Identification of alpha1-adrenoceptor subtypes involved in contraction of young CD rat epididymal vas deferens.

Contraction of rat epididymal vas deferens is regulated via a release of neurotransmitters from autonomic nerves and is mediated by alpha(1)-adrenoceptors. This study was directed to the characterization of alpha(1)-adrenoceptors involved in the contraction of the epididymal portion of young CD rat vas deferens, that were selectively discriminated in two populations through the irreversible blockade of two beta-chloroethyamines, 1 and 2. The antagonist activity of known subtype-selective alpha(1)-adrenoceptor antagonists, WB4101, 5-MU, and RS17053 (alpha(1A)), (+)-cyclazosin (alpha(1B)), and B

Nociceptin modulates renal sympathetic nerve activity through a central action in conscious rats.

Nociceptin, an endogenous agonist of the opioid receptor-like(1) receptor, is expressed in the hypothalamus, where it is implicated in autonomic nervous system control. However, the central actions of nociceptin on sympathetic nerve activity have not been studied. We investigated the effect of intracerebroventricularly administered nociceptin (2-10 nmol) on blood pressure, heart rate (HR), and renal sympathetic nerve activity (RSNA) in conscious rats and sinoaortic-denervated (SAD) rats. Intracerebroventricularly administered nociceptin resulted in a dose-dependent decrease in mean arterial pr

3,4-diaminopyridine for the treatment of Lambert-Eaton myasthenic syndrome.

The Lambert-Eaton myasthenic syndrome (LEMS) is an autoimmune disease in which antibodies against voltage-gated calcium channels inhibit cholinergic neurotransmission. LEMS is clinically characterized by muscle weakness and autonomic dysfunction. 3,4-diaminopyridine (3,4-DAP) blocks potassium channels in nerve terminals, resulting in an increase in acetylcholine release. This article describes the four randomized placebo-controlled trials of 3,4-DAP in patients with LEMS. All trials demonstrated a significant effect on muscle strength and compound muscle action potential amplitude. Furthermore

Autonomic degeneration and altered blood pressure control in humans.

The study of patients with partial or total defects of their sympathetic nerves can help clarify the role of the sympathetic nervous system in blood pressure control. Denervation supersensitivity of both beta and alpha receptors develops in humans and is proportional to the degree of sympathetic withdrawal. Although alterations in beta-receptor sensitivity are familiar responses to beta agonists or antagonists, patients with decreased norepinephrine (NE) levels appear to develop alpha-receptor supersensitivity of greater hemodynamic importance. When beta supersensitivity is marked, there may b

Autonomic pain: features and methods of assessment.

The distribution of pain originating in the sympathetic nervous system does not match the somatic segmental sensory distribution at the postganglionic level. The two types of distribution are separate and different. At the preganglionic level, fibers show typical segmental sensory distribution, which resembles but is not identical to somatic segmental sensory distribution. Instead, sympathetic pain has its own distribution along the vascular supply and some peripheral nerves. It cannot be called atypical in terms of somatic segmental sensory distribution. Several techniques are available to as

Sympathomimetic pressor responses to thyrotropin-releasing hormone in rats.

Cardiovascular responses to centrally administered thyrotropin-releasing hormone (TRH) were studied in urethan-anesthetized rats to allow continuous recording of attendant changes in sympathetic nerve activity. Intracerebroventricular infusions of TRH (0.05-5.0 micrograms) consistently increased not only blood pressure and heart rate, but also spike frequency in splanchnic, renal, or cervical sympathetic nerves. Parasympathetic inhibition seemed unlikely because TRH responses were unaltered by cholinergic blockade with atropine, and efferent vagal nerve firing, instead of being reduced, was ac

Efficacy and Safety of Celiac Plexus Block in Chronic Pancreatitis: A Multicenter Cohort Study.

