Impact of surgical strategy on functional outcome and recurrence in spinal arachnoid cysts.

BACKGROUND: Spinal arachnoid cysts (SACs) are rare benign lesions that account for 1-3% of spinal space-occupying lesions. This study evaluated clinical/radiological characteristics, surgical outcomes, and predictors of functional recovery, recurrence, and complications in SAC surgery. MATERIALS AND METHODS: We retrospectively reviewed 41 patients who underwent microsurgical treatment for histologically confirmed SACs (January 2013-May 2023), analysing demographics, clinical presentation, imaging, surgical technique, functional outcomes, postoperative complications and recurrence. RESULTS: T

Arachnoid granulation morphologies and β-amyloid and tau pathology in older adults.

INTRODUCTION: Arachnoid granulations (AG) prominently enlarge with age, yet limited studies evaluate AG in neurodegeneration. Here, we investigate the associations of AG morphologies with Alzheimer's disease (AD). METHODS: Macroscopic AG properties were evaluated along dorsal post mortem brain specimens (n = 882). Regression models were used to analyze their associations with AD neuropathology and dementia, adjusting for demographics. RESULTS: Participants died at mean age of 90.62 (SD = 6.98) years. β-amyloid (odds ratio [OR], 0.79 [95% confidence interval {CI}, 0.66-0.95]) and neurofibrill

Arachnoid Web With Atypical Imaging Findings and Clinical Manifestations: A Case Report.

Spinal arachnoid web (SAW) is a rare intradural lesion that can result in spinal cord compression and myelopathy. This report describes a case of SAW with atypical imaging findings and clinical manifestations in a 63-year-old male patient who presented with progressive lower limb paresthesia and left-dominant muscle weakness. The patient also exhibited temperature and pain sensory disturbances in the right leg. Magnetic resonance imaging (MRI) revealed spinal cord atrophy, intramedullary hyperintensity, and a dorsal flow void without the scalpel sign at the T7 level. Laminectomy and surgical r

Correlation of collagen damage and failure mechanics in porcine pia-arachnoid complex.

Mild traumatic brain injury (mTBI) accounts for approximately 75% of all TBI cases, and the mechanisms are still poorly understood, in part due to limitations of current diagnostic tools. Yet, there is a critical need to detect the presence of mTBI to mitigate risk of further injury. In this study, we explore the potential of collagen hybridizing peptides (CHPs), which selectively bind to damaged collagen, to detect damage in the pia-arachnoid complex (PAC), a major load-transferring interface during head trauma. To generate damage, porcine PAC samples underwent peel tests. Peak force to failu

Acute ischemic stroke caused by compression of the artery of Percheron by arachnoid cyst.

Ischemic stroke is a rare presentation for intracranial mass lesions in children. It has been described in association with intrasellar tumours, especially with pituitary adenomas and less frequently with craniopharyngiomas. Vascular events have been exceptionally described in association with arachnoid cysts, with only three cases reported. We present here a unique case of a paediatric intraventricular arachnoid cyst that spread both intraventricularly (right trigone) and intracisternally (quadrigeminal plate) and presented as an acute ischemic stroke, secondary to compression of the artery o

Cognition, FDG metabolism, and amyloid accumulation in relation to intracranial arachnoid cysts.

BACKGROUND: Though traditionally regarded as harmless, incidental findings, recent literature indicates neuropsychological symptoms associated with intracranial arachnoid cysts (AC). Pathogenesis is unknown, but compression of parenchyma and altered metabolism has been suggested. Patients suspected of dementia often undergo evaluation which can lead to identification of an AC. It is uncertain whether AC can be a primary or contributing cause to symptoms in these and AC have sometimes been raised as a differential diagnosis. METHODS: In this cross-sectional study, patients with AC ≥ 2 cm in a

Massive traumatic epidural hematoma in a child with suprasellar arachnoid cyst and hydrocephalus.

