On the nature of mental disorder: towards an objectivist account.

According to the predominant view within contemporary philosophy of psychiatry, mental disorders involve essentially personal and societal values, and thus, the concept of mental disorder cannot, even in principle, be elucidated in a thoroughly objective manner. Several arguments have been adduced in support of this impossibility thesis. My critical examination of two master arguments advanced to this effect by Derek Bolton and Jerome Wakefield, respectively, raises serious doubts about their soundness. Furthermore, I articulate an alternative, thoroughly objective, though in part normative, f

Trauma-Focused Psychodynamic Psychotherapy of a Patient With PTSD in a Veterans Affairs Setting.

OBJECTIVE:: This article aims to articulate the use of trauma-focused psychodynamic psychotherapy (TFPP) for a 33-year-old U.S. Army veteran with posttraumatic stress disorder (PTSD) in a Veterans Affairs (VA) setting. METHODS:: The patient was treated with TFPP, a manualized brief psychotherapy provided as part of a pilot study. TFPP differs from traditional dynamic psychotherapies in its focus on symptoms of trauma and associated dynamics. The patient was seen for an initial 60-minute intake session and then for 16 50-minute sessions over 5.5 months at a VA medical center. RESULTS:: Follow

Progressive loss of speech: a neuropsychological profile of premotor dysfunction.

Several patients with 'progressive loss of speech output' or 'progressive anarthria' of degenerative origin have been reported in the literature. We report 5 clinical cases with slowly progressive loss of speech output and initially no deficit in other cognitive domains. The early clinical features were analysed in an attempt to identify the anatomo-functional systems implied in the degenerative process. The first phase of the disorder was characterised by impaired articulation consistent with speech apraxia, telegraphic style and a difficulty to elaborate a series of orofacial or hand movemen

Phonological and articulation treatment approaches in Portuguese children with speech and language impairments: a randomized controlled intervention study.

BACKGROUND: In Portugal, the routine clinical practice of speech and language therapists (SLTs) in treating children with all types of speech sound disorder (SSD) continues to be articulation therapy (AT). There is limited use of phonological therapy (PT) or phonological awareness training in Portugal. Additionally, at an international level there is a focus on collecting information on and differentiating between the effectiveness of PT and AT for children with different types of phonologically based SSD, as well as on the role of phonological awareness in remediating SSD. It is important to

Speech articulation performance of francophone children in the early school years: norming of the Test de Dépistage Francophone de Phonologie.

Good quality normative data are essential for clinical practice in speech-language pathology but are largely lacking for French-speaking children. We investigated speech production accuracy by French-speaking children attending kindergarten (maternelle) and first grade (première année). The study aimed to provide normative data for a new screening test - the Test de Dépistage Francophone de Phonologie. Sixty-one children named 30 pictures depicting words selected to be representative of the distribution of phonemes, syllable shapes and word lengths characteristic of Québec French. Percent cons

The nance obturator, a new fixed obturator for patients with cleft palate and fistula.

In patients with fistulas that impair function (e.g., feeding, resonance, intelligibility), obturators are used to improve feeding and reduce nasal air emission by occluding the abnormal opening between the oral and nasal cavities. This report describes a novel method for occluding an anterior palatal fistula in patients with cleft palates. The new design for a fixed obturator is based on the Nance appliance, which was originally used as a space maintainer, but has been redesigned for closing an anterior palatal fistula in a patient with cleft lip and palate. The Nance obturator may be used wh

Developmental dyslexia: exploring how much phonological and visual attention span disorders are linked to simultaneous auditory processing deficits.

The simultaneous auditory processing skills of 17 dyslexic children and 17 skilled readers were measured using a dichotic listening task. Results showed that the dyslexic children exhibited difficulties reporting syllabic material when presented simultaneously. As a measure of simultaneous visual processing, visual attention span skills were assessed in the dyslexic children. We presented the dyslexic children with a phonological short-term memory task and a phonemic awareness task to quantify their phonological skills. Visual attention spans correlated positively with individual scores obtain

An accessory tongue in a child: A clinical case.

