In a consecutive series of 75 patients with unstable cervical spine injuries treated with halo-vest, 1976-1984, a total of 14 patients (comprising 18.7%) had odontoid fractures. Of the patients with odontoid fractures, 1 patient had an incomplete tetraparesis. All patients except 1 were treated within the first day after the trauma. A total of 50% were injured in motor vehicle accidents. The odontoid fracture was reduced by skull traction and then stabilized with a halo-vest for 12 weeks. One patient died within the first 2 weeks after the injury. There were no other serious complications duri

In higher vertebrates, the formation of the body axis proceeds in a craniocaudal direction during gastrulation. Cell biological evidence suggests that mesoderm formation and specification of axial positions occur simultaneously. Exposure of gastrulating embryos to retinoic acid induces changes in axial patterns, e.g. anterior and posterior homeotic transformations of vertebrae. These morphological changes are accompanied by changes in the nonidentical, overlapping expression domains of Hox genes. In this report the influence of retinoic acid, administered at the end of and after gastrulation,

PURPOSE: "Plough" fracture, in which the odontoid ploughs through and causes a high-energy shear fracture of the anterior arch of the atlas, has been documented in clinical case studies and classified as clinically unstable. Our objectives were to develop a biomechanical model to simulate atlantal plough fracture and investigate injury mechanisms.
METHODS: Horizontally aligned head impacts into a padded barrier were simulated using a human upper cervical spine specimen (occiput through C3) mounted to a surrogate torso mass on a sled and carrying a surrogate head. We divided 13 specimens into

The craniovertebral junction (CVJ) is the bony transition between the cranium and cervical spine. It is a biomechanically complex articulation comprising the occipital condyles (Oc) the atlas (C1) and axis (C2). Pathologies affecting the CVJ in children are myriad with clinical features resulting from biomechanical instability, deformity, or neuraxial compression. Establishing the natural history and clinical burden of a condition is challenging in infants and young children, often complicated by co-existing neuromuscular and cognitive impairment. This makes investigation and treatment plannin

BACKGROUND: Atlantoaxial instability, which can arise in the setting of trauma, degenerative diseases, and neoplasm, is often managed surgically with C1-C2 arthrodesis. Classical C1-C2 fusion techniques require placement of instrumentation in close proximity to the vertebral artery and C2 nerve root.
OBJECTIVE: To report a novel C1-C2 fusion technique that utilizes C2 translaminar screws and C1 sublaminar cables to decrease the risk of injury to the vertebral artery and C2 nerve root.
METHODS: To facilitate fixation to the atlas, while minimizing the risk of injury to the vertebral artery an

A variety of pathological conditions may affect the clivus and the craniovertebral junction (CVJ). These include congenital disorders, chronic inflammation, neoplasms, infections, and posttraumatic conditions that could all result in CVJ compression and myelopathy Endoscopic-assisted procedures have been further developed for CVJ decompression and they have now become conventional approaches. The aims of the present study were:(1) to compare "radiological" and "surgical" nasoaxial lines (NAxLs); (2) to introduce an analogous radiological line as a predictor of the superior extension of the tra

OBJECT: Most upper cervical spine injuries are able to heal conservatively by halo vest application. The acceptance of the halo is different among patients due to the weight of the apparatus and the limited mobility it causes. Additionally, the fracture healing rate in older patients seems to be inferior to that in younger patients, which would make an operative stabilization procedure more appropriate for the elderly. Furthermore, the risk of complications is assumed to be higher in older people. The purpose of this retrospective study was to find out if there are differences in the clinical

Atlanto-axial rotatory subluxation (AARS) is an uncommon condition involving dislocation and abnormal fixation of the atlas on the axis. AARS is difficult to diagnose and if improperly treated it may lead to permanent neck deformity. We report a retrospective series of patients diagnosed with AARS seen during the last five years. The children presented with neck pain, head tilt, and reduction in neck mobility. In three of the cases the condition was secondary to cervical trauma and in the fourth it was associated with otitis (Grisel syndrome). The diagnosis was established by three-dimensional

Atlanto-axial dislocations (AADs) may be classified into four varieties depending upon the direction and plane of the dislocation i.e. anteroposterior, rotatory, central, and mixed dislocations. However, from the surgical point of view these are divided into two categories i.e. reducible (RAADs) and irreducible (IAADs). Posterior fusion is the treatment of choice for RAAD. Transarticular screw fixation with sub-laminar wiring is the most stable& method of posterior fusion. Often, IAAD is due to inadequate extension in dynamic X-ray study which may also be due to spasm of muscles. If the anatom

STUDY DESIGN: A retrospective multicenter study.
OBJECTIVE: To identify the incidence of anterior arch fracture (AAF) of the atlas following C1 (first cervical vertebra) laminectomy without fusion, and its risk factors.
SUMMARY OF BACKGROUND DATA: C1 laminectomy without fusion is a widely accepted surgical procedure performed to decompress the spinal cord that is compromised at the C1/C2 level, but without instability. Several case series have reported spontaneous AAF following this procedure. However, the incidence of post-laminectomy AAF and its risk factors have not been studied.

The simple and cost-effective cable-pulley apparatus proposed by Crawford et al. (1995) has been long and widely used to assess the kinematics and biomechanics of the spine in-vitro. A major limitation of that fixed-ring system relies on the manual readjustment of the cable guides, which is required to maintain parallelism of the tension-cables during spine rotations. While several solutions have since been suggested to improve this loading approach, their implementation may be challenging for research groups with limited resources. In this study we propose an upgrade to the traditional fixed-

STUDY DESIGN: Prospective clinical study with postoperative radiologic control of pedicle screw placement in the cervical spine.
OBJECTIVES: To evaluate whether cervical pedicle screws can be placed safely in a conventional technique when using cannulated screws and separate stab incisions. Also, to evaluate if accuracy and safety of pedicle screw placement can be improved using a computer-assisted surgery (CAS) system (VectorVision; BrainLAB AG, Heimstetten, Germany).
SUMMARY OF BACKGROUND DATA: Pedicle screws are rarely used in the cervical spine compared to the use in lumbar and thoracic

BACKGROUND: We report a case of an elderly patient who was diagnosed with lateral atlantoaxial subluxation with type II odontoid fracture, an extremely uncommon upper cervical spine injury that has not been previously reported in the literature to the knowledge of the authors.
METHODS: An 87-year-old male reported to the emergency room following a syncopal episode after sustaining a fall. He complained of dizziness, fatigue, disruption of vision and audition, and worsening neck pain. Computed tomographic scans were positive of partial dislocation of the C1 relative to C2 and chronic fracture

Percutaneous laser discectomy is one common and effective treatment for cervical radicular pain. Currently, the surgery is performed with blind cannulation technique, mainly relies on the experience of surgeon. However, it still remains unsafe and difficult to reach the target. As an alternative, ultrasound-guided cannulation provides visualization of important structures, thus increasing the precision and safety. The primary goal of this study is to report the detail of the ultrasound-guided technique in the percutaneous laser cervical discectomy. The secondary purpose is to evaluate the feas

Bow hunter's syndrome is an ischemic manifestation of vertebrobasilar artery (VA) insufficiency due to stenosis or occlusion of the contralateral VA at the bony elements of the atlas and axis during neck rotation. In early reports, VA stenosis at the craniovertebral junction was the main cause, but later, symptoms due to VA occlusion at the middle and lower cervical vertebrae were also included in this pathology. Although the confirmed diagnosis is usually determined by dynamic digital subtraction angiography (DSA), we have experienced a method of minimally invasive MR angiogram (MRA) that pro
