1999 the Dutch College of General Practitioners (NHG) and the Dutch Institute for Healthcare Improvement (CBO) published revisions of their guidelines on urinary tract infections. Contrary to previous editions, the revised guidelines agree on most points, but not all. To diagnose a urinary tract infection in non-pregnant women with dysuria, the NHG guideline advises a nitrite test and a dip-slide culture if the nitrite test is negative. Although the dip-slide improves the diagnostic accuracy, a considerable number of patients have to wait at least 24 hours before therapy is given. The diagn

Postoperative voiding, bacteriuria and urinary tract infection with Foley catheterization after gynecological surgery.

BACKGROUND: The use of bladder drainage to avoid urinary retention after gynecological surgery is more or less custom based, and duration of drainage varies considerably. In this paper the use of 1-day drainage by transurethral Foley catheter was investigated with regard to impaired voiding, asymptomatic bacteriuria, and urinary tract infection. Furthermore, the use of methenamine hippurate was studied with regard to postoperative asymptomatic bacteriuria and urinary tract infection. METHODS. This summary is based on six published papers totaling 917 patients. In three case series, 1-day cathe

Resistance in gram-negative bacteria: enterobacteriaceae.

The emergence and spread of resistance in Enterobacteriaceae are complicating the treatment of serious nosocomial infections and threatening to create species resistant to all currently available agents. Approximately 20% of Klebsiella pneumoniae infections and 31% of Enterobacter spp infections in intensive care units in the United States now involve strains not susceptible to third-generation cephalosporins. Such resistance in K pneumoniae to third-generation cephalosporins is typically caused by the acquisition of plasmids containing genes that encode for extended-spectrum beta-lactamases (

Purple Urine Bag Syndrome.

The purple urine bag syndrome (PUBS) i.e. purple discolouration of urine, is an uncommon and alarming condition in patients with long term urinary catheterisation. Though the condition is benign, it should draw immediate attention to the possibility of an underlying urinary tract infection. The postulated hypothesis for this unusual event is probably a chemical reaction involving tryptophan from food in the gut. We report a case with this unusual and interesting phenomenon in a 65 year old female.

Development and characterization of an infection inhibiting urinary catheter.

Catheter associated bacturia is common in hospitals and nursing homes. The objective of this study was to develop an infection inhibiting urinary catheter for prolonged use. Methods were established to add chlorhexidine digluconate (CHG) to a silicone elastomer and to compression mold the material to form a urinary catheter. CHG was randomly dispersed in the elastomer to be released through elution. Samples of the material, with CHG concentrations ranging from 1 to 4% by weight, were tested for in vitro release characteristics over a 28 day period and for in vivo toxicity over a 7 day period.

Urinary tract infections in the adult. The Postgraduate Medicine Lecture.

Diagnosis of urinary tract infection should be made only on the basis of complete evidence. Examination and culture of the urine are necessary steps in correct diagnosis and treatment. Urinary obstruction is almost always present in recurrent infections. Contributory factors include sex practices conducive to entry of bacteria, poor perineal hygiene, occupations that require long periods of driving or riding, chronic constipation, and use of oral contraceptives, tampons, and douches. Many antibacterial drugs are effective in eliminating infection. Contributing factors should be modified or eli

[Association of disseminated nocardiosis and Kaposi's sarcoma after kidney transplantation].

We report a 40-year-old kidney recipient who developed disseminated nocardiosis associated with cutaneous Kaposi's sarcoma. The withdrawal of immunosuppressive therapy and prolonged antibiotic therapy, including imipenem and trimethoprim-sulfamethoxazole, resulted in a favourable outcome of both disorders. Three years later, graft function remains stable with a complete regression of skin and pulmonary abnormalities. This case report illustrates the predisposing role of immunosuppressive treatment in the occurrence of infectious and neoplastic complications observed after solid-organ transplan

Purple urine bag syndrome: case series.

Purple urine bag syndrome is an uncommon condition that occurs in patients with long-standing indwelling urinary catheters, due to urinary tract infection (UTI) by bacteria producing sulfatase or phosphatase enzymes. This results in the formation of the pigments indigo (blue) and indirubin (red), together providing a purple appearance to urine. Management includes a change of catheter and appropriate antibiotics. Although prognosis is usually favourable, rare incidences of complications like Fournier's gangrene have been reported. We report three patients who developed purple urine bag syndrome

Urinary Tract Infection and Neurogenic Bladder.

Urinary tract infections (UTIs) are frequent, recurrent, and lifelong for patients with neurogenic bladder and present challenges in diagnosis and treatment. Patients often present without classic symptoms of UTI but with abdominal or back pain, increased spasticity, and urinary incontinence. Failure to recognize and treat infections can quickly lead to life-threatening autonomic dysreflexia or sepsis, whereas overtreatment contributes to antibiotic resistance, thus limiting future treatment options. Multiple prevention methods are used but evidence-based practices are few. Prevention and trea