A 20-month-old male patient presented with a tumor on his tongue. The tumor was detected at birth with an initial size of 5 mm; thereafter, it gradually grew larger. The patient had no symptoms, but he frequently touched the tumor, as he grew older. The authors performed a tumor resection under general anesthesia, and the histopathological diagnosis confirmed that it was an accessory tongue, with the same components as that of a normal tongue. An accessory tongue regularly grows bigger as a patient grows older, resulting in dysphagia and articulation disorder. Younger patients should get acces

3-Tesla magnetic resonance imaging evaluation of posterior tibial tendon dysfunction with relevance to clinical staging.

The posterior tibial tendon (PTT) is the most important dynamic stabilizer of the medial ankle and longitudinal arch of the foot. PTT dysfunction is a degenerative disorder of the tendon, which secondarily involves multiple ligaments, joint capsules, fascia, articulations, and bony structures of the ankle, hindfoot, midfoot, and forefoot. When the tendon progressively attenuates, the patient develops a painful, progressive collapsed flatfoot or pes planovalgus deformity. This comprehensive review illustrates the 3-Tesla magnetic resonance imaging (3T MRI) features of PTT dysfunction. In additi

Type IV Ehlers-Danlos syndrome with isolated arterial involvement.

Ehlers-Danlos syndrome is a connective tissue disorder characterized by skin hyperelasticity and friability, hyperextensible small articulations, vascular fragility, and ocular and visceral symptoms. Ten types of Ehlers-Danlos syndrome have been described to date. Type IV is characterized by isolated arterial involvement with the risk of rupture, making diagnosis difficult. A 25-year-old man was operated on for a ruptured left and then right iliac artery and once again for a hepatic artery aneurysm within a nine month period. The diagnosis of Ehlers-Danlos syndrome was based on clinical and hi

[Progressive multifocal leukoencephalopathy. Late complication in chronic lymphatic leukemia].

In a 44 year old patient with chronic lymphatic leukemia and secondary antibody deficiency syndrome, a disorder of articulation and a left hemiparesis developed during a thrombocytopenic phase. Computer tomography of the cranium, CSF diagnostics and the electroencephalogram did not provide any indication for the cause of the rapidly progressive cerebral symptoms. In the NMR tomogram, a diffusedly increasing intensity in the T2-weighted tomograms were shown around the central region in the right brain. The patient died of a Pseudomonas septicemia. At autopsy, a typical finding of progressive mu

[Phoniatric and laryngological aspects of systemic scleroderma (author's transl)].

Organs which contain collagen can have connective tissue new growth in systemic scleroderma. The occurrence of scleroderma of the larynx and organs of speech is rare. The disease develops in the mucous membrane in three stages: oedema; infiltration and induration; and atrophy. The oedema stage is generally the shortest. Three patients with systemic scleroderma with involvement of larynx and organs of speech have been seen. In one is described (graphically, sonographically and with stroboscopy) the progress of the disease from an initial severe hoarseness to later resolution of the oedema. In a

How to meet the neighbors: Modality effects on phonological generalization.

Long-term auditory priming of words from dense neighborhoods has been posited as a learning mechanism that affects change in the phonological structure of children's lexical representations. An apparent confound associated with the modality of priming responsible for structural change has been introduced in the literature, which challenges this proposal. Thus, our purpose was to evaluate prime modality in the treatment of children with phonological delay. Nine children were assigned to auditory-visual, auditory, or visual priming of words from dense neighborhoods prior to the treatment of prod

Division of tongue tie: review of practice through a tertiary paediatric otorhinolaryngology service.

OBJECTIVES: Recent NICE guidance declared that evidence regarding surgical division of tongue tie was adequate to support the use of the procedure, provided that normal arrangements are in place for consent, audit and clinical governance. This work aimed to carry out a retrospective review of those patients who have previously undergone tongue tie division through a tertiary paediatric otorhinolaryngology service. We further aimed to identify the referral patterns, indications for, and outcomes following, division. METHODS: A retrospective chart review of all patients attending for division o

Speech therapy for errors secondary to cleft palate and velopharyngeal dysfunction.

Individuals with a history of cleft lip/palate or velopharyngeal dysfunction may demonstrate any combination of speech sound errors, hypernasality, and nasal emission. Speech sound distortion can also occur due to other structural anomalies, including malocclusion. Whenever there are structural anomalies, speech can be affected by obligatory distortions or compensatory errors. Obligatory distortions (including hypernasality due to velopharyngeal insufficiency) are caused by abnormal structure and not by abnormal function. Therefore, surgery or other forms of physical management are needed for
