An Eight Year Clinico-Microbiological Retrospective Study on Brain Abscesses in India.

BACKGROUND: Intracranial abscesses have been a diagnostic and therapeutic challenge since time immemorial for both the microbiologists and the neurosurgeons. There is paucity of detailed studies documenting the infecting organism causing brain abscesses in South India. AIMS: The study aimed at identifying and assessing the prevalence of aerobic, anaerobic bacteria and fungi associated with brain abscesses at a tertiary care hospital in South India. METHODS AND MATERIAL: Eight years data was collected from the records of culture reports from 2007 to 2010 and 2013 to 2018. The corresponding cl…

Multifocal brain abscesses caused by invasive Streptococcus intermedia: A case report.

Multifocal brain abscesses caused by invasive Streptococcus intermedia are relatively rare. Here, we present a 67-year-old male was admitted to the hospital for unconsciousness and fever. The computed tomography (CT) examination showed multiple intracranial space-occupying and "cavity-like" changes in the right lower lung. The examination of cerebrospinal fluid (CSF) was consistent with typical bacterial meningitis, CSF analyses revealed leukocytosis (10,300 × 106/L), elevated protein levels (140.39 mg/dL), decreased glucose levels (0.27 mmol/L), and normal chloride concentration level (120.2 …

Multiple Brain Abscesses Caused by Nocardia Beijingensis in a Patient With HIV Infection.

The etiologic agents of central nervous system infections in HIV-infected patients comprise a broad range of opportunistic pathogens. We presented a 49-year-old male patient with HIV infection and low adherence to antiretroviral therapy. He presented with multiple cerebral abscesses, and his microbiological diagnosis approach resulted in the isolation of Nocardia beijingensis , a species rarely reported in America. Central nervous system nocardial infection in HIV-infected patients should be considered, and a diagnosis at species level is mandatory because the antibiotic susceptibility profile

Surgical management of a brain abscess due to plant foreign body in a dog.

BACKGROUND: Intracranial abscesses as a result of grass awn migration have been rarely described in the veterinary literature. The identification of their radiological features is mandatory for proper diagnosis. As occurs with abscesses in other organs, surgical drainage and directed antibiotic therapy should be considered the treatment of choice. CASE DESCRIPTION: A clinical case of a Great Dane dog with forebrain signs and magnetic resonance imaging and computed tomography findings compatible with intracranial abscess associated with inflammatory changes in orbital musculature is described.

Bacillus cereus bacteraemia complicated by a brain abscess in a pre-term neonate.

Bacillus cereus is a common laboratory and environmental contaminant. Reports of severe infections are mainly limited to immunocompromised individuals. In reported cases, the time interval between bacteraemia and neuro-invasion appears to be very short, highlighting the importance of rapid and definitive identification and susceptibility testing of invasive B. cereus. We report a case of a neonatal B. cereus bacteraemia complicated by a brain abscess from a neonatal intensive care unit. The neonate presented with bradycardia and desaturations with increased oxygen requirements. Initial blood c…

Aspergillus Terreus Brain Abscess Complicated by Tension Pneumocephalus in a Patient with Angiosarcoma.

BACKGROUND Aspergillus terreus is an evolving opportunistic pathogen, and patients with A. terreus often have poor outcomes due to its intrinsic resistance to several systemic antifungal agents. Here we present a unique case of intracranial abscesses of A. terreus in a patient with recurrent angiosarcoma, complicated by development of tension pneumocephalus. CASE REPORT A 67-year old gentleman with history of scalp angiosarcoma with wide excision two years prior presented to the hospital for left arm clumsiness, altered mental status, and low-grade fever. Staphylococcus aureus and Proteus mira

Multiple cortical brain abscesses due to Listeria monocytogenes in an immunocompetent patient.

Listeria monocytogenes is an intracellular organism which is well recognised for its ability to cause meningeal infections in neonates, immunosuppressed, debilitated and elderly individuals. 1 Other less common central nervous system (CNS) infections caused by Listeria spp. include rhomboencephalitis, cerebritis and abscesses in the brain, brain stem and spinal cord. The neuroradiological appearance of Listeria brain abscesses is similar to other types and may also mimic primary or metastatic brain tumours. 2 , 3 We report a case of Listeria brain abscesses in a patient who was being treated f…

Prevotella brain abscess in a healthy young patient with a patent foramen ovale.

Brain abscesses are frequently caused by poly-microbial conditions. Comparatively, brain abscesses caused by Prevotella species are very rare. Right-to-left cardiac shunting due to a patent foramen ovale may predispose patients to infection. We report an isolated Prevotella brain abscess that occurred in a healthy, young, male patient with a patent foramen ovale. The patient did not have a clinically obvious odontogenic source of infection, and no other distant extracranial infectious sources were observed. The patient was successfully treated with stereotactic aspiration and antibiotics.

Aggregatibacter aphrophilus misidentified as Brucella spp. in an immunocompetent patient with brain abscess.

OBJECTIVE: We report the clinicopathologic features of a solitary fibrous tumor (SFT) having undergone malignant transformation and being intimately associated with a WHO Grade II astrocytoma. CLINICAL PRESENTATION: A 7-month old patient presented with delayed motor development and hydrocephalus. INTERVENTION: Histologic and immunocytologic methods were applied in the study of the tumors. Resection was initially employed and the SIOP protocol employing vincristine and carboplatin was applied upon tumor recurrence. CONCLUSION: The biologic basis for the association of SFT and astrocytoma is

[OPTIMIZATION OF THE TREATMENT TACTICS IN BRAIN ABSCESS, USING THE PUNCTURE STEREOTAXIC ASPIRATION].