OBJECTIVES: Celiac plexus block (CPB) is used for pain management in chronic pancreatitis (CP), but real-world evidence is limited. This study evaluated clinical outcomes, safety, and utilization trends of CPB in CP. METHODS: We conducted a retrospective cohort study using the TriNetX Research Network. Adults with CP were identified; those with pancreatic malignancy or prior pancreatic surgery were excluded. CP patients who underwent CPB were compared with those without CPB. Propensity score matching (1:1) balanced demographics and comorbidities. Primary outcomes were the prevalence of opioid

Successful management of cardiac electrical storm after aortic valve replacement using stellate ganglion block: a case report.

Cardiac electrical storm (CES) is a life-threatening state of electrical instability characterized by 3 or more episodes of sustained ventricular arrhythmia within 24 h. The application of stellate ganglion block for the treatment of cardiac surgery has rarely been reported. Herein, we report a patient whose ventricular arrhythmia was completely relieved after receiving a stellate ganglion block. We present a case of a 35-year-old male who underwent aortic valve replacement. The patient who had aortic valve stenosis was scheduled for aortic valve replacement and simultaneous tricuspid valvulop

Stellate ganglion block for preventing postoperative arrhythmias: a protocol for a systematic review and meta-analysis.

INTRODUCTION: Postoperative arrhythmias are common and clinically significant complications. They are a cause of increased morbidity and mortality rates in surgical patients. Although various pharmacological and procedural strategies have been explored for preventing postoperative arrhythmia, evidence regarding their effectiveness remains inconsistent. The stellate ganglion block (SGB) has emerged as a promising alternative to reduce the occurrence of postoperative arrhythmias. By summarising the existing evidence, this meta-analysis aims to assess the effectiveness of SGB in preventing postop

Clinical effect of perioperative stellate ganglion block on mechanical ventilation and respiratory function of elderly patients with septic shock.

BACKGROUND: Here we investigated the effect of a stellate ganglion block on the perioperative mechanical ventilation and postoperative recovery of respiratory function of elderly patients with infectious shock. METHODS: Thirty-six elderly patients with septic shock who underwent emergency general anesthesia at our hospital were randomly divided into treatment (T) and control (C) groups (n = 18 each). Group T received a preoperative stellate ganglion block, whereas group C received normal saline. Procalcitonin and C-reactive protein levels were compared preoperatively and at 1 and 7 days posto

Impact of Celiac Plexus Neurolysis on Survival in Patients with Unresectable Pancreatic Cancer: A Retrospective, Propensity Score Matching Analysis.

BACKGROUND: Pain caused by pancreatic cancer (PC) is difficult to control. Celiac plexus neurolysis (CPN) can effectively control the pain and reduce the use of opioids. However, the effect of CPN on survival for patients with unresectable PC remains controversial. OBJECTIVES: To determine if CPN is associated with survival benefits for these patients. STUDY DESIGN: Retrospective, observational cohort study. SETTING: National Cancer Center in Korea. METHODS: The CPN group included patients who were diagnosed with unresectable PC and underwent fluoroscopically guided bilateral CPN (10 mL de

A lateral paracarotid approach for ultrasound-guided stellate ganglion block with a linear probe.

Recent reports suggest that ultrasound-guided stellate ganglion block (SGB) is safer and more accurate than classic SGB by the using the surface anatomical landmark. However, previous reports concern the classic paratracheal approach using a small specialized curved probe, which may not be appropriate in some patients. The authors have attempted several approaches, including paratracheal, trans-thyroidal, lateral paracarotid, and lateral approaches, to find a safe and suitable method for real-time ultrasound-guided SGB using a standard high-frequency linear probe. A total of 27 injections were

Awake, sedated or anaesthetised for regional anaesthesia block placements?: A retrospective registry analysis of acute complications and patient satisfaction in adults.

BACKGROUND: Whether adults should be awake, sedated or anaesthetised during establishment of regional anaesthesia is still debated and there is little information on the relative safety of each. In paediatric practice, there is often little choice but to use sedation or anaesthesia as otherwise the procedures would be too distressing and patient movement would be hazardous. OBJECTIVE(S): The objective of this study was to evaluate complications related to central and peripheral regional block and patient satisfaction in awake, sedated and anaesthetised adult patients. DESIGN: A retrospective
