Dizziness is a common condition that can significantly reduce quality of life and increase the risk of falls. This review examines frequent vestibular causes, including benign paroxysmal positional vertigo, vestibular migraine, Ménière's disease, and vestibular neuritis. Vestibular rehabilitation is evidence-based and enhances compensation, balance, gaze, and functional mobility in daily life through tailored exercises, that include adaptation, substitution, and habituation. Given the significant variation in implementation across Denmark, it is essential to prioritise efforts for improved qua…

Vertigo is defined as an abnormal sensation of body motion or of its surrounding objects. It is a common chief complaint in emergency departments comprising 2 to 3% of these consultations worldwide. Vertigo is classified as peripheral or central, according to its origin, and can also be occasionally mixed, the most common cause of peripheral involvement being benign paroxysmal positional vertigo. The initial findings on clinical evaluation of patients are the clues for making a correct diagnosis. The differentiation between central and peripheral vertigo can be optimized by analysing nystagmus…

OBJECTIVE: To facilitate the diagnosis of vestibular disorders by patient-initiated capture of ictal nystagmus.
METHODS: Adults from an Australian neurology outpatient clinic reporting recurrent vertigo were recruited prospectively and taught to self-record spontaneous and positional nystagmus at home while symptomatic, using miniature video-oculography goggles. Consenting patients with ictal videorecordings and a final unblinded clinical diagnosis of Ménière disease (MD), vestibular migraine (VM), or benign paroxysmal positional vertigo (BPPV) were included.
RESULTS: Ictal eye videos of 117…

INTRODUCTION AND IMPORTANCE: Benign paroxysmal positional vertigo (BPPV) is the most common cause of peripheral vertigo, typically due to dislodged otoconia within the semicircular canals. Patulous Eustachian tube (PET) is an uncommon condition characterized by an abnormally open Eustachian tube, which may alter middle ear pressure and potentially affect vestibular function.
PRESENTATION OF CASE: We report a 36-year-old female with known PET who developed sudden-onset vertigo after using a hypertonic saline rinse recommended on social media. She instilled the solution into her ear canal, whic…

PURPOSE OF REVIEW: This review covers recent advances in our understanding of migraine and childhood periodic syndromes in children and adolescents, as well as the treatment of these disorders.
RECENT FINDINGS: The childhood periodic syndromes include benign paroxysmal torticollis, benign paroxysmal vertigo, abdominal migraine, and cyclic vomiting syndrome. Recent research suggests infant colic may also fit into this category. Migraine headache is common in children and adolescents, and chronic migraine effects 0.8-1.8% of adolescents and 0.6% of children. Two triptans are now FDA-approved fo

Sleep disorders have a high comorbidity rate with vestibular diseases. The results of some small sample clinical studies have confirmed the effect of sleep deprivation on vestibular function. However, the mechanism of the effect of sleep disorder on vestibular system is still unclear. This article analyzes the possible mechanism of the effect of sleep disorders on vestibular function from the aspects of neuroanatomy and neurotransmitters, and summarizes the relationship between sleep disorder and benign paroxysmal positional vertigo, Meniere's disease, vestibular migraine and other diseases.

The dizzy patient often presents a challenge to the physician. The history is the most important component of the evaluation of the dizzy patient and often allows the cause of the dizziness to be categorized as peripheral or central. Peripheral causes include benign paroxysmal positional vertigo, Meniere's disease, and vestibular neuritis. Central causes include migraine-associated dizziness, postconcussion syndromes, cerebrovascular disease, and multiple sclerosis. Treatment depends on the cause of the dizziness and may include dietary modifications, diuretics, vestibular suppressants, vestib…

The theory of canalolithiasis assumes that benign paroxysmal vertigo and nystagmus is due to particles within the semicircular canal which move freely in the endolymph under gravitational forces, thereby deflecting its cupula. According to this theory the particles could be removed from the posterior canal by manoeuvres applying a succession of different head positions. We describe the pathophysiological mechanism of benign paroxysmal vertigo and our experience with one of these manoeuvres, the Semont manoeuvre. The success rate is impressive and the therapy is highly rewarding in a vertigo sy

Benign paroxysmal vertigo of childhood (or recurrent vertigo of childhood) is the most common cause of vertigo in young children. It is considered a pediatric migraine variant or precursor disorder, and children with the condition have an increased likelihood of developing migraine later in life than the general population. Episodes are typically associated with room-spinning vertigo in conjunction with other migrainous symptoms (e.g. pallor, nausea, etc.), but it is rarely associated with headaches. Episodes typically only last for a few minutes and occur with a frequency of days to weeks wit

OBJECTIVE: We aimed to clarify the burden of vertigo in patients' homes.
METHODS: This was a questionnaire survey among patients with vestibular vertigo. Four main questions were prepared. Q1: Where did you first notice vertigo? Q2: Where have you had the most difficulty with vertigo in your home? Q3: Where do you have difficulty at present? Q4: What household equipment have you used as a countermeasure to prevent further problems with vertigo?
RESULTS: Sixty patients completed the questionnaire. Benign paroxysmal positional vertigo (BPPV) was most common among respondents, followed by Méniè…

Symptoms such as vertigo and unsteady gait occur in various diseases and are among the relatively common chief complaints. Even at present, the mechanisms underlying these disorders are unclear. We report a significant correlation between a prolonged period of resolution of benign paroxysmal positional vertigo (BPPV) and histories of lifestyle-related illnesses. We consider the possibility of correlating between BPPV prognosis and arteriosclerotic changes. Using carotid ultrasonography, we examined maximum intima-media thickness (IMT), maximum common carotid artery IMT, and biochemical examina…

Association between vestibular function and immune inflammatory response has garnered increasing interest. Immune responses can lead to anatomical or functional alterations of the vestibular system, and inflammatory reactions may impair hearing and balance. Vestibular disorders comprise a variety of conditions, such as vestibular neuritis, benign paroxysmal positional vertigo, Meniere's disease, vestibular migraine, posterior circulation ischemia, and bilateral vestibular disease. Moreover, some patients with autoimmune diseases develop vestibulocochlear symptom. This paper offers an overview …

In case of non-self regression of Benign Paroxysmal Positional Vertigo (BPPV), reeducation is mandatory. Two reeducation protocols have been compared on 32 subjects with BPPV. Fourteen were handled by the classic habituation exercises, and 18 by the more recent Semont's manoeuvre. Moreover a specific rotatory nystagmus habituation has been investigated by caloric stimulation in Brunning III position. Semont's technique shows quicker and more efficient results. This manoeuvre doesn't provoke any habituation, in accordance with the hypothesis of mechanical dispersion of otoconia, based on the cu…

PURPOSE: This article reviews the most common conditions that are caused by changes in head or body positions. Practical clinical methods to help distinguish vestibular from nonvestibular and central from peripheral vestibular positional dizziness are discussed. This article also reviews the treatment methods of selected canal variants of benign paroxysmal positional vertigo (BPPV).
RECENT FINDINGS: Two recent evidence-based guidelines have established canalith repositioning maneuvers (Epley and Semont maneuvers) as safe and highly effective in the treatment of posterior canal BPPV. Recent st…

A decision tree is an artificial intelligence program that is adaptive and is closely related to a neural network, but can handle missing or nondecisive data in decision-making. Data on patients with Meniere's disease, vestibular schwannoma, traumatic vertigo, sudden deafness, benign paroxysmal positional vertigo, and vestibular neuritis were retrieved from the database of the otoneurologic expert system ONE for the development and testing of the accuracy of decision trees in the diagnostic workup. Decision trees were constructed separately for each disease. The accuracies of the best decision…
