Prospective, randomized, double-blind trial comparing teicoplanin and cefazolin as antibiotic prophylaxis in prosthetic vascular surgery.

To compare efficacy, tolerability, and cost of antibiotic prophylaxis with teicoplanin and cefazolin in clean prosthetic vascular surgery, a randomized, prospective, double-blind study was performed at the Vascular Surgery Unit of a tertiary-care university hospital. Two-hundred thirty-eight consecutive patients undergoing elective, clean, abdominal or lower-limb prosthetic vascular surgery were allocated to receive a single intravenous dose of teicoplanin (400 mg) or cefazolin (2 g) at the induction of anesthesia. Surgical-site infections occurred in 5.9% of teicoplanin recipients (4.2% wound…

Cefamandole and isoxazolyl penicillins in antibiotic prophylaxis of patients undergoing total hip or knee-joint arthroplasty.

In a prospective comparative clinical efficacy and safety study, 58 patients undergoing total hip or knee arthroplasty were randomly allocat … s of cloxacillin were measured after 16-30 min. Thus, cefamandole seems to be more recommendable as antibiotic prophylaxis in total hip and knee replacements. The CRP level decreased to below 60 mg/l in all 16 patients on the 6th postoperative day.

Dental Students' Knowledge and Opinion on Optimizing Antibiotic Prophylaxis Guidelines: A Cross-Sectional Survey in India.

BACKGROUND: Antibiotic prophylaxis (AP) prescription is recommended to aid the body's defense in clearing tissues of microbial pathogens in patients at high risk of adverse outcomes, bacteremia, or infection undergoing invasive oral and dental procedures. A variety of influences impact dentists' decisions regarding AP prescribing. There is a paucity of data on diverse prescribing patterns of antibiotics as prophylaxis among dental students and professionals. Despite accumulating antimicrobial resistance (AMR), knowledge gaps regarding optimal prophylaxis practices persist among dental students…

Intrapartum group B streptococcus antibiotic prophylaxis in beta-lactams allergic women and risk of neonatal infection.

OBJECTIVE: In women with Group B Streptococcus (GBS) vaginal colonization who report a beta-lactams allergy, the prevention of neonatal GBS early-onset disease (EOD) mainly relies on maternal antibiotic prophylaxis with lincosamides during labor. With approximately 30% of GBS being resistant to macrolides or lincosamides, this antibiotic prophylaxis may be inappropriate, leading to a risk of EOD. We aim to measure the proportion of inappropriate antibiotic prophylaxis despite the existence of an antibiotic susceptibility testing, and its possible impact on the newborn. METHODS: Single-center …

Does antibiotic prophylaxis have an effect on postoperative infection in temporomandibular joint surgery? - A systematic review.

There is ongoing debate about the necessity of antibiotic prophylaxis regarding temporomandibular joint (TMJ) surgery. With antibiotic resistance being a major concern and challenge, developing solid recommendations on antibiotic use is essential to prevent overuse, misuse, and to enhance patient safety. The aim of this systematic review (SR) was to evaluate the scientific evidence regarding the effectiveness of antibiotics in preventing postoperative infections following TMJ surgery. Comprehensive literature searches were conducted using The Cochrane Library, Medline, EMBASE, and Web of Scien…

Antibiotic Prophylaxis in Men Undergoing Transurethral Resection of Prostate: A Systematic Review and Network Meta-Analysis.

OBJECTIVES: To analyze the effects of antibiotic prophylaxis (ATBp) prior to transurethral resection of the prostate (TURP) and rank the antibiotic classes. METHODS: We searched PubMed, Web of Science, Embase, CENTRAL, and clinical trials registries for randomized controlled trials (RCTs) from their inceptions through October 2024. We included all RCTs that compared ATBp to placebo or no agent (Plac/No agt) and to each other. Risk ratios (RR), mean differences (MD) and 95% confidence intervals (CI) were estimated by a frequentist network meta-analysis. Treatments were ranked by the surface un…

Cefazolin Alone Versus Cefazolin With Tobramycin or Gentamicin as Intraoperative Antibiotic Prophylaxis for Total Joint Arthroplasty.