There was analyzed the application efficacy of stereotaxic aspiration of the brain abscess (BA) content, localized in a deep and functionally significant parts of the brain, in 12 patients--with supratentorial BA. Stereotaxic interventions were performed, using stereotaxic systems CRW Radionics (USA) and the security program for calculation of coordynates StereoFusion, StereoPlan (Radionics), Fraime Link (Medtronic, USA). Postoperative observation period have lasted from 1 to 36 mo, (14 ± 3.6) mo at average. lntraoperatively in 11 (91.7%) patients a purulent content of BA was evacuated, and in…

Histoplasmosis brain abscesses in an immunocompetent adult. A case report and literature review.

We describe the case of a 62-year-old man, who presented with a new onset of focal seizures of his right leg. There were no other clinical symptoms, and laboratory results were normal. Brain magnetic resonance imaging revealed multiple lesions, two supratentorial lesions were ring-enhancing. The brain biopsy tissue showed Histoplasma capsulatum abscesses. He improved on treatment with Amphotericin B. This case is reported since cerebral ring-enhancing lesions are rarely associated with histoplasmosis, which is also rare in an immunocompetent individual. We review the literature and discuss the

[Persistent fever syndrome secondary to brain abscesses: a rare complication of the meningococcemia].

Meningococcal disease is caused by hematogenous spreading of Neisseria meningitidis. Meningococcal sepsis occurs in around 20% of cases of meningococcal disease, meningitis in 50% and, to a lesser extent, other conditions such as pneumonia, arthritis, urethritis, conjunctivitis or pericarditis. Brain abscesses are a rare complication.

Intracranial calcification caused by a brain abscess : a rare cause of intracranial calcification.

Intracranial calcifications are relatively common computed tomographic findings in the field of neurosurgery, and cysticercosis, tuberculosis, HIV, and cryptococcus are acquired intracranial infections typically associated with calcifications. However, intracranial calcification caused by a bacterial brain abscess is rare. Here, we present a rare case of intracranial calcification caused by a bacterial brain abscess, from which staphylococcus hominis was isolated. To the best of our knowledge, no previous report has been published on intracranial calcification caused by bacterial brain abscess…

Brain Abscess Caused by Enterococcus avium: A Case Report and Review of the Literature.

Brain abscesses can be highly lethal if appropriate treatment is not administered, and reports of such an abscess caused by Enterococcus avium are very rare. Here,we report a case of a 48-year-old man presenting with chronic otitis media. He initially presented with a headache and right otalgia. An otoscopic evaluation performed on the day of admission showed exudation of fresh pus from the right ear. Brain magnetic resonance imaging demonstrated a hypodense area in the right temporoparietal lobe, suggestive of a brain abscess. A culturing of the ear discharge and brain abscess aspirate proved…

Madurella mycetomatis as an agent of brain abscess: case report and review of literature.

Fungal cerebral abscesses are rare and usually seen in immunocompromised individuals. We report a case and review published literature of Madurella mycetomatis as an agent of cerebral abscess. We found contiguous head and neck infections to be the principal cause of cerebral maduromycosis caused by M. mycetomatis. Early recognition of Madurella spp. as the causative agent is essential to avoid cerebral spread.

[Brain abscess due to Fusobacterium necrophorum in a patient with convulsion and no signs of meningitis].

Here, we report brain abscess due to Fusobacterium necrophorum (F. necrophorum) in a 78-year-old healthy man. He developed convulsion and did not have any signs of meningitis. Although the brain magnetic resonance imaging findings of the left occipital lobe were typical of a brain abscess, his cerebrospinal fluid examination revealed only slight pleocytosis and mild increase in protein levels. Thus, it was difficult to rule out the possibility of metastatic brain tumor; the patient's condition was provisionally diagnosed as symptomatic epilepsy secondary to brain abscess. His convulsion disapp…

A brain abscess of probable odontogenic origin in a child with cyanotic heart disease.

The purpose of the present report was to present a rare case of a brain abscess in a child with heterotaxy syndrome, severe cardiac anomalies, and extensive dental caries. The pathogen was Streptococcus intermedius isolated from the cerebrospinal fluid. The source of the pathogen was probably an infection of a primary molar with a dentoalveolar abscess involving the bud of the permanent successor. After a long course of antibiotic regimens followed by a craniotomy with abscess drainage, a shunt, and comprehensive dental treatment, the patient was discharged from the hospital without any neurol…

[Successful treatment of brain stem and thalamic abscesses with high-dose meropenem].

The incidence of brain abscess remains high, despite the development of novel antibiotics. Vancomycin or carbapenems, which are third-generation cephems, are recommended as standard therapy for bacterial meningitis or brain abscess. The effectiveness of the high-dose meropenem therapy on brain abscess has occasionally been reported. We experienced 2 consecutive cases of brain abscess in adults. The first patient was a 67-year-old man with diplopia, dizziness, and dysesthesia on the left upper and lower extremities. Images of T1-weighted magnetic resonance imaging (MRI) with contrast medium and…
