The diagnostic utility of matrix metalloproteinase-3 and high-sensitivity C-reactive protein for predicting rheumatoid arthritis in anti-cyclic citrullinated peptide antibody-negative patients with recent-onset undifferentiated arthritis.

Anti-cyclic citrullinated peptide (anti-CCP) antibodies are well-established serological markers that show high sensitivity and specificity in early rheumatoid arthritis (RA) and are associated with bone erosions of RA. However, some patients subsequently progress to RA even if there is no presence of anti-CCP antibodies in an early stage. The aim of this study is to evaluate the diagnostic utility of matrix metalloproteinase-3 (MMP-3), high-sensitivity C-reactive protein (hsCRP) and IgM rheumatoid factor for predicting RA in anti-CCP-negative patients with recent-onset undifferentiated arthri

The Risk of Hepatitis B Virus Reactivation in Rheumatoid Arthritis Patients Receiving Tocilizumab: A Systematic Review and Meta-Analysis.

BACKGROUND: Tocilizumab has demonstrated optimal efficacy and safety in patients with rheumatoid arthritis (RA) from clinical trials. However, the risk of hepatitis B virus reactivation (HBVr) in these patients remains uncertain because patients with underlying HBV have been excluded in phase III studies. METHODS: Systematical reviews were conducted on PubMed, Embase, and the Cochrane Central Register of Controlled Trials up to 21 February 2023. Random-effects meta-analysis was performed to calculate the pooled incidence of HBV reactivation. RESULTS: We included 0 clinical trials and 11 obse

Immune responses to Mycobacterial heat shock protein 70 accompany self-reactivity to human BiP in rheumatoid arthritis.

Rheumatoid arthritis (RA) is an autoimmune disease, and a member of human heat shock protein (HSP) 70 protein family, Binding Immunoglobulin Protein (BiP), has been identified as an important autoantigen for T and B cells. We herein focused on Mycobacterial (Myc) HSPs and immune responses to MycHSPs in RA patients. Serum titers of antibodies against MycHSP70 were significantly elevated in RA patients and correlated with serum anti-BiP antibody titers. A MycHSP70-derived HLA-DR4 major epitope was identified using the proliferative capacity of RA PBMCs as an indicator. The major epitope, MycHSP7

B-cell epitope spreading and inflammation in a mouse model of arthritis is associated with a deficiency in reactive oxygen species production.

Autoantibody-mediated inflammation contributes to the development of rheumatoid arthritis (RA), and anti-type II collagen (CII) antibodies are present in the serum, synovial fluid, and cartilage of RA patients. We had previously generated and characterized knock-in mice expressing a germline-encoded, CII-specific IgH (B10Q.ACB), which demonstrated positive selection of self-reactive B cells. Here, we show that despite the spontaneous production of CII-specific autoantibodies, B10Q.ACB mice are protected from collagen-induced arthritis. Introducing a mutation in the Ncf1 gene, leading to ROS de

Evidence that LPS-reactive arthritis in rats depends on the glial activity and the fractalkine-TNF-α signaling in the spinal cord.

It is known that primary afferent central terminal sensitization can influence peripheral inflammation, however, it remains to be understood whether spinal cord glia can also contribute to this process. Our aim was to investigate the effect of spinal cord glia inhibition on the pathogenesis of LPS-induced knee-joint monoarthritis in rats and also to investigate the role of fractalkine and TNF-α. LPS was injected into the knee-joint previously primed with carrageenan to cause articular incapacitation, edema, synovial leukocyte infiltration, and GFAP and CD11b/c spinal immunoreactivity (glia-IR)

Association of treatment-resistant chronic Lyme arthritis with HLA-DR4 and antibody reactivity to OspA and OspB of Borrelia burgdorferi.

Chronic Lyme arthritis that is unresponsive to antibiotic therapy is associated with an increased frequency of the HLA-DR4 specificity. To determine whether the immune response to a particular polypeptide of Borrelia burgdorferi may be associated with treatment-resistant chronic Lyme arthritis, we correlated the clinical courses and HLA-DR specificities of 128 patients with Lyme disease with their antibody responses to spirochetal polypeptides. Antibody reactivity was determined by Western blotting (immunoblotting) with sonicated whole B. burgdorferi and recombinant forms of its outer surface

Predictive value of the C-reactive protein to albumin ratio in the treatment of septic arthritis in young children: a retrospective study.

INTRODUCTION: Septic arthritis (SA) can cause lifelong disability in children due to joint dysfunction but there is controversy regarding the timing of surgery in SA. The C-reactive protein to albumin ratio (CAR) has emerged as a novel marker of inflammation and has been extensively used in predicting inflammatory bowel disease, arthritis, and systemic inflammation. Despite advancements, few studies have evaluated the role of CAR in SA. Therefore, the present study was aimed to investigate whether CAR could serve as predictive indicators for determining whether patients under four years old wi

Pneumonia and Meningoencephalitis Due to Varicella-zoster Virus Reinfection and Epstein-Barr Virus Reactivation in a Patient with Rheumatoid Arthritis.

