Cephalexin levels in human bile in presence of biliary tract disease.

Cephalexin was given to 24 patients before and after operation on the bile ducts and gall bladder. Two patients had obstructive jaundice. Samples of the bile were taken either directly from the gall bladder at operation or via the T-tube. Cephalexin was excreted in the bile, peak levels being obtained after two to three hours. These levels could be raised if probenecid was given concurrently. Higher levels were found in patients with functioning gall-bladders. A trial of cephalexin seems justified for the treatment of typhoid carriers.

Biliary tract anomalies: the utilization of modern techniques in differential diagnosis.

The diagnostic aids used in dealing with biliary disease in adults were applied to the study in infants of the principal congenital anomalies of the biliary tract such as choledochal cyst, biliary atresia and biliary stenosis. Choledochal cysts were distinguished from other upper abdominal masses occurring in childhood by the use of intravenous cholecystography. Since the clinical manifestations in infants with biliary atresia or stenosis are almost identical to those associated with the obstructive phase of neonatal hepatitis, the problem of differentiation is difficult. The serial total seru…

Association between Helicobacter bilis in bile and biliary tract malignancies: H. bilis in bile from Japanese and Thai patients with benign and malignant diseases in the biliary tract.

Japan and Thailand have high incidences of bile duct carcinoma and gallstones. The presence of Helicobacter bilis (H. bilis) detected by polymerase chain reaction (PCR) and 16S rRNA analysis in bile samples from Chileans with chronic cholecystitis was reported. The association between H. bilis in bile and biliary tract malignancies has not been investigated, and therefore the aim of this study is to determine whether malignant diseases of the biliary tract are associated with the presence of H. bilis in bile samples obtained from two high-risk populations. Bile samples from 45 Japanese and 40 …

Two cases of durvalumab-induced Evans syndrome in biliary tract cancer.

We present two cases of Evans syndrome (ES) during durvalumab monotherapy after combination chemotherapy with durvalumab for advanced hilar cholangiocarcinoma. The patients were men aged 71 and 73 years with satisfactory performance status. They received eight cycles of gemcitabine, cisplatin, and durvalumab, followed by durvalumab monotherapy (five and six cycles, respectively) for a total of approximately 11 months. Subsequently, they developed autoimmune hemolytic anemia, characterized by reticulocytosis, indirect hyperbilirubinemia, elevated lactate dehydrogenase levels, and a positive dir

Adipose tissue and survival among patients with advanced biliary tract cancer.

BACKGROUND: Adiposity may promote the progression of different types of cancer. The prognostic significance of visceral and subcutaneous adipose tissue has not been thoroughly investigated in patients with advanced biliary tract cancer. METHODS: Using a single-institutional series of 386 patients with unresectable or recurrent biliary tract cancer, we measured visceral and subcutaneous adipose tissue areas (both normalized by body height) based on abdominal computed tomography images. Using the Cox regression model with adjustment for potential confounders, we calculated hazard ratios (HRs) a…

Epidemiology, biomarkers, and therapeutic approach in early-stage biliary tract cancers.

Biliary tract cancer (BTC) represents a heterogeneous group of malignancies, including intrahepatic, perihilar, and distal cholangiocarcinoma, as well as gallbladder cancer. Surgical resection is the only potential curative treatment for early-stage BTC, though outcomes with surgery alone were poor, with high rates of recurrence. In recent years, studies of adjuvant chemotherapy with capecitabine or S-1 have shown improved survival compared to postoperative observation alone. Ongoing clinical trials are incorporating multiple treatment modalities, including radiotherapy, immunotherapy, and tar…

Immunotherapy plus chemotherapy in advanced biliary tract cancers: pros and cons.

INTRODUCTION: Biliary tract cancers (BTC) are highly immunosuppressive 'cold' tumors with limited treatment options. In unresectable or advanced-stage disease, the combination of immune checkpoint inhibition with chemotherapy resulted in a modest survival benefit over chemotherapy. AREAS COVERED: In this review, current obstacles and strategies to improve the benefit of immunotherapy are highlighted. This includes challenges in optimizing patient selection, understanding the tumor microenvironment and interactions with the gut microbiome, and improving combination treatment strategies, whethe…

Biliary tract neoplasms from common to rare: diagnosis, assessment, and management.

Biliary tract neoplasms include a diverse set of benign and malignant entities that share both clinical presentations and imaging features. Radiologists need to understand the diagnostic features of these neoplasms to recognize them at initial diagnosis, and we must also provide key information to our surgical and oncology colleagues to facilitate proper management. This work reviews the most important intrahepatic and extrahepatic biliary neoplasms with an emphasis on radiological findings and differentiating features. This work also reviews normal anatomy and key variants of the biliary tree…

Essential Updates 2023/2024: Minimally Invasive Surgery for Biliary Tract Cancer.

Minimally invasive surgery (MIS) for biliary tract cancer (BTC) has gained traction with advancements in laparoscopic and robotic techniques. However, its oncological impact remains uncertain. This review highlights key studies from 2023 and 2024 on MIS for BTC, including distal bile duct, hilar bile duct, gallbladder, and intrahepatic bile duct cancer. Although feasibility and safety are supported in experienced centers, long-term prognostic equivalence to open surgery remains debatable. Further studies are required to clarify surgical and oncological outcomes.

Recurrent hemobilia secondary to extrahepatic biliary tract cholangiocarcinoma. A diagnostic challenge.

The etiology of hemobilia has mainly iatrogenic (>50%), followed by traumatic causes. Others are biliopathy due to portal high pressure, or neoplastic or infective biliopathy. In the case of non-clear hemobilia, direct-vision-cholangioscopy can change the management in >34% of cases. Our patient had episodes of obstructive hemobilia with secondary cholangitis without objectifying underlying pathology. When she was referred to our center, SpyGlass®-cholangioscopy identified the suspicious lesion compatible with early-stage cholangiocarcinoma despite the diagnostic delay. In conclusion, it is im

Genomics driven precision oncology in advanced biliary tract cancer improves survival.

BACKGROUND: Biliary tract cancers (BTCs) including intrahepatic, perihilar, and distal cholangiocarcinoma as well as gallbladder cancer, are rare but aggressive malignancies with few effective standard of care therapies. METHODS: We implemented integrative clinical sequencing of advanced BTC tumors from 124 consecutive patients who progressed on standard therapies (N=92 with MI-ONCOSEQ and N=32 with commercial gene panels) enrolled between 2011-2020. RESULTS: Genomic profiling of paired tumor and normal DNA and tumor transcriptome (RNA) sequencing identified actionable somatic and germline g…
