We report the long-term effects on practice patterns at a single institution before (1995-2004) and after (2009-2018) publication of the results of the Infant Aphakia Treatment Study (IATS). The medical records of children <7 months of age who underwent cataract surgery at the Medical University of South Carolina were reviewed. The authors have designed an anterior-chamber intraocular lens (IOL) with flexible supporting elements and examined the possibility of its application for secondary correction of aphakia. Secondary implantations of such IOLs have been performed in 25 eyes. The implantation has proved to be low-traumatic and effective: the vision acuity has been unchanged or improved in 92% of cases, the loss of endothelial cells has made up 8.5% when the Healon bioprotector has been used and 10.3% without it. Criteria have been developed for the selection of patients for secondary implantation of IOL.

Twenty-one patients, aged 4-42 years, with unilateral aphakia after perforating injury, were corrected with a combination of spectacles and contact lenses. While taking stereopsis and aniseikonia into consideration, each patient was corrected with the combination of contact lens and spectacles to achieve the best stereoscopic vision. The synthesis of crystallins was studied by means of indirect immunofluorescence in 10-18 days old embryos and newborn ak/ak mice. Immune gamma-globulins to alpha-, beta- and gamma-crystallins of adult mice were used. A small amount of alpha-crystallins was found in an abnormal lens vesicle of 14 days old ak/ak embryo. The initiation of synthesis of these proteins is, hence, delayed by 4 days, as compared with that in the normal mice. The visual results, stereopsis, and complications have been assessed on 50 patients in a presenile age group treated for uniocular aphakia. Twenty-five patients received a Binkhorst four-loop lens implant at the time of cataract surgery and 25 were offered a conventional hard contact lens trial postoperatively.