A 72-year-old woman with rheumatoid arthritis was treated with methotrexate (MTX) and iguratimod. Upon examination of a liver tumor, blisters due to varicella-zoster virus (VZV) infection were observed. Despite oral administration of valacyclovir, she developed varicella pneumonia and meningoencephalitis. A VZV antibody test revealed reinfection. The liver tumor shrank after discontinuance of MTX, and polymerase chain reaction revealed the reactivation of the Epstein-Barr virus (EBV). Therefore, we were unable to deny MTX-associated lymphoproliferative disorder (MTX-LPD). This is the first cas

Salivary and serum procalcitonin and C-reactive protein as biomarkers of periodontitis in United States veterans with osteoarthritis or rheumatoid arthritis.

Serum procalcitonin (ProCT) is elevated in response to bacterial infections, whereas high sensitivity C-reactive protein (hsCRP) is a nonspecific inflammatory marker that is increased by excess adipose tissue. We examined the efficacy of ProCT and hsCRP as biomarkers of periodontitis in the saliva and serum of patients with arthritis, which is characterized by variable levels of systemic inflammation that potentially can confound the interpretation of inflammatory biomarkers. Blood and unstimulated whole saliva were collected from 33 patients with rheumatoid arthritis (RA) and 50 with osteoart

Comparative assessment of the C-reactive protein-to-albumin ratio, platelet-to-albumin ratio, and prognostic nutritional index in rheumatoid arthritis.

Rheumatoid arthritis (RA) is a chronic autoimmune disease that requires regular disease activity assessment to guide treatment and improve outcomes. Composite inflammatory and nutritional biomarkers are promising adjunctive tools for monitoring disease activity. This comparative study aimed to evaluate the levels of C-reactive protein-to-albumin ratio (CAR), platelet-to-albumin ratio (PAR), and prognostic nutritional index (PNI) in RA patients and healthy controls, and to compare their diagnostic performance and utility for disease activity assessment. The study included 165 RA patients and 16

When the Infection Clears but Inflammation Persists: A Case of Chlamydia-Associated Reactive Arthritis Requiring Disease-Modifying Antirheumatic Drug (DMARD) Therapy.

Reactive arthritis (ReA) can be a persistent and debilitating form of arthritis that is a consequence of a gastrointestinal or genitourinary infection. Although most cases self-resolve, a small subset of cases may persist and require an escalation of care to manage their symptoms. We present a case of recurrent ReA induced by Chlamydia trachomatis  infection in a young male patient who required a stepwise escalation of his medical management. This case outlines a 32-year-old man followed over six months from his diagnosis. Initially, the patient presented with persistent joint pain in the lowe

Serum calprotectin and complement factor C3 are superior biomarkers of inflammation in early psoriatic arthritis as compared with C-reactive protein.

OBJECTIVES: C reactive protein (CRP) is frequently normal in psoriatic arthritis (PsA) despite active disease, complicating inflammation assessment. This study aimed to evaluate alternative biomarkers of inflammation in early PsA. METHODS: Adult patients with early, treatment-naïve PsA were enrolled in the prospective multicentre cohort and compared with early rheumatoid arthritis (RA) and healthy controls (HCs). Clinical assessments, inflammatory markers and peripheral blood counts were collected. For this study, baseline and 1-year data were used. Serum complement factor 3 (C3), calprotecti

Hypotonic stress promotes ATP release, reactive oxygen species production and cell proliferation via TRPV4 activation in rheumatoid arthritis rat synovial fibroblasts.

Rheumatoid arthritis (RA) is a chronic and systemic autoimmune-disease with complex and unclear etiology. Hypotonicity of synovial fluid is a typical characteristic of RA, which may play pivotal roles in RA pathogenesis. In this work, we studied the responses of RA synovial fibroblasts to hypotonic stress in vitro and further explored the underlying mechanisms. Data showed that hyposmotic solutions significantly triggered increases in cytosolic calcium concentration ([Ca2+]c) of synoviocytes. Subsequently, it caused rapid release of ATP, as well as remarkable production of intracellular reacti

[Rheumatoid factors, C-reactive protein and circulating immunocomplexes: laser-nephelometric determination using the latex aggregation method. Preliminary evaluation in rheumatoid arthritis].

C-reactive protein and rheumatoid factor assays are relevant laboratory parameters, among the most used in the clinical practice, being very useful in diagnosing and "monitoring" rheumatic diseases. A quantitative test to detect these two serum proteins is an important, more specific, new tool for clinical medicine. In our study we evaluated two new laser nephelometric assays, both quantitative, based on the agglutination of polystyrene latex particles, able to show serum concentration of CRP and RF. With a similar technique we tested the presence of circulating immune complexes. We performed

Association Between C-Reactive Protein-Triglyceride Glucose Index, and the Risk of Osteoarthritis and Rheumatoid Arthritis: Results From NHANES 1999-2010.

The serum C-reactive protein-triglyceride glucose index (CTI) is a parameter integrating inflammation and insulin resistance, which are potential mechanisms for osteoarthritis (OA) or rheumatoid arthritis (RA). Our study aims to investigate its association with the risk of OA or RA. This study is based on cross-sectional data from the National Health and Nutrition Examination Survey (NHANES) 1999-2010, involving 6395 participants aged ≥ 20 years. Weighted logistic regression, restricted cubic splines (RCS), and subgroup analysis were employed to assess the relationship between CTI and OA, RA.