BACKGROUND: Prosthetic joint infection is a serious complication of total knee arthroplasty (TKA) and total hip arthroplasty (THA). While cefazolin is the standard prophylactic antibiotic, some groups have considered adding aminoglycosides (gentamycin or tobramycin) to this regimen. METHODS: Adult primary THA and TKA patients were identified from 2010-2021 Q1 PearlDiver M151 database. Inclusion criteria were activity in the database ≥ 90 days postoperative and no infectious, neoplastic, or traumatic diagnoses within 90 days preoperative. Intraoperative antibiotic prophylaxis regimens were det…

Inappropriate antibiotic prophylaxis before dental procedures in congenital heart disease: Insights from a nationwide Korean cohort.

BACKGROUND: The 2007 American Heart Association (AHA) guidelines strictly restrict antibiotic prophylaxis (AP) before dental procedures to patients at the highest risk of infective endocarditis (IE). However, national adherence to these guidelines in Korea, a country with high outpatient antibiotic use, remains unevaluated. METHODS: This population-based, retrospective cohort study using National Health Insurance Service System claims (2002-2022) included patients with atrial septal defect, ventricular septal defect, tetralogy of Fallot, or prosthetic cardiac valves. Dental claims within 14 d…

Duration of surgical antibiotic prophylaxis and surgical site infection in orthopaedic surgery: a prospective cohort study.

BACKGROUND: The optimal duration for surgical antibiotic prophylaxis (SAP) for preventing surgical site infection (SSI) in orthopaedic surgeries remains poorly supported by high-level evidence. This study aimed to assess the association between SAP duration and the occurrence of SSI within one year postoperatively. METHODS: This prospective cohort study was based on the database from Surgical Site Infection Surveillance and Improvement Project (SISIP) of a tertiary orthopaedic university hospital from October 2014 to December 2020. The main outcome was SSI, defined according to the CDC/NHSN c…

Antibiotic prophylaxis in alloplastic temporomandibular joint replacement surgery: A multi-national survey of temporomandibular joint surgeons.

Temporomandibular disorders include a wide spectrum of extra-articular and intra-articular conditions affecting the temporomandibular joint (TMJ). In salvage cases involving intra-articular end-stage disease, alloplastic temporomandibular joint replacement (TMJR) is a management option which can be utilised to rehabilitate a disabled joint's function and form. Whilst post-TMJR infection is rare, it is one of the most serious complications. The principles governing TMJR surgery antibiotic prophylaxis have been based on those established in orthopaedic surgery literature. Antibiotic resistance d…

Addition of vancomycin to cefazolin is often unnecessary for preoperative antibiotic prophylaxis during total joint arthroplasties.

PURPOSE: The gold standard to decrease total joint arthroplasty (TJA) periprosthetic joint infection (PJI) is preoperative antibiotic prophylaxis. Despite substantial prevention efforts, rates of PJIs are increasing. While cefazolin is the drug of choice for preoperative prophylaxis, adjunctive vancomycin therapy has been used in methicillin-resistant Staphylococcus aureus (MRSA) endemic areas. However, studies examining these combinations are lacking. Therefore, we sought to examine complications among vancomycin plus cefazolin and cefazolin-only recipients prior to primary TJA in a single in…

Long-term antibiotic prophylaxis for prevention of rheumatic fever recurrence and progression to rheumatic heart disease.

BACKGROUND: Rheumatic fever is a non-suppurative, inflammatory sequela of group A Streptococcus pharyngitis that can occur at two to four weeks after infection. Following an episode of rheumatic fever, there is a risk of developing rheumatic heart disease (RHD) later in life that carries significant risk of morbidity and mortality. RHD remains the largest global cause of cardiovascular disease in the young (age < 25 years). The historical literature provides inconclusive evidence that antibiotic prophylaxis is beneficial in reducing the risk of recurrence of rheumatic fever and development of …
