United States Military Parachute Injuries: Part 2: Interventions Reducing Military Parachute Injuries in Training and Operations.

This is part 2 of an article detailing the reduction in airbornerelated injuries over time. Part 1 examined the early history of airborne operations and provided evidence for the reduction in injuries over time; part 2 discusses interventions associated with the decline in injury rates. In 1943 at the United States (US) Army Airborne School, data showed that injuries were substantially reduced from 120 to 18 injuries/1000 trainees. Credit for the reduction was given to development of the parachute landing fall (PLF), better supervision of students while in initial airborne training, intensive

[Air travel during pregnancy].

Nowadays, millions of people travel by air every day and it is common to find pregnant women among the passengers. Travelling during pregnancy raises the question of the woman's safety during the flight. This is due to the risk of exposure to cosmic radiation, the drop in air pressure, and the possibility of thromboembolism due to seating conditions and flight duration. Other risks include obstetric emergencies, such as hemorrhage, premature contractions and actual labor. Further issues associated with air travel during pregnancy have to do with the safety of pregnant aircrew, if emergency eva

Surveillance of dengue vector at thiruvananthapuram (Kerala) International Airport.

A survey of Aedes (Stegomyia) mosquitoes was undertaken in and around the International airport at Thiruvananthapuram during premonsoon season. An attempt has been made in this paper to elaborate on the presence of risk potential for Dengue /yellow fever at the airport with the detection of Aedes breeding. The entomological indices have also been discussed with regard to presence of potential wet containers and rate of infestation by Aedes mosquitoes. The airport area was searched for Aedes (Stegomyia) breeding where 20% of the premises were found positive for Aedes aegypti breeding and one pr

Avianca flight No. 052 accident: a plastic surgical perspective.

The crash of Avianca Airlines flight no. 052 en route to JFK Airport on January 25, 1990, in Cove Neck, New York, resulted in the death of 72 passengers. Eighty-nine victims were admitted to 13 regional hospitals. Despite difficult access to the wooded crash site, early warning and prompt response by 37 volunteer fire and rescue units resulted in organized EMS triage and rapid hospital transport. This report reviews the specific injuries incurred, highlights the team management approach to a major aviation accident in a suburban area, and studies the likelihood of accidents of this magnitude. …

Incidence of airsickness among military parachutists.

This study describes the incidence of airsickness among military parachutists and analyzes the factors involved in its occurrence. Each of 45 healthy male subjects (28 students and 17 advanced parachutists) was studied. Each student participated in five parachute-jump exercises (one daily) and each advanced parachutist participated in one exercise only (proficiency). A questionnaire used for the diagnostic evaluation of motion sickness symptoms was completed by the subjects after each training exercise. A positive diagnosis of airsickness was established for 64% of the students on their first

Alcohol abuse: its implications in trauma care.

Alcohol abuse is the single most important aggravating factor in trauma and the severity of injuries sustained therefrom. The role of alcohol in fatal vehicular crashes is well recognized but is equally great or greater in falls, choking, drowning, burns, and assaults. Since the problem drinker is implicated in most serious highway crashes and since human behavior related to the use of alcohol tends to be constant across situations, persons who violate generally recognized norms of alcohol use in one context are likely to do so in others. The physician involved in the care of the acutely injur

Death and injury in aerial spraying: pre-crash, crash, and post-crash prevention strategies.

To prevent crash-related death and injury among spray pilots, a program including pre-crash, crash and post-crash stages of intervention for aircraft, physical environment, and pilots and ground crews was proposed in accordance with a matrix of options derived from road crash epidemiology. In addition to the dangers of fixed obstacles, low-altitude runs, and heavy work schedules, work hazards included combined exposures to noise, vibration, G forces, heat stress, pesticides, and dehydration. Together, these exposures were believed to have produced slight, but crucial decreases in pilot perform

[Aerosinusitis. Part 2: Diagnosis, therapy and recommencement of flight duties].

Aerosinusitis more frequently affects the frontal sinus than the maxillary sinus and mostly occurs during descent. Sinonasal diseases and anatomic variations leading to obstruction of paranasal sinus ventilation favor the development of aerosinusitis. This Continuing Medical Education (CME) article is based on selective literature searches of the PubMed database (search terms: "aerosinusitis", "barosinusitis", "barotrauma" AND "sinus", "barotrauma" AND "sinusitis", "sinusitis" AND "flying" OR "aviator"). Experimental studies of forensic odontology to aid in the identification process.

The importance of dental identification is on the increase year after year. With the passage of time, the role of forensic odontology has increased as very often teeth and dental restorations are the only means of identification. Forensic odontology has played a key role in identification of persons in mass disasters (aviation, earthquakes, Tsunamis), in crime investigations, in ethnic studies, and in identification of decomposed and disfigured bodies like that of drowned persons, fire victims, and victims of motor vehicle accidents. The various methods employed in forensic odontology include …

Neck injury after repeated flexions due to parachuting.

A 37-yr-old, previously healthy officer developed a severe pain in the right shoulder and parethesia in the right I-III fingers during a high-altitude low-opening parachute jump. The pain started after a sudden neck flexion while he was checking the developing canopy, well before landing. Electroneuromyography revealed damage to the right C7 nerve root. Cervical radiography showed severe spondylosis and discus degeneration in the CV-VII region; magnetic resonance image demonstrated posterior disk protrusions in this area. Military aviators flying high-performance aircraft often have similar pr

The photic sneeze reflex as a risk factor to combat pilots.

The tendency to sneeze upon exposure to bright light is autosomal dominant and affects 18-35% of the population. This uncontrolled sneezing may represent a danger to pilots during flight. Testing was conducted using Ditric narrow band (+/- 10 nm) interference filters. Wavelengths at 430, 532, and 560 nm were tested with a GE 500-W photoflood BCA #1 bulb at constant intensity. Subsequent testing of military and civilian aviation goggles and sunglasses was conducted using a Digikrom 240 monochromator. Repeated testing with interference filters in a clinical setting showed no effect on sneezing i…

Vestibular tests in the selection of cosmonauts.

Vestibulo-vegetative disorders in cosmonauts and astronauts occurring during space flight compel otolaryngologists to search for vestibular tests enabling a precise evaluation of the activity of the vestibular apparatus and showing disposition to motion sickness. Otoneurological investigation of Polish candidates for cosmonaut status consisted of the following vestibular tests: caloric, rotatory, optokinetic, swinging torsion, statokinesimetric and vestibulo-vegetative. The value of various vestibular tests for aviation and space medicine is presented in this paper, taking into account the res…

Blinded by the depths: A case of acute loss of vision in a scuba diver.

Decompression illness (DCI) is a well-known medical emergency among scuba divers, aviators, astronauts, and compressed-air workers. It includes two diseases: decompression sickness (DCS) and arterial gas embolism (AGE). Vision changes are one of the symptoms that can happen in patients with DCI, but acute complete loss of vision is extremely rare. We report a 28-year-old male patient, a novice scuba diver who presented to our hospital with complete loss of vision while he was ascending from 18 m depth. The diagnosis of AGE was made depending on the history, clinical examination, and radiologic

Increasing the margin of patient safety for periodontal and implant treatments: The role of human factors.

Early complications following periodontal and dental implant surgeries are typically attributed to technique or poor biological response, ignoring the possibility of the human element. Interestingly, significant experience is not correlated with increased success, whereas evidence supports the impact of clinical behavior on patient outcome. This is the result of errors, much like those scrutinized in other high-risk technical fields, such as aviation. What can be surprising is that those who make these errors are very well acquainted with best practices. Given this, how is it possible for the …