We present a case of a child with a suprasellar arachnoid cyst and hydrocephalus who developed a massive traumatic epidural hematoma following a fall. This represents the first reported case of such a condition. The case is characterized by a progressive increase in hemorrhage leading to a massive hematoma, yet with relatively mild clinical symptoms. The hemorrhage originated from extensive blood seepage from the dura mater. Intraoperative hemostasis was challenging, and there was a large residual cavity of the epidural hematoma without repositioning of brain tissue after removal of the hemato

Atypical presentation of arachnoid web causing paraparesis with absence of sensory symptoms.

We present a man in his 60s with a dorsal thoracic arachnoid web spanning levels T6-T8. The patient presented with gait abnormalities, severe neuropathic lower back pain and mild urinary incontinence without sensory deficits. He underwent laminectomy with arachnoid web fenestration. At the 6-week postoperative follow-up appointment, he had increased muscle strength in his lower extremities and was able to walk without shuffling his feet, with a straight back and standing upright. This is a marked improvement from his previous hunched and shuffling gait. He has had partial resolution of his neu

Our approach to the treatment of vestibular schwannomas with arachnoid dissection of the facial nerve.

UNLABELLED: Preserving the function of the facial nerve is extremely important in surgery for vestibular schwannomas. Two methods of arachnoid dissection are described for resection of vestibular schwannoma via retrosigmoid approach (from the brain stem and internal auditory canal). OBJECTIVE: To evaluate the results of arachnoid dissection of the facial nerve from internal auditory canal when resecting the vestibular schwannoma. MATERIAL AND METHODS: We analyzed 61 patients with vestibular schwannomas. Patients were divided into 2 groups depending on surgical technique. We estimated facial

Arachnoid webs with spinal cord compression: insights from three cases.

Spinal arachnoid webs are intradural bands of abnormally formed arachnoid tissue, located within the subarachnoid space and causing compression of the dorsal aspect of the spinal cord. Arachnoid webs are uncommon and can be difficult to treat. We report 3 patients presenting with a spinal arachnoid web within a 6-month period. All of them exhibited signs of thoracic myelopathy and the MRI showed the pathognomonic 'scalpel sign'. Two of the patients underwent surgery for removal of their spinal arachnoid web, whereas the third patient case is currently being managed conservatively. We also pres

Giant Arachnoid Granulations: Diagnostic Workup and Characterization in Three Symptomatic Adults.

Giant arachnoid granulations (GAGs) are poorly investigated. Here, we document clinical findings associated with five new GAGs and illustrate the anatomical composition of these structures as well as diagnostic considerations in three symptomatic adults. The GAGs ranged from 1.1 to 3.6 cm (mean, 2.2 cm) in maximum dimension and manifested in middle-aged individuals who presented with long-standing brain mass and/or chronic headache. On imaging examinations, the tissues appeared as irregular parasagittal and/or perisinus structures that demonstrated heterogeneous internal elements. The GAGs abu

Hemianopia: A complication of epidural injection in a patient with arachnoid cyst - case report.

Epidural injections are routinely used for short-term management of radicular pain and chronic low back pain. Prescription of this intervention, in the presence of intracranial abnormalities, is a topic of debate. Intracranial arachnoid cysts are cerebrospinal fluid-filled spaces, which are usually asymptomatic despite being a formidable size. As far as the authors know, there have been no cases depicted in indexed literature regarding asymptomatic supratentorial arachnoid cysts becoming symptomatic post undertaking of spinal epidural injections. We depict this phenomenon in a 53-year-old woma

Extended endoscopic endonasal approach to the chiasmatic cistern: An anatomical study of the arachnoid.

This paper studied the arachnoid of the chiasmatic cistern (CC) and the methods for increasing the exposure of the CC from an endoscopic perspective. Eight anatomical specimens with vascular injection were used for endoscopic endonasal dissection. The anatomical characteristics of the CC were studied and documented, and anatomical measurements were collected. The CC is an unpaired five-walled arachnoid cistern located between the optic nerve, optic chiasm, and the diaphragma sellae. The average exposed area of the CC before the anterior intercavernous sinus (AICS) was transected was 66.67 ± 33
